Sacroiliitis and severe disability due to isotretinoin therapy.

We report a case of acute sacroiliitis with severe disability after only 3 weeks of isotretinoin therapy with graduate reduction of pain, limitation and muscle incompetence after discontinuation of the drug and ACTH-depo injection and Ethodolac therapy. Naranjo probability scale indicated a probable relationship between isotretinoin therapy and bilateral sacroiliitis. We discussed possible mechanisms of sacroiliitis and disability due to isotretinoin treatment.

Inflammatory sacroiliitis in a man with spinal cord injury.

BACKGROUND: Inflammatory sacroiliitis associated with spinal cord injury (SCI) as an unusual cause of elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) level has not been reported previously to our knowledge. OBJECTIVE: To represent a case of SCI associated with bilateral sacroiliitis causing ESR and CRP level elevation. METHODS: Case report of a man with T9 paraplegia. FINDINGS: ESR and CRP levels were high. Pelvic radiography was nearly normal, except for mildly blurred sacroiliac joints with normal margins. A 3-phase bone scan revealed bilateral sacroiliitis and …

Pneumococcal sacroiliitis in a 4-year-old boy.

Pyogenic sacroiliitis is an extremely rare manifestation of invasive pneumococcal disease in childhood as only four cases have been described to date. We report and comment on a case of pneumococcal sacroiliitis in a 4-year-old boy. This patient was diagnosed promptly on account of the symptom triad of fever, buttock pain, and limping gait, along with characteristic findings in magnetic resonance imaging (MRI) and bone scans, and recovered fully after 6 weeks of antimicrobial therapy. Pyogenic sacroiliitis is an uncommon disease in which the diagnosis is often delayed because of nonspecific cl…

A minimally-invasive technique for the treatment of pyogenic sacroiliitis.

We report the management of an adult patient with septic sacroiliitis. This is an uncommon condition. Debridement, decompression and spontaneous fusion are the treatment of choice when symptoms do not resolve with routine initial intravenous antibiotic therapy. A percutaneous technique is described, using the principles of sacroiliac screw insertion commonly used for pelvic reconstruction surgery. After successful evacuation of the infected joint, fusion was observed in our patient.

Sacroiliitis as an initial manifestation of acute myelogenous leukemia.

Sacroiliitis is the most pathognomonic and earliest manifestation of ankylosing spondylitis. We herein report a 28-year-old female patient who presented with sacroiliitis as an initial manifestation of acute myelogenous leukemia (AML). She had a 3-month history of anemia and walking difficulty. Bone marrow findings revealed an increase of blasts with trilineage dysplasia. Although she was initially diagnosed with myelodysplastic syndrome (MDS), blasts rapidly increased and AML developed 1 month after the diagnosis of MDS with Sacroiliitis. Induction chemotherapy failed to induce a complete rem…

The investigation of sacroiliitis with different imaging techniques in spondyloarthropathies.

PURPOSE: The aim of this study was to compare the value of different imaging techniques in spondyloarthropathy (SpA) patients with inflammat … nance imaging (MRI) were performed to investigate the value of these techniques to the diagnosis of sacroiliitis, and the sacroiliac joint:sacrum uptake ratios were calculated. Scintiscanning was done in 80 healthy subjects to define the normal range. RESULTS: The sensitivities of plain radiography, quantitative SI scintigraphy, and MRI were 61%, 55%, and 89% …

Brucella spondylitis and sacroiliitis in the general population in Mumbai.

OBJECTIVE: 1) To identify patients of brucella spondylitis and sacroiliitis amongst patients of 'infective spondylitis' and 'sacroiliitis', 2) To study clinical and radiographic features and outcome of brucella spondylitis. METHODS: We reviewed 21 patients of infective spondylitis and 12 of sacroiliitis attending our hospital between March and September 2000 and followed them to identify patients of brucella spondylitis and brucella sacroiliitis, using serological tests. RESULTS: 1) Twenty six patients (17 spondylitis and nine sacroiliitis) were detected to have brucella infection to be the …

Adult celiac disease is frequently associated with sacroiliitis.

No data are available on the presence and frequency of peripheral or central joint disease, routinely determined by bone scintigraphy with 740 MBq of [99mTc]MDP, in adult celiac disease. Bone scintigraphy was carried out to detect early acute inflammatory lesions in 22 adult celiac patients (15 females and seven males; mean age 36.72 years, range 17-63). Bone scintigraphy was positive for sacroiliitis in 14 cases (63.6%). Except in the case of one patient suffering from rheumatoid arthritis, laboratory data were normal. Our data suggest that as in other chronic intestinal diseases, celiac dise…

Pyogenic sacroiliitis in children: report of three cases.

We report three children who were treated for pyogenic infection of the sacroiliac joint. The disease, usually present in late childhood or adolescence, is uncommon and difficult to assess so that the diagnosis is usually delayed. The three patients were two boys and one girl, aged 14, 15 and 12 years, respectively. A detailed history and physical examination are very important for establishment of the diagnosis. All the three cases presented with typical clinical triad of fever, limping gait and buttock pain. Pelvis compression maneuver, which directly stresses the sacroiliac joint, may aggra

Ewing's sarcoma of the ilium presenting as unilateral sacroiliitis.

We describe a 24-year-old man who presented with unilateral sacroiliac pain associated with constitutional symptoms. Subsequent biopsy and pathohistologic evaluation demonstrated Ewing's sarcoma. We emphasize a consideration of Ewing's sarcoma in the differential diagnosis of unilateral sacroiliac pain.

Clinico-biologic aspects and evolutive tendencies in sacroiliitis.

A group of 103 patients with radiologic changes of sacroiliitis were investigated and followed up over a mean period of 3.5 years. The biologic explorations (nonspecific inflammatory tests, humoral immunologic determinations and serum fibronectin assay) as well as sacroiliac and spine radiographic examinations, were repeated every 6th-12th months. In 32 patients (31%) clinical, biologic and radiologic signs of evolutive disease, were detected. In such cases, sacroiliitis representing in fact a form of onset of ankylosing spondylitis, the HLA-B27 phenotype was also determined and was found pres…

[Diagnosis of inflammatory sacroiliitis. Comparison of complementary examinations].

In order to test the sensitivity of different techniques in the diagnosis of sacroiliitis, we have made a prospective and non-controlled study of 22 patients using standard radiography of the sacroiliac joint; conventional tomography and axial scintigraphy (calculation of an ISI sacroiliac index). The normal upper limit of the sacroiliac index (1.42) was studied in parallel, using a group of 63 control subjects. Correlation between the radiological examinations was average, but was very poor between radiology and scintigraphy. Conventional tomography and calculation of the sacroiliac index wer…

Pyogenic sacroiliitis: another imitator of the acute abdomen.

Acute suppurative infection of the sacroiliac joint can mimic the acute abdomen. Experience with a patient who had exploratory laparotomy for acute appendicitis prompted a review of the literature, which indicated that 12.6% of patients with pyogenic sacroiliitis have acute abdominal pain. A high index of suspicion is necessary to establish this diagnosis.

The risk of sacroiliitis in B27 positive persons: a reappraisal.

The prevalence of sacroiliitis was assessed clinically and radiologically in 261/323 1st degree relatives of 47 B27 positive probands who had ankylosing spondylitis (AS). Of the 151 relatives who were B27 positive, 23 had bilateral sacroiliitis (asymptomatic in 4), 2 had had Reiter's syndrome (RS) and 3 acute anterior uveitis (AAU). Similar assessment of 148/182 relatives of 34 B27 positive patients without demonstrable sacroiliitis (including 15 who had had RS and 11 AAU) showed 84 to be B27 positive. Only 1 had sacroiliitis, 2 had RS, 2 AAU and 2 chronic polyarthritis. Features of this study…
