[Chondroma of the skull base and maxilla].

INTRODUCTION: Chondromas are uncommon benign tumors of cartilaginous origin. Although chonodroma of the jaw is extremely rare, the commonly … TERIAL AND METHODS: We present a patient with a resected and histologically proven chondroma of the skull base and maxilla. A 65-year-old female was admitted to our clinic with swelling and breathing difficulties. MRI showed a large soft tissue mass of the skull base and maxilla. Clinicopathological and radiological features were examined by computed to …

Plasma cell tumors of the skull.

BACKGROUND: Plasma cell tumors are rare neoplasms, which include extramedullary plasmacytoma, solitary plasmacytoma of the bone, and multiple myeloma. Although indistinguishable histopathologically, these entities need to be differentiated as treatment and prognosis vary. METHODS: This study was conducted by retrospective chart review and correspondence with patients diagnosed to have plasma cell tumors of the skull from 1992 to 2004. The clinico-diagnostic investigations, treatment, and subsequent follow-up of the patients were obtained. RESULTS: We report 5 cases of plasma cell tumors of t…

The low skull base-is it important?

PURPOSE OF REVIEW: Little is published regarding variations of skull-base anatomy and their relevance to the performance of endoscopic sinus surgery. Several catastrophic complications have occurred in patients with low-lying skull-base variation. This review's purpose is to make the reader aware of skull-base variation, their recognition, and a surgical plan for sinus surgery in these patients. RECENT FINDINGS: Only a few papers are available for review discussing variations of the low-skull base. Recent findings in reviewing multiple CT scan indicate several variations of the skull. It has …

Common surgical pitfalls in the skull.

BACKGROUND: A detailed knowledge of the morphologic variations in the ossicles, foramina, and ridges of the skull vault and skull base is vital to performing safe radical surgery. METHODS: A surgical reminder of possible pitfalls was composed based on the incidences of most of the minor variations such as the supraorbital notch, frontal foramen, metopism, foramen caecum, parietal foramina, bony defects in the fossa occipitalis cerebellaris, Inca bone, foramen lacerum anterius, incomplete posterolateral wall of the foramen ovale, absence of the medial or posterior wall of the foramen spinosum,…

Epidermoid cyst with perforation of the skull.

A rare case of an epidermoid cyst associated with a skull defect is reported. The patient was a 59-year-old healthy woman from Japan who exhibited an elastic, soft, subcutaneous tumor on the left side of the parietal scalp for approximately 10 years without a history of trauma. On radiographic findings, a punched-out lesion was shown. Computed tomography of the cranium revealed a mass on the left parietal side that adhered to the cerebrum through the bone defect. Surgical excision resulted in remission. The tumor was peridurally located and showed no adhesion to the dura mater encephali. Histo…

Posttreatment imaging of the skull base.

Imaging the skull base after surgery can be challenging because anatomic structures may have been destroyed by an underlying process or removed at surgery. Foreign substances may be introduced to fill a void left by tumor resection, for hemostasis, and to repair dural defects. Previous imaging studies must be available for comparison to understand the characteristics of an underlying lesion. By following the progression of a lesion on subsequent imaging studies, the nature of treatment-related changes and residual or recurrent pathology is best realized.

[The dry skull model in orthodontics].

Orthodontic therapy aims to displace teeth by means of orthodontic appliances. Forces, applied on the teeth, induce remodeling of the alveolar process in upper and lower jaw resulting in a new position of the teeth into both jaws. In this publication an attempt has been made to test a model (dry skull model) which could provide a better understanding of the effect of force application on tooth and bone displacement. Initial tooth displacement (microscopical), right after force application, could give an indication for the longitudinal tooth displacement to be expected. Variables as the point o…

Derivation of the mammalian skull vault.

This review describes the evolutionary history of the mammalian skull vault as a basis for understanding its complex structure. Current information on the developmental tissue origins of the skull vault bones (mesoderm and neural crest) is assessed for mammals and other tetrapods. This information is discussed in the context of evolutionary changes in the proportions of the skull vault bones at the sarcopterygian-tetrapod transition. The dual tissue origin of the skull vault is considered in relation to the molecular mechanisms underlying osteogenic cell proliferation and differentiation in th…

Strain in the galago facial skull.

Little experimental work has been directed at understanding the distribution of stresses along the facial skull during routine masticatory behaviors. Such information is important for understanding the functional significance of the mammalian circumorbital region. In this study, bone strain was recorded along the dorsal interorbit, postorbital bar, and mandibular corpus in Otolemur garnettii and O. crassicaudatus (greater galagos) during molar chewing and biting. We determined principal-strain magnitudes and directions, compared peak shear-strain magnitudes between various regions of the face,…

Neurofibromatosis of the orbit and skull base.

Von Recklinghausen's disease is characterized by multiple neurofibromas, pigmentations, and pachydermatoceles of the skin depending on the disorder of the neural crest derivative. We report on a 25-year-old patient suffering from neurofibromatosis localized in the cranio-orbital region, whose main problem was a pulsating right exophthalmos caused by herniation of the frontotemporal lobe through a defect of the greater wing of the sphenoid. The lesion was approached through a combined route (i.e., transfrontally and transfacially [upper eyelid incision]). Surgical strategy and associated proble

[Trauma of the facial bones and skull].

Facial trauma is frequent and mainly caused by motor vehicle accidents. Due to this main etiologic factor, trauma to the facial skeleton is often associated with serious injuries, commonly involving the brain, chest or abdomen. As a consequence, the initial clinical management of these patients includes control of hemorrhage and immediate assessment of life-threatening injuries, including the maintenance of the airways. Patients presenting with facial trauma are initially evaluated with a systematic clinical examination because many fractures can be accurately diagnosed by inspection and palpa

Telangiectatic osteosarcoma of the skull base.

We present a patient with a distinctive lesion of the skull base, which, at imaging, showed expansile destruction of the sphenoid bone, hemorrhage with fluid-fluid levels, and contrast enhancement of a solid portion. These features ordinarily suggest either giant cell tumor or aneurysmal bone cyst; however, pathologic examination confirmed instead a rare variant of osteosarcoma of the telangiectatic type. Although imaging findings can indicate any of these possibilities, biopsy is an essential step in arriving at the diagnosis.

Occult malformations of the skull base.

Occult malformations of the skull base are rare anomalies. They are often not detected until they give rise to complications such as meningitis or cerebrospinal fluid rhinorrhea. We used high-resolution computed tomography, fluorescein endoscopy, cisternography, magnetic resonance imaging, and duraplasty to diagnose occult malformations of the skull base in 4 patients. The four patients had had between 4 and 6 attacks of meningitis. Cerebrospinal fluid rhinorrhea was confirmed in 3 cases. The following occult malformations were found: (1) an encephalocele of the glabella/cribriform plate and a…

Primary myxomas of the skull base.

Twelve cases of intracranial primary myxomas are discussed. The mean age at presentation was 34.2 years and seven patients were male. All tumours originated extradurally from the skull base. Most of the patients had a chronic clinical course, characterized by unilateral dysfunction of one or several cranial nerves. It was difficult to determine the pathological diagnosis before operation. Radical excision is the best choice of treatment for intracranial myxoma, but the tumour has a tendency to recur. Intracranial primary myxomas have been rarely discussed in the neurosurgical literature. There…

Impacted canine in a prehistoric skull.

Impacted teeth are frequently found in humans today, but current data suggest their existance in skulls originating from the prehistoric age. This report describes the skull of an adult female that was found during excavation in the Croation enolithic site of Vucedol (2700-24--BC). The skull is well preserved and, according to cranial and facial index calculation, is classified as dolichocephalic and mesoprospic. Cephalometric analysis revealed bimaxillary prognathism. The maxillary left canine is impacted and its crown peak is visible through the alveolar maxillary bone fenestration. The posi…

Gigantic epidermoid cyst of the skull.

One case of an unusually giant epidermoid cyst of the skull is described. The neoformation had been present for 40 years, becoming extremely large in the last 20 years, in a 65-year-old woman. Difficulty in wearing glasses was the sole complaint of the patient, who otherwise concealed the mass by wearing a wig. Preoperative examinations (x-rays, CT scan, MRI) were important to better evaluate the nature of the cyst and to make a differential diagnosis. Dissection of the mass from the surrounding tissues was extremely easy because of its capsule. Sixteen months of follow-up have been pleasant, …

[Epidermoid cyst of the base of the skull].

Epidermoid cysts are rare congenital tumours representing less than 1% of all intracranial tumours. They are derived from the development of abnormally situated ectodermal tissue and are often an incidental finding. We report a case confined to the base of the skull, responsible for an extradural mass.

Meningioma of the posterior skull base.

Combined intratemporal and cerebellopontine angle meningiomas are rejatively rare. There are unsolved problems with the stability of the skull and spine and the lower cranial nerves and there is a marked tendency for the tumor to involve the spinal cord. This article reports on five cases of combined intratemporal and cerebellopontine angle meningiomas.
