CD46 Contributes to the severity of group A streptococcal infection.

Streptococcus pyogenes (group A Streptococcus) is a human pathogen that causes a wide variety of diseases ranging from uncomplicated superficial infections to severe infections such as streptococcal toxic shock syndrome and necrotizing fasciitis. These bacteria interact with several host cell receptors, one of which is the cell surface complement regulator CD46. In this study, we demonstrate that infection of epithelial cells with S. pyogenes leads to the shedding of CD46 at the same time as the bacteria induce apoptosis and cell death. Soluble CD46 attached to the streptococcal surface, sugge

Neuropsychiatric movement disorders following streptococcal infection.

The aim of this study was to describe post-streptococcal movement disorders that form part of the acute rheumatic fever complex. The clinica … h African context, it is important to delineate neuropsychiatric movement disorders associated with streptococcal infections. The potential genetic susceptibility should be explored.

Multifocal myoclonus following group A streptococcal infection.

Movement disorders as postinfectious manifestation of group A streptococcal infections have been reported and are thought to occur on an autoimmune basis. We describe an unusual case of multifocal myoclonus following strep throat infection. Clinical description and chart review were the method used. A 10-year-old boy developed focal myoclonus involving his right arm and shoulder 1 week after streptococcal throat infection treated with penicillin. His magnetic resonance image was normal, and he initially responded to clonazepam but did not sustain a response. The myoclonus spread to involve all…

Multiphasic disseminated encephalomyelitis associated with streptococcal infection.

Multiphasic disseminated encephalomyelitis (MDEM) is a rare peculiar diagnosis which is defined as acute demyelinating central nervous system disease with relapses occurring only within 4 weeks of initial manifestation. This report describes the case of a 6-year-old boy with MDEM diagnosed by clinical findings and magnetic resonance imaging. The disease had a biphasic evolution, and with a second course of high-dose corticosteroids a complete recovery without further relapse was obtained during the following 18 months. Serological evidence of streptococcal infection as specific trigger for MDE

Juvenile relapsing periarteritis nodosa and streptococcal infection.

Classic polyarteritis nodosa is a multisystem inflammatory disease associated with necrotizing vasculitis of small and medium arteries. In most cases, the causes of polyarteritis nodosa remain unknown, but viruses (HBV, HCV, HIV) and microbes (especially streptococcus) have been considered as etiologic or contributing factors. A 13-year-old boy was admitted with fever, skin lesions, polyarthritis and muscle involvement. A muscle biopsy demonstrated a necrotizing vasculitis and antistreptolysin titre was tremendously increased. His condition improved following the administration of oral steroid

Childhood glomerulonephritis associated with varicella and streptococcal infection.

Varicella is a usually benign disease of childhood and its complications are uncommon in immunocompetent children. In recent years we have witnessed the increasing virulence of group A beta-haemolytic streptococci (GABHS). In particular, in 1993, 50% of new cases of invasive GABHS disease were associated with varicella infection and all were suppurative complications. Because also a non suppurative complication of varicella as glomerulonephritis associated with GABHS infection, has been published in only one case, we feel that it could be of interest to describe this condition in two other cas

Erythema nodosum associated with streptococcal infection in pregnancy.

BACKGROUND: Erythema nodosum (EN) is a condition characterized by the presence of painful erythematous nodules on the pretibial aspects of the lower extremities. EN is thought to be a local inflammatory, immune-mediated reaction to a number of systemic antigenic stimuli. This condition is noted most often in women between menarche and menopause and is associated with certain drugs, infections, and pregnancy. However, no reports in the literature describe EN as a result of streptococcal infection during pregnancy. CASE: A 21-year-old, white woman, G(3)P(0020), presented at 13 weeks gestation w

The role of streptococcal infection in the initiation of guttate psoriasis.

BACKGROUND AND DESIGN: Although the association between streptococcal infection and guttate psoriasis is well known, to date there has been little information on whether only limited groups and/or serotypes of beta-hemolytic streptococci are involved. One hundred eleven patients with a sudden onset or deterioration of psoriasis were investigated for evidence of streptococcal infection. Of these patients, 34 had acute guttate psoriasis, 30 had a guttate flare of chronic psoriasis, 37 had chronic plaque psoriasis, and 10 had other types of psoriasis. RESULTS: Serologic evidence of recent strept

[Changes in the complement system in acute streptococcal infection].

Results are reported of a study of relationship of complement level, unspecific adaptative reactions, number of circulating immune complexes and some cellular parameters of the immunogram in patients with different forms of streptococcal infection. Different changes were found in acute and chronic forms of the disease. The possible mechanisms of development of these disorders are discussed.

[The rapid diagnosis of acute streptococcal infection in children].

The present work deals with trials of the method of rapid diagnosis of streptococcal infection, carried out in children's infectious hospital, with the use of a new diagnostic kit. The proposed diagnosticum has proved to be highly sensitive and specific in scarlet fever and tonsillitis. The sensitivity and specificity of the diagnosticum depend on the duration of the disease, prehospital treatment and the quality of the bacteriological analysis.

Epidemiology of streptococcal infection with reference to rheumatic fever.

Antistreptolysin antibodies were estimated in 787 normal children and young adults by latex test. This test detects titres of 200 IU/ml and above, which is the western cut off point, for diagnosis. Children below one year showed no antibodies. Unlike western studies where no antibodies are detected below the age of 3 years, our study revealed that 7.9% children between 1-3 years had significantly elevated antibodies. This epidemiological pattern is well reflected in the different clinical profile of younger children developing rheumatic heart disease in our country. Antibodies progressively in

[Secondary immunopathology in patients with an acute streptococcal infection].

The authors studied the activity of complement, nonspecific adaptive reactions of the body, the presence of antibodies to cross-reacting antigens of Streptococcus, the concentration of circulating immune complexes, some cellular characteristics of the immunogram in patients with different forms of streptococcal infection. The changes revealed allow a conclusion about the presence of autoimmune processes, most pronounced in chronic and recurrent forms of diseases. In the formation of streptococcal infection relapses, of the role is pathology of the complement system in combination with disorder

Community outbreak of invasive group A streptococcal infection.

In a two-month period, three cases of septicaemia and one of suppurative tenosynovitis caused by a single strain of Lancefield group A streptococcus, serotype T/M/R 3/3/3, occurred in the district of Nairn (population: 10,239). A primary portal of entry for the organism could not be identified in any of the cases. Laboratory isolates from all four patients were highly susceptible to beta-lactam antibiotics, but prompt treatment failed to prevent the deaths of two previously healthy adults. Group A streptococci of M-type 3 have not previously been associated with outbreaks of invasive infection
