Severe rhinophyma treated by shave excision and electrocautery.

Rhinophyma is a rare disease, causing disfiguring soft-tissue hypertrophy of the nose. Various medical and surgical treatment modalities have been tried, with limited success. We report a case of a 76-year-old man, where shave excision of the rhinophymatous tissue was done with satisfactory cosmetic outcome.

Surgical treatment of rhinophyma using an ordinary bur.

Rhinophyma is a painless benign tumor of the skin of the nose. It is the most severe form of rosacea. Rhinophyma is particularly rare among Asians. It can be treated with surgical resection using various methods, including scalpel excision, dermabrasion, cryosurgery, argon laser, carbon dioxide laser, and electrocautery. However, the gold standard treatment of rhinophyma remains unknown. In the present case, we debulked a giant rhinophyma with a scalpel and contoured it using a bur. Along with a relevant literature review, we present a case of rhinophyma in an Asian patient who was treated at …

Rhinophyma: Combined Surgical Treatment and Quality of Life.

Rhinophyma is a progressive and disfiguring thickening of the nasal skin. It is typically found in middle-aged or elderly Caucasian males. The exact cause of the disease is unknown. There is excessive formation of scar-like tissue and hyperplasia of the sebaceous glands. This article will demonstrate our experience of treating rhinophyma using a combined surgical approach and propose a novel classification system. This is a retrospective analysis of cases operated by a single surgeon over 9 years using the Glasgow Benefit Inventory (GBI) tool. Thirty-three patients were identified. Twenty-nine…

[A systematic review and current recommendation for treatment of rhinophyma].

Rhinophyma is a benign disease involving the skin of the nose, which is characterized by skin thickening and hypertrophy of the sebaceous glands and connective tissue. It occurs predominantly by Caucasians in their fifth to seventh life decades and is 12-30 times more likely to affect males. The etiology and pathogenesis of Rhinophyma remains unclear, however it is generally stated to be the final stage of rosacea. A causative relationship between rhinophyma and alcohol consumption has not yet been proven. This review highlights the treatment options of rhinophyma. Surgery is gold standard for…

Dermabrasion for Scars and Wire Loop Electrocautery for Rhinophyma.

Dermabrasion and wire loop electrocautery are controlled scalpel-less procedures to remove superficial skin layers to treat dermal surface irregularities. Their postprocedure healing involves healing by secondary intention. The purpose of this paper is to discuss the use of diamond fraise dermabrasion to improve scars and the use of wire loop electrocautery to treat rhinophyma surface irregularities. Both techniques are minimally invasive and low cost, and at the same time, can significantly improve facial skin deformities. An advantage in using wire loop electrocautery for rhinophyma excision…

Large Rhinophyma Treated by Surgical Excision and Electrocautery.

Rhinophyma is a benign condition characterized by a large, bulbous nose with prominent pores. It is commonly associated with untreated cases of rosacea. The disease can carry a substantial psychological impact that causes patients to seek advice about how to improve their physical appearance. Many treatment options are available for rhinophyma, but there is no standard treatment protocol. Here, we describe the case of a 65-year-old man with a large rhinophyma that caused him cosmetic and psychosocial embarrassment. The condition was treated by surgical excision and bipolar electrocautery. No c…

Management of severe rhinophyma with sculpting surgical decortication.

UNLABELLED: Rhinophyma, a progressive, disfiguring disease of the nose, is considered the final stage of acne rosacea. In its early stages, rhinophyma can be managed with medical treatment using isotretinoin or oral antibiotics (metronidazole). However, severe cases usually are refractory to medical approaches. Surgical therapies to treat these severe refractory cases have been described. This report describes a simple, safe, efficient, and cost-effective approach to the treatment of severe rhinophyma using a scalpel and the electroscalpel, instruments readily available in every operating room…

Further evidence for the role of fibrosis in the pathobiology of rhinophyma.

Recent evidence suggests that fibrosis may play an important role in the pathobiology of rhinophyma. The fibrogenic cytokine transforming growth factor (TGF)-beta2 has been reported to be up-regulated in rhinophyma tissue. Of the three common isoforms of TGF-beta, TGF-beta1 and TGF-beta2 are considered fibrogenic, whereas TGF-beta3 has antiscarring properties. To provide further evidence for the role of fibrosis in the pathobiology of rhinophyma, specimens from 8 patients with rhinophyma were compared with nine specimens of normal nasal skin. Immunohistochemistry was used to compare intensity …

Conventional cold excision combined with dermabrasion for rhinophyma.

A 65-year-old man, farmer by occupation, presented with redness and gradual enlargement of the nose. Examination revealed marked nodular enlargement of the nose and loss of normal nasal contours. Sebaceous material could be expressed from widened pores. The patient was diagnosed as rhinophyma of moderate degree. He was treated with cold knife excision combined with dermabrasion. A literature scan revealed that currently there is no evidence of superiority of much popular laser surgery over conventional cold knife surgery combined with dermabrasion for rhinophyma. Conventional surgery is time-t…

Treatment of massive rhinophyma with the carbon dioxide laser.

A case of massive rhinophyma that produced significant functional and cosmetic difficulties was treated using the carbon dioxide laser. Minimal bleeding occurred during the operative procedure despite the grotesque enlargement and high degree of vascularity of the skin and soft tissue. Using this technique, restoration of normal function and excellent cosmesis was possible without complication.
