Between 1975 and 1981 7 cases of rupture of the uterus were observed at this department with an incidence of 1:1200. In all cases therapy consisted of hysterectomy: There was no maternal death, one child died within 24 hours after delivery. Rupture of the uterus remains a serious complication. Careful attention to risk factors, the avoidance of traumatizing obstetric operations and the judicious use of oxytocics should further reduce the incidence of rupture of the uterus.

Since 1970 there have been many changes in our knowledge of premature rupture of the membranes (PROM). Studies of its causes have focused on clinical variables, physical properties of the membranes, and subclinical infection, while new diagnostic techniques have included biochemical and histochemical tests, as well as intrauterine dye injection. Within the last decade, new approaches in management have employed corticosteroids, tocolytics, and amniocentesis. Because of limitations of current studies, however, it is difficult to compare one study to another. Consequently, management of PROM rem…

Sixty-three cases of traumatic rupture of the spleen were analyzed. The patients were admitted to our hospitals during the last 24 years and there has been a steady increase in the numbers seen. The great majority of these cases resulted from road traffic accidents, and 33% of the patients had multiple injuries. Most were in the second and fourth decades; males outnumbered females by nearly four to one. Associated injuries, which occurred in 33% of cases, significantly raised the mortality. The mortality was also higher in the older age group and in those patients who had multiple injuries and…

Two cases of intramural rupture of the oesophagus are described. In both cases the diagnosis was made endoscopically and confirmed radiologically. Both patients were successfully managed conservatively but were later found to have disordered oesophageal motility. The clinical presentation of severe retrosternal chest pain followed by haematemesis and then dysphagia is stressed, and the diagnostic features on barium swallow examination and endoscopy are described. It is suggested that the diagnosis should be made on clinical presentation and barium swallow, and that the possible hazards of endo…

An increase of perilymphatic fluid pressure was found to be an important factor in the aetiology of round window membrane rupture. The critical pressure causing membrane rupture, as determined in anaesthetized cats, was in the range of 10-30 mm Hg (mean, 23.4 +/- 17.1). This value was compared to the pressure increases caused by thoracic and abdominal compression, cervical strangulation, forced Trendelenburg position, coughing, and sneezing.

A review of the literature reveals 9 cases of spontaneous renal rupture in pregnancy, to which we add another case. Most cases occurred in hydronephrotic kidneys. Four cases consisted of renal rupture without associated infection, or tumor, 3 were associated with abscesses, and 3 ruptured through renal hamartomas. Previously, all ruptured kidneys were removed. In our case the kidney was salvaged satisfactorily by prompt placement of a nephrostomy tube and drainage of the perirenal tissue. Attempt at renal salvage is urged for renal supture in pregnancy.

In a consecutive series of 93 patients with premature rupture of the membranes at 20 to 34 weeks of gestation, the perinatal mortality was 23.7%. One-quater of the deaths were due to lethal congenital abnormalities, 18% were due to intrauterine infection, and 36% resulted from severe respiratory distress syndrome (RDS). Corticosteroid therapy increased the risk of incompetence, and did not reduce the incidence or severity of RDS. Almost one-third of the patients were delivered within 48 hours of membrane rupture; however, short-term treatment with salbutamol was able to delay the delivery for …

The case is reported of a 15-year-old girl who presented following trauma and was found to have a massive congenital diaphragmatic hernia through the foramen of Bochdalek. The spleen, which was intrathoracic, had ruptured. No other similar cases have been reported in the literature hitherto.

A 44-year-old man has been seen by the present authors, apparently the third reported case of triceps brachii rupture. He had had bilateral nephrectomies one year earlier and since then has been medicated with Dilantin for grand mal seizures which followed hypovolemia during dialysis. A grand mal seizure immediately preceded the patient's right triceps brachii rupture and other multiple orthopaedic injuries. Following repair of the tendon defect the patient regained an active range of motion. The possible relationship of tendon rupture and avulsion to primary and secondary hyperparathyroidism …

Eleven uterine ruptures occurred among 11415 vacuum extractions, and these cases are analysed. Cervical tears extending into the lower uterine segment or into the broad ligament occurred in 4 cases, lower segment tears in 6 and a fundal tear in 1. One patient died of respiratory failure. The possible cause of uterine rupture with vacuum extraction appear to be a combination of an unengaged fetal head, an incompletely dilated cervix and an undetected mild cephalopelvic disproportion especially in grandmultiparae, dehiscent or weak uterine scars, prolonged traction without progress, manipulation…
