[Surgical treatment of lumbar spinal canal stenosis in elderly patients].

Lumbar spinal stenosis is one of the most frequent causes of spinal surgical interventions in over 60-year olds. The exact relationship between degenerative changes and the resulting symptoms is unclear since imaging shows stenotic changes in the spines of many symptom-free patients. The concurrence of imaging findings, the symptoms described and manifestations is crucial for the indication of surgical decompression. Nevertheless, spinal claudication that is refractory to conservative therapy is the most frequent indication for surgery. The success rate two years after OP is approximately 80%

Tuberculoma of the cervical spinal canal mimicking en plaque meningioma.

A previously healthy, HIV-negative, 40-year-old man presented with a 2-month history of progressive weakness of his left arm. Magnetic resonance imaging revealed an intradural, extramedullary plaque-shaped lesion at C6-T1 levels with high contrast enhancement. Based on the patient's clinical and radiologic findings, it was believed that the patient had an en plaque meningioma, and he was operated on. Histologic examination of the mass revealed granulomas with multinucleated and Langhans-type giant cells, typical of a tuberculoma. Intradural extramedullary tuberculomas should be considered in t

Primary intradural pontocerebellar chordoma metastasizing in the subarachnoid spinal canal.

Chordomas that are entirely extraosseous and intradural are rare. Additionally subarachnoid spinal implantation from such a cranial, intradural chordoma has never been reported before. The authors present a case of a widespread primary intradural chordoma in the basal cisterns of a 48-year-old woman which shows seeding of neoplastic cells to the spinal leptomeninges. It is concluded that also in cases of intradural and intracranial chordomas a tumor staging should include the search for spinal subarachnoid metastases.

[Image and clinical correlative studies on cervical spinal canal stenosis].

Clinical finding, X-ray and MR imagings in 47 patients who underwent operation for degenerative cervical stenosis were correalatively studied. JOA score was used for evaluation and it varied from 6 to 14 points, with an average of 10.13 points. All Torg ratios were smaller than normal value and that of C4 was the smallest. The reserve space, sagittal diameter of spinal cord decreased, especially in the diseased segments. Sagittal diameter of spinal cord showed on T2 weighted imaging and Torg ratio were in good correlation with JOA score (P < 0.01, r = 0.4815, r = 0.0993), MRI being superior to

Lumbar vertebral canal morphometry for computerised tomography in spinal stenosis.

Computerised tomography is described in recent literature as a sophisticated but noninvasive technique with a particularly appropriate application in the discovery of space-occupying lesions in the lumbar vertebral canal and nerve root tunnels. Although the spinal stenosis syndrome must remain a clinical diagnosis in the first instance, computerised tomography is expected to play an increasing role in confirming the presumptive diagnosis of developmental or degenerative spinal stenosis, and in confirming the presence of a trefoil configuration or narrow lateral recess. However, the diagnosis o

The lumbar spinal canal in hypophosphataemic vitamin D-resistant rickets.

Lumbar spine radiographs of 17 adult patients with hypophosphataemic vitamin D-resistant rickets have been examined and measured in their anteroposterior and transverse dimensions. Comparison of these measurements with those of a group of normal individuals showed the HPR patient group to have narrower spinal canals in both diameters. No differences were found between males and females in either group. The lumbar spinal canal stenosis in adult HPR patients may make them more susceptible to spinal cord compromise from space occupying lesions within the canal, causing back pain and lower limb we…

[Spinal canal stenosis : Widespread disease or just a radiological diagnosis?].

Lumbar spinal canal stenosis primarily affects older individuals over 65 years old and is often associated with typical symptoms, such as spinal claudication. Aside from rare congenital causes, it usually develops secondarily due to age-related changes, such as disc protrusion, facet joint degeneration and ligament hypertrophy. Apart from the typical clinical symptoms, native upright X‑ray images in two planes and magnetic resonance imaging (MRI) are decisive. Patients without relevant neurological deficits should primarily undergo conservative treatment. In cases of pain that cannot be allevi…

A Case of Sacral Osteomyelitis Causing Ascending Spinal Canal Infection.

Sacral osteomyelitis is an infection of the bone that extends posterior to the pelvis at the base of the spine. This condition typically occurs in elderly or bedbound/immobile patients and is treated with wound debridement, wound care, surgery, and antibiotic therapy. In this report, a case is presented of a rare complication of sacral osteomyelitis where the infection extended through the bone into the spinal canal causing an ascending spinal canal infection. This report is meant to provide an individual example of this rare complication which can hopefully be used to improve surveillance, tr…

Extent of Decompression of Lumbar Spinal Canal after Endoscopic Surgery.

Introduction Endoscopic techniques are being used in lumbar disk disease and lumbar canal stenosis to decompress the spinal canal. The present study analyzed pre- and postoperative magnetic resonance imaging (MRI) measurements of the lumbar canal. Material and Methods This was a prospective study of 30 lumbar levels. Patients < 18 years of age with unilateral compression, previous surgery at the same level, and spinal instability were excluded. Endoscopic posterior decompression was used. Pre- and postoperative MRIs of all the patients were performed. Anteroposterior (AP), transverse, interfac…

Labor Epidural Intolerance Due to a Congenitally Narrowed Spinal Canal.

Reports exist of severe upper back pain of unknown etiology after administration of large volumes into the epidural space. We present a case of an otherwise healthy parturient who developed severe upper back and neck pain after receiving only a small volume of epidural medication. Magnetic resonance imaging revealed a congenitally narrowed spinal canal because of short pedicle syndrome. Epidural injectate occupies and compresses a percentage of the spinal canal and its neuronal contents. This may result in pain and epidural intolerance when continued injectate reaches a critical point, a thres…

Bilateral spinal canal decompression via hemilaminectomy in cervical spondylotic myelopathy.

BACKGROUND: In cervical spondylotic myelopathy (CSM), laminoplasty (LP) or laminectomy plus fusion (LF) are accepted operative options and alternatives to anterior approaches. Both LP and LF have distinctive disadvantages, which might be avoided by unilateral hemilaminectomy and bilateral decompression of the spinal cord. METHODS: Description of the surgical technique, indications, and limitations. The potential advantages in comparison to LP and LF are discussed. CONCLUSIONS: Unilateral hemilaminectomy allows bilateral decompression of the whole dorsal circumference of spinal cord from nerv

Percutaneous Kyphoplasty for Kummell Disease with Severe Spinal Canal Stenosis.

BACKGROUND: Percutaneous kyphoplasty (PKP) has been proven as an effective, minimally invasive procedure for the treatment of Kummell's disease in the early stages. However, a risk of cement leakage and further neurological damage remains during and after PKP, especially in chronic osteoporotic stage III Kummell's disease with severe spinal canal stenosis. OBJECTIVE: To evaluate the feasibility and efficacy of PKP for the treatment of chronic osteoporotic stage III Kummell's disease with severe spinal canal stenosis. STUDY DESIGN: A retrospective evaluation of postoperative radiographs. SET…

Intraosseous schwannoma of thoracic 12 vertebra without spinal canal involvement.

Schwannomas are common tumors of the nerve sheath cells. They present as well-circumscribed masses attached to the peripheral nerves, cranial nerves, or spinal nerve roots. Intraosseous localization of these tumors is rare. The presented case is a 55-year-old female patient, the very first case of intraosseous schwannoma of Th12 vertebra without spinal canal involvement. She was admitted with a progressive back pain and seldom pain and numbness in the right leg for 1 year. Preoperative findings on computed tomography and magnetic resonance images of the lesion resembled that of any primary tum…

[Surgical treatment of cervical spinal canal stenosis in elderly patients].

The diagnosis is established through neurological examinations, laboratory and imaging diagnostics. Conservative treatment such as a neck brace, drug and physical therapies could bring relief. Manifest or progressing deficiency symptoms associated with sensory or motoric deficit, bladder, rectal or erectile disorder are indications for early surgery.

Assessment of lumbar spinal canal stenosis by magnetic resonance phlebography.

There is evidence to suggest that cauda equina intermittent claudication is caused by local circulatory disturbances in the cauda equina as well as compression of the cauda equina. We evaluated the role of magnetic resonance phlebography (MRP) in identifying circulatory disturbances of the vertebral venous system in patients with lumbar spinal canal stenosis. Extensive filling defects of the anterior internal vertebral venous plexus were evident in patients with lumbar spinal canal stenosis ( n = 53), whereas only milder abnormalities were noted in patients with other lumbar diseases ( n = 16)…
