Validation of a Novel Automated Algorithm to Measure Drusen Volume and Area Using Swept Source Optical Coherence Tomography Angiography.

PURPOSE: The purpose of this study was to validate a novel automated swept source optical coherence tomography angiography (SS-OCTA) algorithm to measure elevations of the retinal pigment epithelium (RPE) in eyes with nonexudative age-related macular degeneration (neAMD). METHODS: Patients with drusen were enrolled in a prospective optical coherence tomography (OCT) study and underwent both spectral domain OCT (SD-OCT) and SS-OCTA imaging at the same visit using the 6 × 6 mm scan patterns. The RPE elevation measurements (square root area and cube root volume) from the SS-OCTA algorithm were c

[Spot-like to reticular pigment displacement in a patient with pseudoxanthoma elasticum (Grönblad-Strandberg syndrome)].

The authors report a rare observation of leopard spot-like and reticular pigmentary patterns in a patient with pseudoxanthoma elasticum (PXE). Characteristic fundus changes associated with PXE have been described as angioid streaks, subretinal neovascularization membranes, peau d'orange, focal retinal pigment epithelium atrophy (salmon spots), drusen, and optic nerve head drusen. The new manifestation of PXE in the fundus of the patient described here is a random scattering of small, dark brown pigment dots throughout the macula and around the disk. These clumps may flow into one another or jo

Evaluation of Convolutional Neural Networks (CNNs) in Identifying Retinal Conditions Through Classification of Optical Coherence Tomography (OCT) Images.

Introduction Diabetic retinopathy (DR) is a leading cause of blindness globally, emphasizing the urgent need for efficient diagnostic tools. Machine learning, particularly convolutional neural networks (CNNs), has shown promise in automating the diagnosis of retinal conditions with high accuracy. This study evaluates two CNN models, VGG16 and InceptionV3, for classifying retinal optical coherence tomography (OCT) images into four categories: normal, choroidal neovascularization, diabetic macular edema (DME), and drusen. Methods Using 83,000 OCT images across four categories, the CNNs were trai

Age-Related Macular Degeneration, a Mathematically Tractable Disease.

A progression sequence for age-related macular degeneration onset may be determinable with consensus neuroanatomical nomenclature augmented by drusen biology and eye-tracked clinical imaging. This narrative review proposes to supplement the Early Treatment of Diabetic Retinopathy Study (sETDRS) grid with a ring to capture high rod densities. Published photoreceptor and retinal pigment epithelium (RPE) densities in flat mounted aged-normal donor eyes were recomputed for sETDRS rings including near-periphery rich in rods and cumulatively for circular fovea-centered regions. Literature was review

Understanding age-related macular degeneration (AMD): relationships between the photoreceptor/retinal pigment epithelium/Bruch's membrane/choriocapillaris complex.

There is a mutualistic symbiotic relationship between the components of the photoreceptor/retinal pigment epithelium (RPE)/Bruch's membrane (BrMb)/choriocapillaris (CC) complex that is lost in AMD. Which component in the photoreceptor/RPE/BrMb/CC complex is affected first appears to depend on the type of AMD. In atrophic AMD (~85-90% of cases), it appears that large confluent drusen formation and hyperpigmentation (presumably dysfunction in RPE) are the initial insult and the resorption of these drusen and loss of RPE (hypopigmentation) can be predictive for progression of geographic atrophy (

[Age-related Macular Degeneration in the Japanese].

Age-related macular degeneration (AMD) in the Japanese often shows different clinical features from those described in Caucasians. For example, we often observe choroidal neovascularization (CNV) in elderly patients without drusen in the fundus. The high incidence of polypoidal choroidal vasculopathy (PCV) in AMD among Japanese is well-known. The reason why such differences occur in clinical manifestations of AMD has been one of my main interests. In this review article, I will discuss the characteristics of AMD in the Japanese population, as found in our recent study. I. Prevalence and clinic

The dynamic nature of Bruch's membrane.

Bruch's membrane (BM) is a unique pentalaminar structure, which is strategically located between the retinal pigment epithelium (RPE) and the fenestrated choroidal capillaries of the eye. BM is an elastin- and collagen-rich extracellular matrix that acts as a molecular sieve. BM partly regulates the reciprocal exchange of biomolecules, nutrients, oxygen, fluids and metabolic waste products between the retina and the general circulation. Accumulating evidence suggests that the molecular, structural and functional properties of BM are dependent on age, genetic constitution, environmental factors

The Retinal Complications of C3 Dense Deposit Disease: A Scoping Review.

People with C3 Dense Deposit Disease (C3DDD), a rare autoimmune disease, often also have ocular complications. Due to the rarity of this disease, there is little known about ocular complications in populations across the world. This paper aimed to assess literature on retinal complications in people with C3 Dense Deposit Disease. A scoping review was conducted and three databases (Embase, Medline All, and Web of Science) were searched using agreed search terms and Boolean operators. All references were imported into Covidence for screening by two reviewers. Any conflicts were resolved by a thi

Exacerbation of senile macular degeneration following cataract extraction.

Six eyes in four patients had hemorrhagic detachment of the retinal pigment epithelium following cataract extraction. All eyes had evidence of nonexudative senile macular degeneration preoperatively as manifested by drusen or alterations in the retinal pigment epithelium, or both. Postoperatively, all eyes had evidence of a subretinal neovascular membrane manifested by the presence of subretinal hemorrhage or by evidence on fluorescein anglography. We believe cataract extraction may cause changes in the choroidal hemodynamics that allow pre-existing new blood vessels to leak or bleed, or induc

Updated Guidelines for Imaging the Choriocapillaris in Eyes with Age-Related Macular Degeneration Using Swept-Source Optical Coherence Tomography Angiography.

PURPOSE: To update the recommended guidelines when quantifying choriocapillaris (CC) flow deficits (FDs) in eyes with age-related macular degeneration (AMD) using swept-source optical coherence tomography angiography (SS-OCTA). DESIGN: Evidence-based perspective. METHODS: Review of literature and experience of authors. RESULTS: A current challenge when quantifying CC FDs using SS-OCTA is the implementation of an objective compensation strategy to adjust for the signal attenuation arising under drusen in eyes with AMD. Our previous compensation strategy was used as a general approach to adju

Interactions between Apolipoprotein E Metabolism and Retinal Inflammation in Age-Related Macular Degeneration.

Age-related macular degeneration (AMD) is a multifactorial retinal disorder that is a major global cause of severe visual impairment. The development of an effective therapy to treat geographic atrophy, the predominant form of AMD, remains elusive due to the incomplete understanding of its pathogenesis. Central to AMD diagnosis and pathology are the hallmark lipid and proteinaceous deposits, drusen and reticular pseudodrusen, that accumulate in the subretinal pigment epithelium and subretinal spaces, respectively. Age-related changes and environmental stressors, such as smoking and a high-fat
