Somnolence and stuttering as the primary manifestations of a midbrain stroke.

BACKGROUND: Stroke can occasionally manifest with non-lateralizing findings such as somnolence and stuttering. We describe a case and discuss the anatomical and physiological implications of this rare combination of symptoms. CASE REPORT: A 51-year-old woman presented with 3 days of "feeling drunk". She could further specify her symptoms as blurry vision, slurred speech, and gait instability. She had a history of hypertension and hyperlipidemia. Her examination at presentation was remarkable only for marked somnolence. Over the next several hours she developed mild upgaze limitation and verti…

Recent advances in the treatment of stuttering: a theoretical perspective.

UNLABELLED: Prolonged speech and its variants are widely used in the behavioral treatment of stuttering. Unlike these approaches, which depend on clinician-prescribed speech pattern changes, two behavioral treatment regimens, one for children and another for adults, recently developed at the Australian Stuttering Research Center, promote self-monitoring of speech as a means of controlling stuttering. In these programs, the clients themselves modify their speech in subtle and variable ways to gain control over stuttering and, in that, they appear to be similar to a well-known experimental techn…

Temperamental characteristics of young children who stutter.

The purpose of this investigation was to assess the temperamental characteristics of children who do (CWS) and do not (CWNS) stutter using a norm-referenced parent-report questionnaire. Participants were 31 CWS and 31 CWNS between the ages of 3;0 (years;months) and 5;4 (CWS: mean age = 48.03 months; CWNS: mean age = 48.58 months). The CWS were matched by age (+/- 4 months), gender, and race to the CWNS. All participants had speech, language, and hearing development within normal limits, with the obvious exception of stuttering for CWS. Children's temperamental characteristics were determined u…

Commentary on the Lidcombe Programme of early stuttering intervention.

Dr Onslow's brief articles and his lecture tour in the UK generated interested in the UK because the Lidcombe Programme appeared to offer the possibility of a new, simple and effective package for use with young stammering children. This commentary has sought to examine these aspects more objectively by careful consideration of the paper submitted to this Journal and others. It would seem of great importance to reflect on the theoretical issues which have been covered as well as the relative complexity of the Lidcombe Programme compared with the approaches that are in common usage now in speci

Speech outcomes of a prolonged-speech treatment for stuttering.

It has been shown that people who stutter can speak with greatly reduced stuttering after treatments that use variations of Goldiamond's (1965) prolonged-speech (PS). However, outcome research to date has not taken account of several important issues. In particular, speech outcome measures in that research have been insufficient to show that lasting relief from stuttering has been achieved by clients outside the clinic for meaningful periods. The present study used extensive speech outcome measures across a variety of situations in evaluating the outcome of an intensive PS treatment (Ingham, 1…

A longitudinal study of stuttering in children: a preliminary report.

The objectives of this pilot study were to establish methods for longitudinal research of stuttering in children and to provide preliminary data on the variations that occur in disfluencies during the developmental course of stuttering. Twenty-seven preschool-aged children were followed for a minimum of 2 years shortly after they began stuttering. Tape-recorded speech samples were obtained from the children at several intervals during this period. The number of various types of disfluencies was counted in the speech samples obtained in each testing period. Twenty-one children continued to be f…

[Palilalia and acquired stuttering in a case of Parkinson's disease].

We report palilalia and acquired stuttering in a 60-year-old Japanese male with Parkinson's disease. At the age of 54, he presented with resting tremor in the hand and foot on the left, and gradual slowness in voluntary movements. Two years later, resting tremor involved the right foot, and an expressionless face and frozen gait occurred. A diagnosis of Parkinson's disease was made and treatment with L-dopa and carbidopa resulted in conspicuous improvement. At the age of 57, he developed compulsive repetitions of syllables, words and phrases, and sentences infrequently when he spoke. They have…

Neurogenic stuttering as a manifestation of stroke and a mask of dysphonia.

R. L. was a 52-year-old man who was referred for an SLP consultation to determine the nature of his fluency disorder, whether or not treatment would be beneficial, and finally whether resumption of pre-trauma vocational status was feasible. The patient was involved in a motor vehicle accident with no resulting detectable trauma. However, shortly after the accident, R. L. developed a severe dysfluency that was later described as cortical stuttering. We reviewed the medical and rehabilitation work-up that attempted to determine whether the communication disorder was functional or organic in orig…

Prevalence of stuttering and cluttering in adults with mental handicaps.

A survey (involving questionnaire, screening and assessment procedures) of 793 adults with mental handicaps revealed that 6.3% of the population had ideopathic dysfluencies. Issues in the differential diagnosis between stuttering, cluttering and other dysfluency are considered, and the contaminating factors involved in this diagnosis discussed.

Acoustic and physiological reaction times of stutterers and nonstutterers.

This study investigates reaction times in the fluent speech utterances of stutterers and nonstutterers under several experimental conditions. Twenty adult stutterers and twenty matched controls produced utterances of three lengths--one syllable words, polysyllabic words, and sentences--in two conditions of time pressure (high and low) and two conditions of preparation (delayed and immediate responding) in a reaction-time paradigm. Recordings were made of EMG signals from the orbicularis oris inferior and from the extrinsic laryngeal area, as well as electroglottographic and acoustic signals. M

The regulated-breathing method for stuttering: an experimental evaluation.

The regulated-breathing method of Azrin and Nunn (1974) appears to be one of the most promising treatments of stuttering in adults. Their study, however, has some methodological limitations. In particular no operational definition of stuttering is provided, and all measures of stuttering are based on self-report. The present study is an experimental evaluation of the regulated-breathing method. Thirty-two subjects were randomly assigned either to treatment or to a waiting-list control group. The treatment was completed during one single session of 2-3 hours. Both frequency of stuttering and ra…

Modification of listener-judged naturalness in the speech of stutterers.

This study investigated the effect of regular feedback of listener-judged speech naturalness ratings on the speech of stutterers. Six adult stutterers each participated in a time-series ABA experiment. During the treatment phase the stutterer was instructed to improve a clinician's rating, on a 9-point scale, of the naturalness of each 30-s interval of the stutterer's spontaneous speech. The results indicate that the naturalness ratings and stuttering for 5 of the subjects made favorable changes during the treatment phase. Analyses of the findings show that only some of the naturalness judgmen…

A stuttering-associated Gnptab variant alters fine-motor kinematics.

Precise reach-to-grasp movements rely on complex cortico-basal ganglia and cerebellar circuits, and can be affected by cellular pathway defects. We investigated whether mild Gnptab deficiency, affecting mannose-6-phosphate-dependent lysosomal enzyme targeting, impairs fine motor behaviors in mice. Using the staircase test, high-frame-rate video, and markerless tracking, we assessed reaching and grasping in Gnptab-mutant and control littermates. Our data suggest that Gnptab-mutant mice retrieved about 75% fewer pellets than controls and exhibited abnormal grasping, characterized by increased wr

Abnormal cerebral blood flow in children with developmental stuttering.

BACKGROUND: Stuttering affects approximately 5% of children; however, its neurological basis remains unclear. Identifying imaging biomarkers could aid in early detection. Accordingly, we investigated resting-state cerebral blood flow (CBF) in children with developmental stuttering. METHODS: Pulsed arterial spin labelling magnetic resonance imaging was utilised to quantify CBF in 35 children with developmental stuttering and 27 healthy controls. We compared normalised CBF between the two groups and evaluated the correlation between abnormal CBF and clinical indicators. RESULTS: Compared with …

What works for whom? Multidimensional individualized stuttering therapy (MIST).

PURPOSE: This study reports outcomes from a stuttering therapy approach that combines value and awareness-based elements from Acceptance and Commitment Therapy (ACT) with those of stuttering and speech modification interventions. The approach, entitled Multidimensional Individualized Stuttering Therapy (MIST), includes a combined clinician and client selection of factors across five areas: 1) general breathing patterns and body tension, 2) breathing patterns during speech production, 3) vocal features in speech production, 4) value and mindfulness-based strategies, and 5) general communication…
