Complex shoulder girdle injuries following mountain bike accidents and a review of the literature.

BACKGROUND: Mountain and road bike accidents are particularly common with the increased popularity of the sport. We reviewed the attendances in our emergency department over a 4-year period looking at cycling injuries to detect the level and grade of these injuries and their outcomes. METHOD: Royal Blackburn Hospital caters for a population of 550 000. A search through the Hospital information system revealed 104 patients with fractures following mountain bike injuries. These were looked at in more detail. We present a series of 5 severe shoulder girdle injuries following mountain bike accide

Comparison of magnetic resonance arthrography with arthroscopy for imaging of shoulder injuries: retrospective study.

BACKGROUND Arthroscopy is the established 'gold standard' diagnostic investigation for detection of shoulder disorders. We aimed to compare the diagnostic accuracy of arthroscopy with magnetic resonance arthrography (MRA) for detection of shoulder disorders. METHODS Patients who underwent arthroscopy by a single surgeon and preoperative MRA between February 2011 and March 2012 for shoulder instability were identified. MRAs were reported by experienced musculoskeletal radiologists. Labral tears, anterior labral tears, superior labral anterior posterior (SLAP) lesions, posterior labral tears, ro

[Value of magnetic resonance imaging in the diagnosis of sequels of shoulder joint injuries].

The paper presents the results of examining 45 persons with sequels of shoulder joint injuries, by applying magnetic resonance imaging (MRI) (100%) and arthroscopy (95.6%). The data of arthroscopy were compared with those of MRI; thereafter the sensitivity, specificity, and accuracy of MRI were calculated. The findings suggested that there was no statistically significant difference in the capacities of the two comparable techniques MRI and arthroscopy to diagnose labral tears with degenerative changes, synovitis, bursitis and tendinitis (McNemar's test; p > 0.05). The capacities of MRI are gr

Diagnosis and management of vascular injuries in the shoulder girdle of the overhead athlete.

Vascular injuries represent a rare cause of shoulder pain and functional limitation among overhead athletes. Complaints of heaviness, fatigue, paresthesias, and effort-related pain should prompt the sports medicine clinician to consider vascular pathology as a possible cause of such symptoms. Position-dependent compression of the subclavian and axillary vessels within the thoracic outlet may result in functional limitation and a decline in overhead athletic performance, particularly when symptoms occur in the dominant upper limb. Treatment options include physical therapy and (in the case of t

Assessment and management of spastic shoulder in patients with brain injury.

Following brain injury, neuromotor disorders of the upper limb are a major cause of functional disability and poor quality of life. In association with paresis and muscle changes, spasticity leads to motor imbalances responsible for joint deformities in general and in the shoulder in particular. In this expert narrative review, we address the assessment and management of spastic shoulder in patients with brain injury, including analysis of the discomfort and the clinical examination. Motor blocks and electromyography are essential, complementary tools. The definition of personalized SMART obje

Shoulder Arthroplasty for Sequelae of Obstetrical Brachial Plexus Injury.

PURPOSE: Shoulder arthroplasty following obstetrical brachial plexus injury (OBPI) is technically challenging because glenoid morphology, muscle balance, and humeral version are substantially altered compared with the neurologically intact shoulder. The purpose of this study is to report the outcome of shoulder arthroplasty in a group of patients with end-stage arthritis secondary to OBPI. MATERIALS AND METHODS: Seven patients with OBPI and secondary glenohumeral arthritis were treated with shoulder arthroplasty between 1976 and 2014. Two underwent hemiarthroplasty (HA), 2 underwent total sho

Wearable IMU for Shoulder Injury Prevention in Overhead Sports.

Body-worn inertial sensors have enabled motion capture outside of the laboratory setting. In this work, an inertial measurement unit was attached to the upper arm to track and discriminate between shoulder motion gestures in order to help prevent shoulder over-use injuries in athletics through real-time preventative feedback. We present a detection and classification approach that can be used to count the number of times certain motion gestures occur. The application presented involves tracking baseball throws and volleyball serves, which are common overhead movements that can lead to shoulder

A "needling" problem: shoulder injury related to vaccine administration.

Pancreas transplantation continues to evolve as a strategy in the management of diabetes mellitus. The first combined pancreas-kidney transplant was reported in 1967, but pancreas transplant now represents a number of procedures, each with different indications, risks, benefits, and outcomes. This review will summarize these procedures, including their risks and outcomes in comparison to kidney transplantation alone, and how or if they affect the consequences of diabetes: hyperglycemia, hypoglycemia, and microvascular and macrovascular complications. In addition, the new risks introduced by im

Mechanisms of traumatic shoulder injury in elite rugby players.

BACKGROUND: Shoulder injuries in rugby players are common, but the mechanisms of injury are less well understood. This study aims to elucidate common mechanisms of injury and identify the patterns of injury they produce. MATERIALS AND METHODS: Twenty-four elite rugby players, referred to the senior author for diagnosis and management of shoulder injuries, were selected. Videos of the injuries were independently reviewed by rugby-medical experts to describe the mechanisms of injury. The mechanisms reported were collated and analysed to determine the level of agreement between reviewers and con…

Axillary artery injury secondary to inferior shoulder dislocation.

BACKGROUND: Dislocation injuries of the glenohumeral joint are common in the general public and generally are corrected without complication. One serious complication with shoulder dislocations, or the subsequent reduction, is a lesion to the axillary artery. This specific complication is most frequently seen in the elderly population, where vascular structures have become less flexible. Also, these injuries are most common in association with anterior dislocations of the shoulder. OBJECTIVES: To bring awareness to the possibility of axillary artery injury with inferior dislocation of the sho

Optimizing shoulder dystocia management to prevent birth injury.

A practical clinical review of those aspects of shoulder dystocia management that are directly relevant to birth injury is presented. In contrast to more popular viewpoints, the tenets of this paper are that, with few exceptions, clinically relevant, permanent brachial plexus injury is nearly universally associated with shoulder dystocia, injury is causally related to mechanical stresses induced during shoulder dystocia delivery, and management algorithms can be optimized to reduce the incidence of mechanical birth injury from shoulder dystocia. Advantages of direct rotational manipulation of

In vitro kinematics of the shoulder following rotator cuff injury.

BACKGROUND: Tears in the rotator cuff may result in altered mechanics of the glenohumeral joint. It is not unusual for some patients with cuff tears to exhibit near normal motion with the injured shoulder, while other patients with the same magnitude of injury are unable to achieve full elevation on the injured side. METHODS: The effect of simulated tears of the rotator cuff on active glenohumeral joint kinematics was investigated by testing eight cadaveric specimens using an in vitro shoulder simulator. Active abduction of the humerus was produced by applying forces to simulate loading of th

Anterior shoulder dislocation with axillary artery and nerve injury.

We report a rare case of left axillary artery injury associated with anterior dislocation of the left shoulder in a 25 yrs old male as a result of a road traffic accident. The shoulder dislocation was reduced. A left upper limb angiogram showed an obstructed left axillary artery. The obstructed segment was surgically reconstructed with a Dacron graft. Six months post operation in follow up, he was found to have good left shoulder function and no neurovascular deficit. This is an injury that could have been easily missed without a simple clinical examination.

Isometric shoulder torque in subjects with spinal cord injury.

The etiology of shoulder pain in spinal cord injured (SCI) patients has been attributed to overuse, with dysfunction being more prevalent as the time since injury increases. Impingement syndrome, the most common diagnosis in this population, may be related to weakness of the rotator cuff and shoulder girdle musculature. Shoulder dysfunction is greater in subjects with SCI than in able-bodied patients; consequently, this study compares isometric strength of quadriplegic and paraplegic subjects to able-bodied controls. Peak isometric torque for internal rotation, external rotation, and scapular

Shoulder-hand syndrome in cervical spinal cord injury.

To characterize the occurrence of shoulder-hand syndrome (SHS) complicating the rehabilitation of patients with cervical spinal cord injury, we reviewed the medical records of 43 consecutive patients admitted to the Burke Rehabilitation Center with cervical spinal cord injury, focusing on the clinical features of SHS: shoulder pain, hand/wrist pain, edema, vasomotor changes, trophic changes and osteoporosis on x-ray. Twenty-seven patients (63%) had three or more features of SHS. The number of features correlated with age (r = 0.495, p = 0.0007), but not with the presence of upper or lower moto

Anterior dislocation of the restrained shoulder: a seat-belt injury.

The reduction in morbidity and mortality since the enforcement of seat-belt usage is well documented. Complications from the belt are also reported and the authors present anterior dislocation of the restrained shoulder, an injury not previously described. The mechanism of injury is explained and a change to the present standard of restraints is suggested.
