Myocardial contusion and rib fracture repair in an adult horse.

OBJECTIVE: To describe the clinical findings and management of myocardial injury secondary to blunt thoracic trauma and rib fracture in an adult horse. CASE SUMMARY: A 6-year-old Warmblood gelding presented for treatment of blunt thoracic trauma. Sonographic examination of the thorax revealed a complete, mildly comminuted fracture of the left 5th rib with a fragment overlying the left atrium and coronary artery, hemothorax, and subjective left ventricular dyskinesis. Evidence of myocardial injury included atrial fibrillation, ventricular ectopy, and increased plasma cardiac troponin I concent

[MSCT optimal examination time for unconspicuous rib fracture].

OBJECTIVE: To explore MSCT optimal examination time for patients with unconspicuous rib fracture. METHODS: Sixty-three patients with thoracic trauma from January 2009 to June 2011 were collected. They were examined by MSCT in the first week after trauma and re-examined during eighth weeks after trauma. The number of rib which had been found fractured in the first examination was compared to that in re-examinations. RESULTS: Patients with fine rib fracture often have different diagnostic results at different examination time after trauma. There was statistical difference between the number in

Rib fracture fixation for flail chest: what is the benefit?

BACKGROUND: Recently, rib fracture fixation for flail chest has been used increasingly at both academic and nonacademic trauma centers. Although a few small non-US studies have demonstrated a clinical benefit, it is unclear whether this benefit outweighs the added expense and potential perioperative complications related to the procedure. We therefore sought to determine if open reduction and internal fixation of ribs for flail chest (ORIF-FC) represents a cost-effective means for managing these patients. STUDY DESIGN: A Markov transition state analysis was performed modeling the outcomes of

Isolated fracture of the first rib associated with facial trauma.

Isolated fractures of the first rib are rare and often, though not always, indicative of severe trauma. The causes of first rib fracture are various, as are the sometimes serious complications of such a fracture. Since the oral and maxillofacial surgeon is involved many times in the primary care of the trauma patient, he must consider the possibility of first rib fracture and be alerted to the possible sequelae.

Detection of Costal Cartilage Fractures on CT Images With Computer-aided Detection System for Rib Fractures.

BACKGROUND/AIM: Costal cartilage fractures are associated with poor prognosis in patients with blunt chest trauma. A Computer-Aided Detection (CAD) system for detecting rib fractures has been used in practice, but it is unclear whether this system recognizes costal cartilage fractures. This study investigated whether the CAD system for rib fracture can detect costal cartilage fractures. PATIENTS AND METHODS: A total of 89 patients with costal cartilage fractures from participating hospitals over an 18-year period were included in the study. The presence of a costal cartilage fracture was dete

Surgical Stabilization of Rib Fractures in Severe Polytrauma: A Potential Indication.

BackgroundMost studies demonstrating efficacy of surgical stabilization of rib fractures (SSRF) are in patients with isolated severe chest wall injury. Recent evidence suggests SSRF may reduce mortality in polytrauma patients. The present study examines SSRF outcomes in severe polytrauma patients.MethodsThe 2013-2021 Trauma Quality Improvement Project database was used to identify severe polytrauma patients, defined as Injury Severity Score (ISS) ≥15 and abbreviated injury scale (AIS) ≥2 in 2 or more regions, with rib fractures. Exclusion criteria included AIS 6 in any region, death ≤72 hours,

Ultrasound-guided percutaneous cryoneurolysis of intercostal nerves in traumatic rib fractures.

BACKGROUND: Multimodal pain control is the cornerstone of managing acute traumatic rib fractures. We employed surgeon-administered, ultrasound-guided percutaneous cryoneurolysis of intercostal nerves (USPCNIN) as an adjunct opioid-sparing analgesic modality at the bedside. METHODS: This was a single-institution case series. Patients between 18-64 years of age who sustained traumatic rib fracture between ribs 3-9, deemed ineligible for surgical stabilization, and had pre-procedure numeric pain scores ≥5 underwent USPCNIN within 24 h of study enrollment by an attending chest wall surgeon. Prima

One-year patient-reported outcomes following surgical stabilization of rib fractures.

PURPOSE: Rib fractures are a common injury in trauma patients. Studies suggest surgical stabilization of rib fractures can improve prolonged ventilation and pain; however, there are limited studies reporting outcomes beyond 90-day post-injury. This study evaluates patient-reported outcomes for patients who underwent surgical stabilization of rib fractures (SSRF) over a one-year period. METHODS: Patients who underwent SSRF from January 2019 to December 2023 were analyzed. Single Assessment Numeric Evaluation of improvement (SANE) and the Patient-Reported Outcomes Measurement Information System

Impact of obesity on outcomes of surgical stabilization of rib fractures.

INTRODUCTION: Blunt trauma patients (BTPs) frequently present with rib fractures. Surgical stabilization of rib fractures (SSRF) has shown to improve outcomes by reducing complications. Given the increasing prevalence of obesity, understanding its influence on surgical outcomes after SSRF is warranted. OBJECTIVE: We aimed to evaluate the impact of obesity on SSRF outcomes in BTPs with rib fractures, hypothesizing that obese patients would have an increased associated risk of respiratory complications. METHODS: Trauma Quality Improvement Program database (2017-2022) was queried for BTPs 18 ye

Intrathoracic surgical stabilization of rib fractures: What you need to know.

Rib fractures remain among the most common injuries in trauma patients. The past two decades have seen a sharp increase in the scientific study of chest wall injury and the development of multiple dedicated rib stabilization systems, including some that are deployable intrathoracically. Beyond repair of fractured ribs, chest wall injury also includes management of other intrathoracic pathologies that can make the addition of video-assisted thoracoscopy to chest wall surgery prudent. Indications for rib fracture surgery have been published by numerous surgical societies despite ongoing debate t

Rib Fractures in Trauma Patients: Prevalence, Associated Injuries, and Mortality Trends.

Introduction Rib fractures are a common consequence of thoracic trauma, often signaling the presence of additional injuries and increasing the risk of mortality. This study aimed to elucidate the frequency, patterns, and mortality rates associated with rib fractures in multitrauma patients and to explore the association between rib fractures and other systemic injuries. Methods In a retrospective observational study at a tertiary care hospital, electronic medical records of multi-trauma patients over a four-year period were examined. Inclusion criteria encompassed adult patients who had underg

Age-Neutral Surgical Stabilization of Rib Fractures: Breaking the Ageism Barrier.

INTRODUCTION: Surgical stabilization of rib fractures (SSRF) is associated with lower rates of mortality and fewer complications. This study evaluates whether the decision to undergo SSRF is associated with age, race, ethnicity, and insurance status and assesses associated clinical outcomes. METHODS: This retrospective analysis included patients ≥45 y old with rib fractures who underwent SSRF in the Trauma Quality Improvement Program from 2016 to 2020. Race, ethnicity, and insurance statuses were collected. Age in years was dichotomized into two groups: 45-64 and 65+. Outcomes included ventil

Long-term functional recovery after rib fractures: The impact of frailty.

BACKGROUND: Previous studies have shown that patients with rib fractures experience long-term functional limitations. However, the specific predictors of these worse long-term functional limitations remain under-characterized. METHODS: We conducted a prospective cohort study including patients ≥18 years with an injury severity score ≥9 and isolated chest injury. Patients included had ≥1 rib fracture and were admitted between July 2015 and May 2019 at one of three Level I trauma centers present in our region. We performed stepwise regression analysis to identify predictors of new functional li

Delayed Tension Hemothorax With Nondisplaced Rib Fractures After Blunt Thoracic Trauma.

Blunt thoracic trauma often causes rib fractures, hemothorax, and pneumothorax. Although there is no established definition regarding the duration and management of delayed hemothorax, it commonly occurs in a few days and exhibits at least one displaced rib fracture. Moreover, delayed hemothorax rarely develops tension hemothorax. A 58-year-old male who had a motorcycle accident received conservative treatment from his orthopedic doctor. He felt a sudden severe chest pain 19 days after the accident. Contrast-enhanced computed tomography (CT) of the chest revealed multiple left-sided rib fractu
