BACKGROUND: The use of diaphragmatic pedicle flaps for reconstructive procedures in thoracic surgery is not very popular. Nevertheless, it provides considerable advantages.
METHODS: Our experience covers 10 years (1987-1997) with a total of 25 patients in whom the diaphragmatic flap was used for different purposes. In 6 patients we used the diaphragmatic flap to protect the bronchopleural fistula at its early onset, which was not beyond 12 hours from the clinical diagnosis. We performed prophylactic suture protection after neoadjuvant therapy in 9 high-risk patients who underwent pneumonectom…

OBJECTIVE: To review our experience using antero-superior approaches for resection of a heterogeneous group of tumors, both benign and malignant, involving the thoracic inlet and adjacent structures. These included Pancoast type bronchial carcinomas, primary neurogenic tumors, soft-tissue neoplasms, and metastases from a variety of primary sites.
METHODS: Between October 1993 and January 1998 we undertook 22 operations on 21 patients using a variety of antero-superior approaches. The anterior cervical-transsternal approach was used in 11 operations, the Dartevelle technique was used in five c

OBJECTIVE: This is a prospective study to evaluate the indications and outcome of the hemiclamshell incision (longitudinal partial sternotom … utcome (clinical examination and pulmonary function testing) in order to validate this incision for thoracic surgery.
RESULTS: 25 patients (15 men, 10 women) with an age ranging from 16 to 73 years (mean 43 years) underwent a hemiclamshell incision. The indications for the hemiclamshell approach were (1) chest trauma with massive hemorrhage requiring urgent acces …

The authors present their experience with surgical treatment of achalasia of the oesophagus in child age. During a nine-year period (1984-1992) they operated 11 children with achalasia. The group comprised 6 boys and 5 girls. The mean age at the time of operation was 9.5 years. The youngest patient was operated at the age of 14 months and the oldest one at the age of 15 years. The main clinical symptoms at the time of establishment of the diagnosis were: vomiting in 91%, dysphagia in 64%, the children did not thrive and lost weight in 36%; they suffered from relapsing bronchopneumonia in 27%,

OBJECTIVES: The Women in General Thoracic Surgery (WGTS) Academy, organized by the European Society of Thoracic Surgeons (ESTS), supports early and mid-career women thoracic surgeons through mentorship, leadership development, and clinical immersions in a gender-equitable environment.
METHODS: We conducted a prospective cross-sectional survey-based evaluation of all fellows participating in the WGTS Academy between 2023 and 2025. The survey addressed the value of face-to-face meetings, simulation training, and clinical immersions, as well as professional connections, challenges faced, and sug…

BACKGROUND & AIM: Pulmonary echinococcosis, or pulmonary hydatid disease, caused by Echinococcus larvae, presents a significant health challenge, particularly in endemic areas. While surgery is the primary treatment for viable cysts, the optimal approach for this remains under debate. There is a lack of comparative evidence comparing traditional open thoracic surgery (OTS) with minimally invasive video-assisted thoracic surgery (VATS). This systematic review and meta-analysis aims to comprehensively compare the efficacy and safety of VATS versus OTS for pulmonary hydatid disease; it will expan…

This meta-analysis aimed to compare the outcomes of video-assisted thoracic surgery (VATS) and open thoracotomy in pediatric patients undergoing pulmonary metastasectomy for various malignancies. We systematically searched PubMed, Embase, and Cochrane Library databases for studies comparing VATS and open thoracotomy in pediatric patients. The treatment effects for continuous outcomes were compared using mean differences (MDs), and binary endpoints were evaluated using odds ratios (ORs), with 95% confidence intervals (CIs). Heterogeneity was assessed with I2 statistics. Statistical analyses wer…

BACKGROUND: Surgical resection of the thymus is indicated in the presence of primary thymic diseases such as thymoma. Video-assisted thoracoscopic surgery (VATS) and robot-assisted thoracic surgery (RATS) offer a minimally invasive approach to thymectomy. However, there is no clear conclusion whether RATS can achieve an equal or even better surgical effect when compared with VATS in treatment of thymoma. We performed this meta-analysis to explore and compare the outcomes of RATS versus VATS for thymectomy in patients with thymoma.
METHODS: PubMed, Cochrane Library, EMBASE, China National Know…

BACKGROUND AND OBJECTIVE: The year 2025 brought important refinements in thoracic surgery and thoracic oncology, particularly in risk-adapted screening, biomarker-guided perioperative therapy, minimally invasive surgery, and perioperative recovery. This narrative review summarizes clinically relevant evidence reported in 2025 and discusses how these updates may influence multidisciplinary practice.
METHODS: We conducted a narrative review of literature and conference reports published or presented between January 1 and December 31, 2025. PubMed, Google Scholar, and the official abstract proce…

BACKGROUND: While many hospitals in Germany perform thoracic surgery, anesthetic techniques and methods that are actually used are usually only known for individual departments. This study describes the general anesthetic management of three typical thoracic surgical procedures across multiple institutions.
MATERIAL AND METHODS: The German Thoracic Registry recorded 4614 patients in 5 institutions between 2016 and 2019. Hospitals with a minimum number of more than 50 thoracic procedures per year are eligible for inclusion in the registry. To analyze the anesthetic management, a matching proce…

INTRODUCTION: We report the results of a national consensus paper among Italian thoracic surgeons obtained through a Delphi process, on the bleeding in thoracic surgery (BTS) evaluating influencing factors, use of a validated intraoperative bleeding scale (VIBeS), its management and helping improving practice.
METHODS: A panel of 20 statements (a total of 39 issues) was developed and, after initial validation by six experts, was electronically sent to 60 Italian thoracic surgeons. Participants were asked to score each statement on a 5-point Likert scale and the agreement was scored for evalua…

OBJECTIVES: General anaesthesia (GA) carries high risks of ventilator dependency with increased morbidity and mortality in patients with severe respiratory disease. It also presents an ethical dilemma if surgery remains the only treatment option for patients with advanced terminal chronic respiratory disease. Thoracic epidural anaesthesia for awake thoracic surgery (TEATS) in high-risk patients with dyspnoea at rest could avoid ventilator dependency and speed up recovery even in patients with severe dyspnoea. This retrospective observational study analysed indications, management and outcome o…

OBJECTIVE: To investigate the impact of 2 postoperative analgesic regimens on heart rate variability in patients who underwent thoracotomy.
DESIGN: A prospective, randomized trial.
SETTING: A single-institutional study in a university hospital.
PARTICIPANTS: Fifty patients who underwent thoracotomy under combined general anesthesia and thoracic epidural analgesia divided by a number generator into 2 equal groups (A and B).
INTERVENTIONS: In group A, postoperative analgesia consisted of thoracic epidural analgesia with levobupivacaine for 6 postoperative days. In group B, on the 3rd postope

PURPOSE: The essential points of video-assisted thoracic surgery (VATS) for acute thoracic empyema are the decortication of thickened pleura, resection of necrotic tissues and fibrin blocks, and drainage. Pulsed lavage irrigation, which is commonly used in orthopedic surgery as a method of sufficiently performing the technique, was used under a thoracoscope to study the efficacy of the treatment for acute thoracic empyema.
METHODS: The subjects comprised 31 patients who had undergone VATS for acute thoracic empyema. There were 26 men and 5 women with an average age of 60.5 years. For the surg…
