[Voice handicap in patients with organic and functional dysphonia].

BACKGROUND: The goal of this study was to assess and compare voice handicap in patients with benign organic and functional dysphonia. PATIENTS AND METHODS: Voice handicap was assessed using a German version of the Voice Handicap Index. A total of 108 German speaking patients, 64 women and 44 men aged 45.3+/-15.1 years with benign organic ( n=69) or functional ( n=39) voice disorders completed the questionnaire immediately after clinical examination and without knowing their individual diagnosis. Fifty individuals without voice complaints and normal voice status, 28 women and 22 men aged 47.1+

Intravestibular Balance and Motion Sickness.

INTRODUCTION: A theory is presented to explain the major findings regarding motion sickness and to synthetize current theories concerning its etiology. The theory proposes that an imbalance in the output of the two major organs of the labyrinth-favoring the semicircular canals over the otolith organs-is responsible for most instances of motion sickness as experienced in terrestrial and microgravity environments. METHODS: Strengths and limitations of current theories are first outlined before the different roles of the canals and otoliths in the genesis of motion sickness symptoms are describe

Prevalence and illness beliefs of sleep paralysis among Chinese psychiatric patients in China and the United States.

To investigate the prevalence and illness beliefs of sleep paralysis (SP) among Chinese patients in a psychiatric out-patient clinic, consecutive Chinese/Chinese-American patients who attended psychiatric out-patient clinics in Boston and Shanghai were asked about their lifetime prevalence, personal experience and perceptions regarding the causes, precipitating factors, consequences, and help-seeking of SP. During the 4-month study period, 42 non-psychotic psychiatric out-patients from the Boston site and 150 patients from the Shanghai site were interviewed. The prevalence of SP was found to b

[Psychiatric disorders in intensive care units].

The diagnosis and treatment of psychiatric disorders in intensive care patients have been for a long time neglected. They are nowadays better recognized and managed. These disorders are mainly: delirium; anxiety disorders, from simple anxiety to panic disorder with agitation; adaptation disorders with depressive mood; brief psychotic disorders with persecution ideas. The manifestations of psychiatric disorders occur not only during the stay in intensive care unit (ICU) but also after transfer from ICU and several months after discharge from hospital. Part of psychiatric disorders is caused by

The role of illness, resources, appraisal, and coping strategies in adjustment to HIV/AIDS: the direct and buffering effects.

This study examined the utility of a stress and coping model of adjustment to HIV/AIDS. A total of 114 HIV-infected gay or bisexual men were interviewed and they completed self-administered scales. Predictors included illness variables (disease stage and number of symptoms), coping resources (optimism and social support), appraisal (threat, challenge, and controllability), and coping strategies (problem- and emotion-focused). Adjustment outcomes were depression, global distress, social adjustment, and subjective health status. Results from hierarchical regression analyses indicated that better

[Possibilities for psychotherapy treatment of Alzheimer disease patients in the early stage of the disease].

Psychological changes may precede the neuropsychological symptoms of Alzheimer's disease (AD). In addition, early stages of AD are often accompanied by a depressive disorder. Therefore, we reasoned whether AD patients might profit from psychotherapy. The aim would be to alleviate anxiety, discouragement, and inactivation of patients suffering from the early stages of AD. According to preliminary experience with a small number of treated cases, interpersonal psychotherapy, modified for application in "late-life" (IPT-LL), has been found to be useful for the treatment of AD patients. The case of

Depressive symptoms and self-concept in young people with spina bifida.

Examined self-reported depressive symptoms in 72 young people with spina bifida, ages 9 to 18:11, and matched able-bodied comparison subjects, using the Dimensions of Depression Profile for Children and Adolescents (Harter & Nowakowski, 1987). Independent variables included gender, self-perceptions (including physical appearance), and perceived social support. Young people with spina bifida were at greater risk of depressive mood, low self-worth, and suicidal ideation. Girls, independent of disability, were at greater risk of depressive mood, low self-worth, and self-blame. Multiple regression

Wheelchair use: a physical sign in gastroenterological practice.

Diagnosis of functional abdominal pain requires exclusion of organic causes, and many patients undergo considerable investigation. A positiv … sers gave a clear account of lower limb pain or weakness. Secondary gain with reinforcement of the 'sick role' was felt to be the probable explanation for wheelchair use in the former group. Wheelchair attendance at the gastroenterology clinic, in the absence of lower limb symptoms, is a rare observation but one that may usefully be added to criteria for di …

[Essential hypertension: psychosomatic aspects].

Essential hypertension is a heterogenous disease made up of several still unspecified subgroups. According to the dual theory of hypertension, acquired and genetic factors can lead to vascular muscle defects and increased sympathetic drive, which, either both or each alone, can lead to an increased vascular resistance. Acquired factors can either help a predisposition to become a manifest disease, act by itself to cause a disease or prevent a genetic trait from becoming manifest. Among the acquired factors, we differentiate those lying inside the personality, which means they are early acquire

[Negative cortisone attitude in patients with bronchial asthma].

Patients with bronchial asthma often exhibit irrational fears of cortisone medication beyond justified worries about side-effects. A negative cortisone image, which means overemphasizing the damaging and threatening aspects of cortisone, often underlies non-compliant illness behaviour. In the present study, cortisone image was investigated in 54 patients with bronchial asthma before and after participation in the Düsseldorf Asthma treatment and teaching programme (ATTP) and one year later. By participating in ATTP, a significant reduction of the negative cortisone image and thus a more realist

[Family coping of Gilles de la Tourette syndrome].

A survey over the disorder of chronic multiple tics (Tourette-Syndrom) is given. Two case reports reflect the effects of the syndrome on the family and other relational systems. Specific relations between the unusual and irritating symptomatology and the reaction of the family members are emphasized. In conclusion we offer some proposal for psychotherapeutic interventions, to help the relatives to cope better with the burden of the disorder.

The changing faces of somatization.

The prevalence of several diseases popularized by the media is examined in a group of 50 consecutive patients allegedly suffering from environmental hypersensitivity disorder. Ninety percent of patients reported suffering from at least one other "fashionable" condition, including food allergies that cause psychological symptoms, postinfectious neuromyasthenia, candidiasis hypersensitivity, and severe premenstrual syndrome. Each of the conditions named above was endorsed by at least 50% of patients. Multiple endorsements were common, and the patients' attribution of the etiology of their sympto

Depression and anxiety secondary to medical illness.

Mind and body remain stubbornly one. The distinction between primary and secondary disorders respects this unity. The distinction between "reactive" and "induced" carry causal implications and suggest the former is psychogenic and the latter organic--both of which are probably premature conclusions. The diagnostician, free of the demands on the pathologist, can pursue the correct nosology committed to demonstrating, not the pathophysiology, but the presence of adequate diagnostic criteria. Whenever a secondary disorder meets full criteria it may warrant the same treatment accorded to the prima

Psychological components and the role of adjunct interventions in chronic idiopathic urticaria.

A psychological assessment (Symptom Checklist-90) was performed on 19 patients with chronic idiopathic urticaria. Compared to the control group, the urticaria patients had significantly higher scores on the scales of somatization, obsessive-compulsive, interpersonal sensitivity, depression, and anxiety. In general, the urticaria patients were quite anxious and experienced significant discomfort in interpersonal relationships. Based on the results of this study and other similar reports in the literature, it is suggested that adjunctive treatment of urticaria patients should focus primarily on
