Sexually transmitted diseases among adolescents in developed countries.

CONTEXT: Sexually transmitted diseases (STDs) are responsible for a variety of health problems, and can have especially serious consequences for adolescents and young adults. An international comparison of levels and trends in STDs would be useful to identify countries that are relatively successful in controlling the incidence of STDs, as a first step toward improving policies and programs in countries with high or growing STD incidence. METHODS: Incidence data for the past decade on three common bacterial STDs--syphilis, gonorrhea and chlamydia--were obtained for as many as 16 developed cou…

Epidemiologic trends of sexually transmitted diseases in China.

OBJECTIVE: To describe the current epidemiology of sexually transmitted diseases (STDs), we analyzed the time trends in distribution of reported cases of STDs in China from 1989 to 1998. STUDY DESIGN: Based on the computerized database of the National System of STD Surveillance, a retrospective study of STD cases reported from 1989 to 1998 was carried out. RESULTS: From 1989 to 1998, STD incidence increased significantly in men and women and demonstrated nonlinear growth trends, with the exception of gonorrhea incidence. Between 1990 and 1998, the incidence increased more in females (4.20 ti…

An overview of sexually transmitted diseases. Part II.

UNLABELLED: Sexually transmitted diseases are a persistent problem in the United States and throughout the world. Many of these infections involve the skin and may be encountered in the field of dermatology. This 3-part review highlights the cutaneous features, diagnosis, and treatment of 11 of the most common sexually transmitted diseases, other than AIDS. The second part of this series focuses on anogenital warts, chronic viral hepatitis, molluscum contagiosum, scabies, and pediculosis pubis. Additional features, such as epidemiology and transmission of the organism, are discussed when appli…

Sexually transmitted diseases in children. A practical approach.

Basically, all sexually transmitted diseases occurring in adults can be transmitted to children. Patterns of transmission are as follows: intrauterine, perinatal, by sexual abuse, by voluntary sexual contact, by accidental autoinoculation or heterinoculation, or indirect transmission. In children between 2 and 10 years of age, sexual abuse is first in possible ways of transmission. Taking into consideration the age of the child, the sexually transmittable agent and the localization of the infection helps in assessing the possibility of sexual abuse.

[Sexually transmitted diseases in the History of Medicine].

Classic sexually transmitted diseases (syphilis, gonorrhea, condyloma acuminatum, venereal ulcera) were known since ancient times. Being considered as a "divine punishment" for impure intercourses, they were generally mistaken one for another (except for condylomatosis) until the different pathogenic agents were identified in the course of 19th Century. It was only with the introduction of antibiotics that their incidence drastically fell, at least up to this last decade, as a number of factors determined their fresh outbreak.

[Prevention of sexually transmitted diseases. The norwegian experience].

A significant decrease in the incidence of most STDs has been reported in Norway during the last decade, especially the last 5 years. Today, syphilis, hepatitis B and gonorrhoea are almost non-existent with incidence rates (IR) of 1.1, 0.9 and 4.4 per 100,000 respectively. The frequency of genital herpes, however, has remained unchanged (IR = 45), while chlamydial infection has shown a decrease of 13% during the last year (IR women = 240). The Chlamydia epidemic became evident in the early 1980-ies and since then has represented the major threat to the reproductive condition in young women res

[Sexually transmitted diseases and the HIV/AIDS epidemic].

Studies on sexually transmitted diseases (STD) during the previous years in Mexico are discussed. The information sources were: a) Surveys among commercial sex workers. Since 1990, 1,386 women have been studied in four federal states through structured questionnaires and laboratory tests. Prevalence of different STD's has been significant (syphilis (VDRL, FTA-abs) 23.7%; chlamydiosis (Ag IF) 12.9%; gonorrhea (Ag, ELISA) 11.5%; anti-HBs 11.0%; herpes 1,2 (IgM) 9.3%, HBsAg 5.7%. Frequency of HIV (ELISA, Western blot) has been low; 0.5%. In 1994, 662 women were studied in Mexico City, with differ…

Sexually transmitted diseases in adult, non-pregnant women.

Most of the publications reviewed focus on human papillomavirus (HPV), and a small proportion on herpes simplex virus (HSV), Chlamydia trachomatis, as well as the syndromes of pelvic inflammatory disease and bacterial vaginosis. The present hypothesis associates cervical intraepithelial neoplasia (CIN)-1 with high oncogenic risk HPV types in less than 30% of cases; whereby CIN-2 and CIN-3 are associated with a 90% risk. However, on the basis of HPV types, four categories ['low risk' (HPV 6/11, 42, 43, 44); 'intermediate risk' (HPV 31, 33, 35, 51, 52, 58); 'high risk/HPV 16'; and 'high risk/HPV

The role of fluoroquinolones in sexually transmitted diseases.

The management of sexually transmitted diseases (STDs) has reached a new level in the era of antibiotic resistance and human immunodeficiency virus infection. To date, no single antimicrobial is capable of eradicating the commonly encountered STD pathogens including Neisseria gonorrhoeae, Chlamydia trachomatis, and Treponema pallidum. Among the marketed fluoroquinolones, ciprofloxacin, ofloxacin, lomefloxacin, and enoxacin all provide excellent in vitro activity (MIC90 < 0.06 micrograms/ml) and excellent in vivo efficacy against N. gonorrhoeae, including multiply resistant isolates (penicillin…

The epidemiology and prevention of sexually transmitted diseases.

Each year, approximately 12 million cases of STDs occur in the United States, including an estimated 1.4 million cases of gonorrhea, 130,000 cases of syphilis, 4 million cases of chlamydia, 500,000 to 1 million cases of human papillomavirus, 200,000 to 500,000 cases of genital herpes, and 45,000 cases of AIDS. These numbers should be viewed cautiously, because many STDs are not reported, many are not easily diagnosed, and many are asymptomatic or inapparent. The control of STDs can be achieved only through the continuing efforts of physicians to recognize and treat STDs and to counsel patients

Sexually transmitted diseases in postpubertal female rape victims.

Seventy-six postpubertal women were referred from a municipal hospital emergency room within 60 h of sexual assault for evaluation. Of the 76 victims, 20 (26%) had active Chlamydia trachomatis infection detected by culture (11 subjects), a fourfold serologic titer rise (6), or both (3). The risk of acquiring C. trachomatis infection after sexual assault was 3%-16%. Pelvic inflammatory disease was detected in 8 (11%) of the 76 victims. Bacterial vaginosis was diagnosed in 38 women (50%), at least 8 of whom appeared to have been infected during the assault. Trichomoniasis was found in 17 victims

[Cultural identification of patients with sexually transmitted diseases].

The authors considered psychological problems concerning the communication to patients about a diagnosis of Sexual Transmitted Disease and particularly of HPV disease. In the past this sort of affection was related to some well identified social groups, but with the recent changes in sexual behavior, cultural and moral principles, the reference models are changed too. Consequently today it is difficult to recognize a rigid identification. However it is still possible that during the communication of the diagnosis the physician unconsciously transfers to the patient "his" classification with a

Epidemiology of sexually transmitted diseases: the global picture.

Sexually transmitted diseases (STD) are now the commonest group of notifiable infectious diseases in most countries, particularly in the age group of 15 to 50 years and in infants. Their control is important considering the high incidence of acute infections, complications and sequelae, their socioeconomic impact, and their role in increasing transmission of the human immunodeficiency virus (HIV). The worldwide incidence of major bacterial and viral STD is estimated at over 125 million cases yearly. STD are hyperendemic in many developing countries. In industrialized countries, the bacterial S…

Epidemiology of sexually transmitted diseases in rape victims.

Rape is a violent crime that is increasing rapidly in incidence. Victims can include male and female adults and children. These victims may be at risk for the acquisition of many sexually transmitted conditions. The risk will vary with the sex and socioeconomic status of the assailant as well as with the nature of the sexual contact. Infections with Neisseria gonorrhoeae, Chlamydia trachomatis, Trichomonas vaginalis, herpes simplex virus, human papillomavirus, and Treponema pallidum have all been found to occur following sexual assault of female adults or children. Although data are lacking, m

Enoxacin in the treatment of sexually transmitted diseases.

Enoxacin was evaluated in two double-blind comparative trials in a total of 200 male and female patients with urethral and/or endocervical gonorrhoea. Single 400-mg doses were effective in eradicating Neisseria gonorrhoeae (including penicillinase-producing strains) from patients. Enoxacin was as effective as the parenteral drugs approved for treatment. Adverse events occurred in 3% of patients. Enoxacin has been shown to be an effective well tolerated and convenient treatment for gonorrhoea.

Sexually transmitted diseases. Genital ulceration with regional adenopathy.

The specific agent causing genital ulceration is difficult to identify without laboratory confirmatory testing including darkfield microscopy, syphilis and lymphogranuloma venereum serology, and culture for Haemophilus ducreyi and herpes simplex virus. Empiric antimicrobial therapy risks masking clinical or serologic diagnosis of syphilis.
