Management of strabismus in thyroid eye disease.

Thyroid eye disease is an auto-immune condition characterised by an acute inflammatory phase followed by a fibrotic phase, which sometimes leads to restricted eye movements and diplopia. Medical treatment with systemic steroids with or without orbital radiotherapy and immunosuppression can control the inflammatory response. Strabismus surgery should be carried out only after the inflammation is no longer active and after any decompression surgery. Surgery comprises recession of tight muscles using adjustable sutures so as to maximise the area of binocular single vision. There is debate as to w…

Amniotic membrane in the management of strabismus reoperations.

PURPOSE: To reduce postoperative scar formation and to improve duction using amniotic membrane (AM) in strabismus reoperations. METHODS: A prospective study of interventional case series comprised of 14 patients with restrictive strabismus. Objective clinical findings (visual acuity, angle of deviations and degree of duction deficits) were recorded in both the pre- and post-operative periods. Strabismus surgery included the excision of adhesions and scar tissue, repositioning of extraocular muscles (according to the degree of deviations) and placement of two sheets, one between muscle and ten…

Role of hereditary factors in strabismus occurrence.

AIM: This work was aimed to determine the frequency of hereditary factors in relatives of children who have previously underwent the examination and appropriate diagnostic procedures that proved existence of squint. METHODS: Our research included 113 relatives who responded to our calls for review. RESULTS: In 36 (31.85%) of hereditary factors in the occurrence of one or more members of the same family. Some members had appearance of hereditary factors in the cases of all three generations, while in other cases of strabismus it was present in one or two generations of the same genealogical t…

Perception via the deviated eye in strabismus.

Misalignment of the eyes can lead to double vision and visual confusion. However, these sensations are rare when strabismus is acquired early in life, because the extra image is suppressed. To explore the mechanism of perceptual suppression in strabismus, the visual fields were mapped binocularly in 14 human subjects with exotropia. Subjects wore red/blue filter glasses to permit dichoptic stimulation while fixating a central target on a tangent screen. A purple stimulus was flashed at a peripheral location; its reported color ("red" or "blue") revealed which eye's image was perceived at that …

Strabismus surgery in the over 60s--an update.

BACKGROUND: The proportion of individuals above retirement age is expanding and consequently there is increased demand for ophthalmic surgery, including strabismus. In 2001 we published a pilot study reviewing all strabismus operations performed on patients aged 60 and over between 1992 and 1999. Almost a decade later, we repeated the analysis, aiming to identify any longitudinal trends. METHODS: Using the hospital information system, we conducted a retrospective review of all elective strabismus surgery in patients aged 60 years and over, performed by one surgeon (JPL) between January 2000 a…

Neural mechanisms for smooth pursuit in strabismus.

The visual and oculomotor systems of primates are immature at birth and sensitive to injury. If synergistic interactions between visual and oculomotor systems are compromised during the first months of life, disorders in eye alignment, gaze holding, and smooth pursuit (SP) follow. Here we consider some potential neural mechanisms supporting SP and associated vestibular ocular reflex (VOR) behavior in normal and strabismic monkeys. Experimental strabismus was created by prism goggle wearing or eye muscle surgery in rhesus monkeys (Macaca mulatta). SP and cancellation of the VOR were highly asym…

[Some principles in surgical treatment of strabismus].

Taking into consideration the age of the patient, the strabismus surgery targets different things. Thus, for infants, the goal of the surgical intervention for congenital esotropia, is to prevent the occurrence of amblyopia and binocular vision dysfunctions (ARC, suppression). In preschool children, we operate aiming the recovery of the binocular vision, while in children over ten years old, the surgery is done only for esthetic reasons. On the other hand, in adults, the strabismus surgery has two aspects: for esthetic reasons in monocular strabismus with amblyopia, or for diplopia treatment i…

Recruitment and manpower in pediatric ophthalmology and strabismus.

PURPOSE: Many have observed what appears to be declining interest on the part of ophthalmology graduates in pediatric ophthalmology and strabismus (PO&S) as a career. Four questions might address this concern: (1) Has there been a decline in the number of fellowship positions filled in the period 2000 to 2005? (2) Why do graduates choose other career paths? (3) Assuming there has been a decrease in interest in PO&S, does it reflect dissatisfaction on the part of pediatric ophthalmologists in their field? (4) What can be done to enhance the appeal of the subspecialty? METHODS: Data from the Sa…

Corneal 'dellen' as a complication of strabismus surgery.

PURPOSE: This paper presents results of an analysis on patients operated on for strabismus in order to evaluate frequency and clinical characteristics of corneal alterations. In our experience, this kind of complication occurs more frequently after reoperation and/or after surgery for esotropia in sixth nerve palsy using transposition procedures. METHODS: A retrospective analysis was made of 655 consecutive patients operated on for strabismus on the recti muscles with a limbal approach from January 2001 to July 2003 (30 months). RESULTS: We found 30 corneal dellen out of the 184 eyes (16.30%…

[Dissociated deviation. A strabismus of cortical origin].

BACKGROUND: We undertook this study to determine the cortical alterations related to the origin of dissociated deviation. METHODS: This was a prospective, descriptive and observational study that included 10 children with diagnosis of dissociated horizontal deviation, without previous evidence of neuronal damage. Manual, cerebral and ocular dominance were determined, as well as sensorial state and visual perceptual profile. Cortical response to different stimuli was analyzed: luminous, hyperventilation, slow and saccadic movements of pursuit, gaze and stare through cerebral mapping and neurom

Risk of strabismus and ambylopia in children with hydrocephalus.

OBJECTIVES: The present study was undertaken to determine the risk of strabismus and ambylopia in children who underwent operation for hydrocephalus and to compare our results with those in previous studies. METHODS: Full orthoptic and ophthalmological examinations, including cycloplegic refraction, were performed in all subjects. RESULTS: Ten of 25 patients (40%) were found to have manifest squint. Four of these had esotropia and six had exotropia. No paretic squint or alphabetic pattern strabismus was determined. Refraction measurements revealed amblyogenic refractive errors (significant r…

Influence of viewing distance on vertical strabismus.

BACKGROUND: Vertical strabismus can be modulated by the viewing distance. We report on 19 patients with this disorder. METHODS: The following squint angles were measured by the alternate prism and cover test at a viewing distance of 5 m. At 0.3 m, measurements were taken with and without an addition of 3.0 D to the corrected refraction. Cases of a dissociated vertical deviation were excluded. Fifteen patients underwent surgery. They were reexamined 3 months later. RESULTS: At a viewing distance of 5 m, the vertical deviation ranged from 0 degrees to 16 degrees (median 7 degrees ). At 0.3 m, …

Surgical management of strabismus for dysthyroid ophthalmopathy.

BACKGROUND: The study reports our 8-year experience in the surgical treatment of restrictive myopathy resulting from dysthyroid ophthalmopathy. We tried to determine the factors that contributed to favorable surgical outcomes. METHODS: The charts of patients who had surgical treatment for strabismus related to dysthyroid ophthalmopathy at Chang Gung Memorial Hospital between January 1995 and December 2002 (n=25) were retrospectively reviewed, and factors that possibly influenced the outcome were statistically analyzed for significance. RESULTS: The mean pre-operative vertical deviation of th…

A strabismus susceptibility locus on chromosome 7p.

Strabismus has been known to have a significant genetic component, but the mode of inheritance and the identity of the relevant genes have been enigmatic. This paper reports linkage analysis of nonsyndromic strabismus. The principal results of this study are: (i) the demonstrated feasibility of identifying and recruiting large families in which multiple members have (or had) strabismus; (ii) the linkage in one large family of a presumptive strabismus susceptibility locus to 7p22.1 with a multipoint logarithm of odds score of 4.51 under a model of recessive inheritance; and (iii) the failure to…

Strabismus in infants of opiate-dependent mothers.

AIM: At a follow-up clinic for infants of opiate-dependent mothers it was noted that more infants than expected developed strabismus. This study aimed to assess the prevalence of strabismus and the need for active strabismus surveillance in this population. METHODS: Consecutive infants of opiate-dependent mothers born over an 18 mo period were recalled for ophthalmological assessment by an ophthalmologist and orthoptist. Those unable to attend were surveyed by telephone using a questionnaire. RESULTS: 49 (69%) of the 71 eligible infants were recalled at a mean age of 21 mo (range 6-39); 29 h…

A study of heredity as a risk factor in strabismus.

AIMS: Inheritance is recognised to have a part in the aetiology of strabismus but previous studies have not adequately distinguished between different types of strabismus leading to wide variations in reported findings. The aim of this study was to investigate the importance of heredity in different types of strabismus. METHODS: The parents of children attending for treatment of strabismus over a one-month period were interviewed to identify relatives with a history of strabismus. A complete three-generation pedigree was established for 96 index cases who were classified into four groups: inf…

Impairment of the binocular coordination of saccades in strabismus.

To examine the link between binocular vision and binocular coordination of saccades we studied subjects with convergent strabismus since childhood with mild or no amblyopia: three subjects had small squint (< 10 prism D) and preserved peripheral binocular visual function with gross stereopsis; four subjects had larger squint (18-35 prism D) and no detectable stereopsis. A standard paradigm was used to elicit horizontal saccades; binocular recordings were made with the IRIS device. For subjects with small strabismus, saccades were disconjugate (unequal between the two eyes) typically by 1 deg. …

Components of visual acuity loss in strabismus.

Strabismus, the misalignment of the visual axis of one eye relative to that of the other eye, reduces visual acuity in the affected eye. Several processes contributing to that loss are: amblyopia, which results in a chronic acuity loss whether or not the fellow eye is viewing; strabismic deviation, which shifts the image of an acuity target onto more peripheral, and therefore less acute, retina when the fellow eye fixates; interocular suppression and binocular masking, which reduce visibility in the strabismic eye due to neural influences from the other eye. We measured the losses due to these…
