Tendon transfer to restore extension of the thumb using abductor pollicis longus.

20 patients with rupture or division of extensor pollicis longus (EPL) underwent tendon transfer using abductor pollicis longus (APL). All patients except one were reviewed and examined between 6 and 80 months after surgery. The subjective and objective results were satisfactory and are presented with details of the technique and its advantages and limitations.

Early restoration of pinch grip after ulnar nerve repair and tendon transfer.

Restoration of pinch in ulnar nerve lesions may be achieved within three or four weeks by a one-stage operation combining nerve repair and tendon transfer. We have been using this approach for nerve injuries of the upper limb since 1961, to meet the socio-economic needs of patients who would otherwise need splints and long-term physiotherapy. In our experience, the procedure of choice is transfer of E.I.P. to adductor pollicis and E.P.B. to the first dorsal interosseous. The operative technique and its results in 18 patients followed up for 37-515 days are discussed.

Team physician #2. Peroneus brevis transfer for Achilles tendon rupture in athletes.

We feel peroneus brevis transfer is especially advantageous for patients interested in sports. We have used this technique in 55 athletes, and to date there have been no re-ruptures (Table 1). Two patients had mild superficial wound problems which did not affect the end result. This method has advantages in comparison with nonoperative treatment and other operative techniques. The danger of re-rupture is avoided and loss of strength minimized because the tendon transfer adds power to the injured triceps. In addition, the proximal retracted triceps is restored to length and securely repaired un

Dual-Tendon Transfer for Chronic Extensor Pollicis Longus Ruptures: Augmented Extensor Indicis Proprius Transfer with Proximal EPL Stump Lengthening.

BACKGROUND: Extensor indicis proprius (EIP) transfer augmented with proximal extensor pollicis longus (EPL) stump lengthening restores thumb extension and optimizes function in cases of chronic EPL tendon ruptures, which impair hand dexterity and fine motor skills. Traditional EIP-to-EPL transfers often disrupt the natural oblique course of the EPL around the Lister tubercle, leading to functional deficits1-3. This dual-tendon transfer preserves anatomical alignment and improves thumb biomechanics, enhancing extension strength and the adduction moment arm at the carpometacarpal (CMC) joint. D

Endoscopic versus open flexor hallucis longus tendon transfer for reconstruction of chronic achilles tendon rupture: A prospective randomized pilot trial.

BACKGROUND: Chronic Achilles tendon rupture often requires surgical reconstruction. Flexor hallucis longus (FHL) tendon transfer is widely used, and endoscopic techniques may reduce soft-tissue morbidity and facilitate early recovery. METHODS: Thirty patients with chronic Achilles tendon rupture were randomized to undergo open or endoscopic FHL tendon transfer. Functional outcome was assessed using the Foot and Ankle Ability Measure (FAAM) over 18 months. Secondary outcomes included operative time and wound complications. Exploratory outcomes included postoperative pain assessed by visual ana

Combined Achilles Tendon-bone Block Allograft and Flexor Hallucis Longus Tendon Transfer for Long Segment Defects Involving the Insertional Region.

Long-segment disease involving the insertional region of the Achilles tendon can be a challenging problem. These patients often have significant disability and functional problems. Surgical management requires some type of tissue replacement, regional tissue rearrangement or tendon transfer. Various types of allograft tissue as well as synthetic materials have been described. Patients often have residual weakness and functional deficits following surgery. We have utilized an Achilles tendon-bone block allograft in combination with a flexor hallucis longus tendon transfer for long-segment Achil

Endoscopic Flexor Hallucis Longus Tendon Transfer for Reconstruction of the Achilles Tendon Rupture in High-Risk Patients: A Case Series.

The neglected Achilles tendon rupture requires surgical reconstruction for the best functional outcome. According to the current literature, there are many reconstructive options available that demonstrate acceptable functional results in most cases. These procedures require large incisions, leading to potential wound-healing complications. Therefore, these procedures may not be suitable for patients who are at high risk for wound-healing problems. A minimally invasive approach is desirable in this situation to decrease the risk of this potential complication. Endoscopic transfer of the flexor

Single-stage reconstruction of achilles tendon rupture with flexor hallucis longus tendon transfer and simultaneous free radial fasciocutaneous forearm flap.

Reconstruction of the Achilles tendon is challenging but critical for successful ambulation. The goals of Achilles tendon reconstruction are to restore power of plantar flexion, restore normal range of movement of the ankle joint, and ensure durable and pliable soft tissue coverage. We present a dehiscence of the Achilles tendon and partial soft tissue loss secondary to infection. Simultaneous Achilles tendon reconstruction with flexor hallucis longus (FHL) tendon transfer and soft tissue reconstruction with free radial forearm flap was performed as a single-stage procedure. The FHL transfer p

Principles and biomechanics of muscle tendon unit transfer: application in temporalis muscle tendon transposition for smile improvement in facial paralysis.

Muscle tendon unit (MTU) transfer is a common procedure performed to restore hand function after peripheral nerve or spinal cord injury. The principles of MTU transfer established for hand surgery can be adopted to optimize the dynamic excursion of the temporalis tendon transfer procedure for facial reanimation. Additionally, the force generating ability of a transferred MTU depends on the ideal length-tension relationship of the donor muscle. There are unclear guideline for selecting the ideal tension at which a transferred MTU will generate maximum force and excursion and current practice of

[Intramuscular lengthening and range of motion after local tendon transfer for repair of retracted supraspinatus tendon defects. A biomechanical study].

BACKGROUND: The current cadaver study was performed to evaluate and compare infraspinatus (ISP) vs. subscapularis (SSC) tendon transfer used for the repair of a large supraspinatus (SSP) rotator cuff defect. The main outcome measures were the range of motion and intramuscular length changes during motion. METHODS: Thirteen fresh-frozen cadaver shoulders were used in the study. A supraspinatus defect extending to the apex of the humeral head (Patte size II) was created. Transosseous repair was then attempted with the ISP and with the SSC in all cases; intramuscular length changes were measured

Tendon Transfer Versus Tendon Graft Reconstruction of Flexor Pollicis Longus Attritional Ruptures Secondary to Volar Plating of Distal Radius Fractures.

BACKGROUND: Attritional rupture of the flexor pollicis longus (FPL) after volar plating of distal radius fractures is an uncommon but morbid complication, secondary to plate prominence at or distal to the watershed zone. No consensus exists regarding the optimal reconstruction, as both tendon grafting and tendon transfer are routinely utilized. This study evaluated clinical outcomes of tendon transfer versus tendon grafting for FPL rupture following volar plate fixation and compared presenting features, radiographic prominence, and complications between cohorts. METHODS: A retrospective revie

Split tendon transfer of the posterior tibialis for spastic equinovarus foot deformity: Does tendon routing impact post-operative ankle kinematics?

BACKGROUND: Split tendon transfer of the posterior tibialis (SPOTT) is a surgical procedure in which the split posterior tibialis tendon is transferred posterior to the fibula (PO) with insertion on the peroneus brevis tendon to rebalance the forces across the hindfoot. Routing of the split tendon through the interosseous membrane (IO) is a variation with the potential benefit of augmenting ankle dorsiflexion in swing. RESEARCH QUESTION: Does IO routing improve ankle dorsiflexion in swing and/or varus in stance compared to PO routing? METHODS: A retrospective chart review was completed to id

Tendon transfer for radial nerve palsy: a single tendon to restore finger extension as well as thumb extension/radial abduction.

Since 1994, the author has been treating irreparable radial nerve palsy with pronator teres to the extensor carpi radialis brevis (for wrist extension) and a single tendon (flexor carpi radialis or ulnaris) transfer to restore finger extension as well as thumb extension/radial abduction. We sought to investigate whether these patients are able to flex the fingers with the thumb in abduction/extension posture. This was a prospective study over a 5-year period, and the results of this transfer in 15 consecutive patients (mean age 28 years) were analyzed. At final follow-up (mean 30 months), all

Early postoperative recovery comparisons of superior capsule reconstruction to tendon transfers.

BACKGROUND: The management of massive posterosuperior rotator cuff tears is controversial, with no gold standard. Two recently developed techniques that have shown promising initial results include arthroscopic superior capsular reconstruction (SCR) and tendon transfers (latissimus or lower trapezius). However, there remains a scarcity of studies examining each procedure's early postoperative clinical outcomes individually or in comparison to each other. The purpose of this study is to compare the early postoperative recovery outcomes of tendon transfers (TTs) to SCR. METHODS: Using the surgi

Variants of latissimus dorsi with a perspective on tendon transfer surgery: an anatomic study.

BACKGROUND: The latissimus dorsi (LD) is often used for tendon transfers to treat massive irreparable posterosuperior rotator cuff tears. The operation requires the LD tendon to be mobilized to reduce tension on the tendon. In that respect, any connection between the LD tendon and contiguous muscles may hamper tendon mobility and affect the surgical outcome. The goal of this study was to document the occurrence of connections between the LD and adjacent muscles and nerves. METHODS: We studied the scapular region on 48 embalmed cadavers. The skin and superficial fascia were removed according t

[Results of extensor indicis transfer for reconstruction of the extensor pollicis longus tendon].

Rupture of the extensor pollicis longus tendon is a frequent complication after distal radius fractures or other traumatic and non-traumatic events. Typical is the loss of the function of extension in the thumb. The extensor indicis transfer is a simple and effective technique for reconstruction of the extension with a low donor-site morbidity. We report on 38 patients with extensor indicis transfer. Postoperatively the patients received a dynamic motion splint for 4 weeks. For evaluation we have done pre- and postoperative ultrasound scans of the thumb, evaluated the DASH score and carried ou
