BACKGROUND Anti-LGI1 encephalitis is a type of autoimmune limbic encephalitis. This case report elucidates features of anti-LGI1 limbic encephalitis, focusing on clinical findings and outcome as well as on rarely reported sinus arrest and its pathophysiology. CASE REPORT A 49-year-old female patient presented at the Emergency Department (ED) because of twitching and an acute confusional state. Initial neurological examination revealed cognitive disturbance with disorientation, inattention, and amnestic deficits. While in the ED, twitching of the right arm was observed and shortly thereafter sh

Syncope is defined as a transient loss of consciousness due to cerebral hypoperfusion followed by spontaneous recovery. Common causes of syncope include vasovagal syncope, situational syncope, orthostatic hypotension, carotid sinus hypersensitivity, left- and right-sided obstructive cardiac lesions, and cardiac arrhythmias. History and physical examination often provide valuable clues about the underlying etiology of syncope. Admission decisions in the emergency department can be guided by various risk prediction scores. Evaluation of a patient with syncope involves a large battery of diagnost

BACKGROUND: Syncope is a commonly encountered problem in an emergency hospital. Global cerebral hypoperfusion is the final pathway common to all presentations of syncope, but this symptom presentation has a broad differential diagnosis. It is important to identify patients for whom syncope is a symptom of a potentially life-threatening condition.
MATERIAL AND METHODS: We identified adult patients presenting with syncope to the Emergency Department of our hospital from January 2012 to June 2014. Of 590 patients found in the hospital database we further selected 217 patients who met our criteri

Syncope is caused by cerebral hypoperfusion. Most fainting is simply vasovagal syncope and the challenge lies in identifying the few patients who have potentially life-threatening causes of syncope. Syncope patients constitute 1%-2% of emergency department visits and approximately 30%-50% are admitted to hospital. The known causes are vasovagal syncope (50%), and orthostatic hypotension and cardiac syncope (7% each). Structural heart disease is noted in 3%. The 30-day mortality is only 0.7%, and the 30-day adverse outcome is 4.5%. Risk stratification and diagnosis are important. High-risk pati

Syncope describes a sudden and brief transient loss of consciousness (TLOC) with postural failure due to cerebral global hypoperfusion. The term TLOC is used when the cause is either unrelated to cerebral hypoperfusion or is unknown. The most common causes of syncopal TLOC include: (1) cardiogenic syncope (cardiac arrhythmias, structural cardiac diseases, others); (2) orthostatic hypotension (due to drugs, hypovolemia, primary or secondary autonomic failure, others); (3) neurally mediated syncope (cardioinhibitory, vasodepressor, and mixed forms). Rarely neurologic disorders (such as epilepsy,

The rich cardiology literature of the past 100 years in which the most important forms of syncope are described - including vasodepressor syncope, postural orthostatic hypotension, and Morgagni-Stokes-Adams syncope - is fascinating. These conditions were of interest to some of our most astute clinicians who were also excellent writers. We thus have available for review the remarkable description of patients with these entities written by clinicians such as Soma Weiss, Sir Thomas Lewis, Laurence Ellis, David Sherf, Milton Shy, and Glenn Drager. In their detailed clinical descriptions we identif

BACKGROUND: Vasovagal syncope is an acute manifestation of autonomic nervous system dysfunction. This type of syncope is often associated with other dysautonomic expressions such as migraine, gastroparesis, or postural tachycardia syndrome. Autonomic nervous system dysfunction has been proposed as a key element in the pathogenesis of fibromyalgia.
OBJECTIVES: The objectives of this study were to estimate the frequency of fibromyalgia in a sample of patients with vasovagal syncope and also to correlate the presence of syncope and fibromyalgia with different dysautonomic manifestations.

Pediatric syncope accounts for 3% of the consultations in emergency department. It is frequent in pediatrics, particularly in teenager girls between 15 and 19 years old, and up to a 25% of them have suffered from a syncopal episode. Most causes are benign, but the aim in its initial evaluation is ruling out those which mean any danger. Heart disease can lead to syncope in a 10-28% of cases. A detailed medical history and an exhaustive exploration are enough to rule out organic pathology in most situations. We present the case of a diffuse large B cell lymphoma located in the mediastinum diagno

Syncope and palpitations are common complaints that all physicians confront during daily clinical practice. Single center and multicenter cohort studies have found that syncope accounts for 1%-3% of emergency department evaluations and that palpitations are the primary symptom for approximately 16% of patients who arrive at an outpatient clinic with a cardiac complaint. For both conditions, women make up approximately 60% of the cohorts. In general, the evaluation of both syncope and palpitations can be challenging because of the heterogeneity of causes and, consequently, the variability of cl

Vasovagal syncope (VVS) is characterized by sudden hypotension and bradycardia, due to inhibition of the sympathetic system and activation of the vagal system, respectively. Major lines of evidence suggest that classical (emotional and orthostatic) VVS is not a disease, but a characteristic of the individual. It is, therefore, interesting to investigate the factors that can explain its origin and evolution and, to this purpose, we investigated the available literature data on the vasovagal reflex in animals, including humans. We found two processes in vertebrates, which appear relevant to the

BACKGROUND: Vasovagal syncope (VVS) is the most common type of syncope. Despite its benignity, quality of life may be severely affected in a significant proportion of highly symptomatic patients.
AIM: To review achral knowledgement concerning vasovagal syncope, to assess the utility of diagnostic investigation and report the updat therapeutic management.
METHODS: An electronic search of the relevant literature was carried out using medline. Key words used for the final search were "Vasovagal syncope" "pathophysiology" "diagnosis" "tilt-table testing", "imlantableloop recoder", "treatment", "

Cough syncope occurs primarily in middle-aged male smokers with chronic obstructive pulmonary disease (COPD). It has also been described in children with asthma. I report the case of a 34-year-old nonsmoker who had syncope due to coughing, and who also related a 1-year history of cough with wheezing. Chest examination revealed diffuse wheezing and a prolonged expiratory phase, and pulmonary function testing with a methacholine challenge confirmed hyperreactive airways. Notably, while undergoing spirometric testing he had a recurrent syncopal episode. His syncope resolved with medical therapy f

Syncope is the sudden loss of consciousness and postural tone resulting from an abrupt, transient cerebral malfunction, followed by spontaneous recovery. It is common among adolescents and is usually due to a benign neurocardiogenic (vasovagal) etiology. Rarely, syncope is premonitory of sudden death. The physician must be knowledgeable about the characteristics of neurocardiogenic syncope and what distinguishes it from life-threatening causes. Evaluation of syncope should be based on a complete personal and family history, a thorough physical examination, and an electrocardiogram. Risk factor

The optimal approach to the adolescent with syncope relies on a good working knowledge of its varied etiology; specifically, knowledge of the most common and generally benign type, vasovagal syncope, as well as the important distinguishing characteristics of the more serious and life threatening forms of cardiac syncope. A thorough history and physical examination are the cornerstones of investigation and will suggest the diagnosis in the vast majority of cases. Screening electrocardiography complements the history and physical, and is important in ruling out symptomatic arrhythmias impossible

To assess the role of echocardiography in patients with syncope, we studied 439 consecutive adults who were referred over a 7-year period. Of all patients, 251 (57.2%) were women, who were generally younger than male referrals. All 439 patients had interpretable echocardiographic studies. Results showed that of patients younger than 40 years of age, 72.8% had scans that were normal and 18.5% had mitral valve prolapse (MVP). All (other) patients in this age group with abnormal echocardiographic findings (8.7%) had either abnormal physical findings over the heart, an abnormal electrocardiogram,

Neurocardiogenic syncope is a collective term used to describe the clinical syndromes of syncope that result from inappropriate, and often excessive, autonomic reflex activity, and manifest as abnormalities in the control of vascular tone and heart rate. These include carotid sinus syndrome, vasovagal syncope, and the syndromes of cough, deglutition, and micturition syncope. Orthostatic hypotension, which, in contrast, results from a failure of autonomic reflexes, is not considered part of this family of closely related syndromes. This review will focus on vasovagal and carotid sinus syndromes
