Deformity of the calcaneocuboid joint in patients who have talipes equinovarus.

A retrospective analysis was done of the records and radiographs of 100 club feet (in sixty-six patients) that had been consecutively treated with an operation. Twenty-six feet (26 per cent) had had a Grade-II deformity of the calcaneocuboid joint, as determined with a radiographic classification that had been developed on the basis of the degree of medial displacement of the cuboid. When the calcaneocuboid joint is in normal alignment, the central point of the cuboid ossification center lies on the mid-longitudinal axis of the calcaneus; when there is a Grade-I deformity, the mid-point of the

A single-gene explanation for the probability of having idiopathic talipes equinovarus.

It has been hypothesized that the pathogenesis of idiopathic talipes equinovarus (ITEV, or clubfoot) is explained by genetic regulation of development and growth. The objective of the present study was to determine whether a single Mendelian gene explains the probability of having ITEV in a sample of 143 Caucasian pedigrees from Iowa. These pedigrees were ascertained through probands with ITEV. Complex segregation analyses were undertaken using a regressive logistic model. The results of these analyses strongly rejected the hypotheses that the probability of having ITEV in these pedigrees was …

Talipes equinocavovarus deformities corrected with the aid of a hinged-distraction apparatus.

In 56 patients (70 feet) with talipes equinocavovarus, various components of deformity were surgically corrected in multiple stages with the aid of a hinged-distraction apparatus and concurrent training and rehabilitation of motion in the ankle joint. Good results were obtained in 53 feet, satisfactory results in 13 feet, and unsatisfactory results in four feet. The complications brought on by apparatus-assisted therapy of talipes equinocavovarus deformities require additional treatment but do not depreciate the method.

Anatomy of uncorrected talipes equinovarus in a fifteenth-century American Indian.

Although congenital talipes equinovarus is the most common major musculoskeletal malformation in contemporary populations, its occurrence in archaeological specimens is rare. The deformity of untreated clubfoot in an adult American Indian male is discussed.

Talectomy in the treatment of resistant talipes equinovarus deformity in myelomeningocele and arthrogryposis.

Eighteen patients (28 feet) with spina bifida and arthrogryposis had talectomy for correction of severe equinovarus deformity. In 26 feet, posteromedial release had been performed previously. The average age at surgery was 4 1/2 years, with a range of 1-9 years of age. The length of follow-up averaged 4 years and ranged from 12 months to 8 years. Twenty-three feet were rated good, and five were poor. Six feet, because of forefoot adduction, required further surgery (metatarsal osteotomy). Talectomy is an effective procedure for correction of hindfoot deformity. Forefoot problems must be treate

Dysplasia and the effects of soft tissue release in congenital talipes equinovarus.

The anatomic abnormalities and the effects of serial release of soft tissue are described in seven stillborn fetal clubfeet. The essential anatomic abnormality is a dislocation of the talocalcaneonavicular joint, with dysplastic changes in the talus and less severe changes in the adjacent bones. By gradually releasing the structures contributing to the tether, the amount of correction of deformity was equated to three current types of surgical procedures: posterior release, posteromedial release, and a combined full subtalar, midtarsal, and posteromedial release. In severe deformities, only by

Nine patients with Xp22.31 microduplication, cognitive deficits, seizures, and talipes anomalies.

Comparative genomic hybridization (CGH) arrays have significantly changed the approach to identifying genetic alterations causing intellectu … analysis in the course of genetic workup for developmental delay, associated with a combination of talipes anomalies, seizures and/or feeding difficulties. The size of the Xp22.31 duplications ranged from 294 kb to 1.6 Mb. We show a comparison of the breakpoints, inheritance and clinical phenotype, and a review of the literature. This clinically detailed …

SOX9 overexpression plays a potential role in idiopathic congenital talipes equinovarus.

The collagen, type IX, alpha 1 (COL9A1) gene was previously identified as a candidate gene for idiopathic congenital talipes equinovarus (ICTEV), a congenital lower limb deformity in humans. In the present study, increased expression levels of COL9A1 were identified in the abductor hallucis muscle of ICTEV patients compared with those in control samples. The COL9A1 gene is regulated by SRY (sex‑determining region Y)‑box 9 (SOX9). Immunofluorescence analysis of SOX9 and COL9A1 proteins identified colocalization to the sarcolemma, endomysium and muscle membrane in muscle samples of ICTEV. No mut…

[Mechanism of GLI3 gene transcription regulation in idiopathic congenital talipes equinovarus].

OBJECTIVE: To investigate the mechanism of transcription regulation of GLI3 gene in idiopathic congenital talipes equinovarus. METHODS: pGL3-Gli3 luciferase report vectors were constructed, and the activity of Gli3 promoter was explored. A P-Match software was used to analyze the sequence upstream of the transcription start site of rat Gli3 gene, which was subsequently verified with chromatin immunoprecipitation assay (CHIP) and electrophoretic mobility shift assay (EMSA). Expression of the Gli3 gene was analyzed in L6 cells transfected with Hoxd13 small interference RNA(siRNA) and Hoxd13 exp…

[Analysis of association between COL9A1 gene and idiopathic congenital talipes equinovarus].

Genotypes of 2 SNPs(rs592121 and rs1135056) within COL9A1 gene in 84 ICTEV nuclear pedigrees were analyzed by restriction fragment length polymorphism and DNA sequencing. Analysis of association between SNP locus and ICTEV was performed using ETDT software. Haplotypes and their frequencies in 84 nuclear pedigrees were established and analyzed by TRANSMIT software. RT-PCR was used to detect the expressions of COL9A1 mRNA in 25 patients with ICTEV. The results showed that rs592121 and rs1135056 loci within COL9A1 gene existed transmission disequilibrium in 84 nuclear pedigrees (P<0.05). Expressi

Surgical correction of resistant talipes equinovarus: observations and analysis - preliminary report.

The preliminary results of a comprehensive talipes equinovarus release on 16 patients with 20 idiopathic clubfoot deformities were analyzed with an 18-month follow-up. Our conclusions were that each component of the clubfoot can be managed in a single stage operative procedure without any significant risk to the patient; bilateral surgery could be done at the same sitting; and dorsiflexion was difficult to achieve because of the anatomical deformity of the talus and resistant anterior capsule. Fixation after comprehensive release with Kirschner wires is important in maintaining the reduction, …

Fetal congenital talipes equinovarus: genomic abnormalities and obstetric follow-up results.

OBJECTIVE: The etiology of congenital talipes equinovarus (CTEV) is unknown, and the relationship between chromosome microdeletion/microduplication and fetal CTEV is rarely reported. In this study, we retrospectively analyzed fetal CTEV to explore the relationship among the CTEV phenotype, chromosome microdeletion/microduplication, and obstetric outcomes. METHODS: Chromosome karyotype analysis and single nucleotide polymorphism (SNP) array were performed for the 68 fetuses with CTEV. RESULTS: An SNP array was performed for 68 fetuses with CTEV; pathogenic copy number variations (CNVs) were d…

Congenital talipes equinovarus and congenital vertical talus secondary to sacral agenesis.

Sacral agenesis is a rare congenital defect which is associated with foot deformities such as congenital talipes equinovarus (CTEV) and less commonly congenital vertical talus (CVT). We report a 3-year-old Caucasian girl who was born with right CTEV and left CVT secondary to sacral agenesis. Her right foot was managed with a Ponseti casting method at 2 weeks, followed by an Achilles tenotomy at 4 months. The left foot was initially managed with a nocturnal dorsi-flexion splint. Both feet remained resistant and received open foot surgery at 10 months producing plantigrade feet with neutral hind…

Risk factors for isolated talipes equinovarus in Western Australia, 1980-1994.

A case-control study based on routinely collected data for a West Australian (WA) cohort of births born 1980-94 inclusive was used to identify potential risk factors for the foot deformity isolated talipes equinovarus. Race and sex were identified as risk factors and risk modifiers of the condition in WA infants. In comparison with their female counterparts, Aboriginal males were at greater risk of the deformity than Caucasian males (odd ratio [OR] 7.15, 95% confidence interval [CI] 2.68, 21.12 vs. OR 1.63, 95% CI 1.24, 2.15 respectively). Sex-specific risk estimates of the deformity showed th…
