Does right thoracotomy increase the risk of mitral valve reoperation?

OBJECTIVE: The study objective was to determine whether a right thoracotomy approach increases the risk of mitral valve reoperation. METHODS: Between January of 1993 and January of 2004, 2469 patients with mitral valve disease underwent 2570 reoperations (1508 replacements, 1062 repairs). The approach was median sternotomy in 2444 patients, right thoracotomy in 80 patients, and other in 46 patients. Multivariable logistic regression was used to identify factors associated with median sternotomy versus right thoracotomy, mitral valve repair versus replacement, hospital death, and stroke. Facto

Atrial septal defect repair through limited lateral thoracotomy in children.

OBJECTIVE: We have developed a surgical method for atrial septal defect repair through a limited right lateral thoracotomy in which the incision line begins 2 cm caudal from the lower angle of the scapula and ends at the midaxial line, thereby improving patient satisfaction with the cosmetic results of treatment. METHODS: We performed a retrospective review of 28 patients who underwent isolated atrial septal defect repair through a limited right lateral thoracotomy between January 2002 and August 2004. The mean age and mean body weight at the time of the operation were 85.8 months (range 9-23

Repair of atrial septal defects via limited right anterolateral thoracotomy.

INTRODUCTION: Right thoracotomy is a well known alternative for median sternotomy to gain access to the right atrium. The Port-Access technique is a surgical option to reduce the skin incision to 5 cm and have a considerable gain in cosmesis and post-operative rehabilitation. PATIENTS AND METHODS: From February 1997 to May 2000, 50 patients (19M/31F) had Port-Access atrial septal defect repair (ASD) with the Heartport Endo-CPB and Endo-aortic clamp system. Mean age was 46 years (range 10.5-74 years). Forty-nine patients had a type II ASD. Most of the patients were asymptomatic (48 were NYHA c

Transsternal bilateral thoracotomy for pericardiectomy after coronary artery bypass grafting.

Surgery for constrictive pericarditis was conducted through a transsternal bilateral thoracotomy in a 45-year-old man who developed the condition 12 months after coronary artery bypass grafting with left internal thoracic artery and vein grafts. The grafts ran just beneath the sternum. To avoid injury to the bypass grafts during sternotomy and mediastinal dissection, we conducted a transsternal bilateral thoracotomy, which provided excellent exposure of the heart. Complete pericardiectomy was done safely without cardiopulmonary bypass. Constrictive pericarditis following cardiac surgery is an

Bilateral thoracotomy and inferior sternotomy for bypass grafting after esophagostomy.

In the presence of a cervical esophagostomy, a median sternotomy may jeopardize the stoma, risk mediastinal or sternal infection, and potentially compromise future esophageal operations. Coronary artery bypass grafting was performed using a modified bilateral thoracotomy before esophageal reconstruction in a patient with cervical esophagostomy. The surgical technique is described with a review of the relevant literature.

[Postoperative Pain Syndromes After Thoracoscopy and Thoracotomy - Prevention and Management].

BACKGROUND: Despite minimally invasive techniques, chronic pain after thoracic surgery - often referred to as post-thoracoscopy/post-thoracotomy pain syndrome (PTPS) - remains an important clinical challenge. METHODS: This narrative review summarises current evidence on the epidemiology, pathophysiology, prevention and treatment of chronic postoperative pain (CPSP) following thoracic surgical procedures. The focus is placed on analgesic strategies in the context of early mobilisation and fast-track concepts. RESULTS: Acute postoperative pain is the most important predictor of pain chronifica

Resuscitation following emergency room clamshell thoracotomy and cardiac laceration repair.

The resuscitative thoracotomy (RT) is a time-sensitive procedure performed on trauma patients with impending fatality, particularly those with penetrating thoracic injuries. We present a case of a male in his twenties with pulseless electrical activity following a gunshot wound (GSW) to the right chest. This patient experienced rapid haemodynamic deterioration in concurrence with signs of cardiac tamponade, leading to the performance of a clamshell thoracotomy, followed by a pericardiotomy, and subsequent myocardial repair. Direct haemorrhage control, cardiac massage and rapid transfusion achi

Emergency Department Thoracotomy: Time to Stop - A Trauma Video Review.

INTRODUCTION: Emergency department thoracotomy (EDT) can restore spontaneous circulation in selected trauma patients as a bridge to definitive operating room repair. Compared with medical cardiopulmonary resuscitation, there is little guidance on EDT resuscitation duration in trauma. We hypothesized that prolonged EDT resuscitation efforts was not associated with durable return of spontaneous circulation (ROSC) (ROSC with survival to operating room). METHODS: This institutional review board approved study was performed at a level 1 trauma center from January 2022 to January 2023. Trauma bay v

Spinal deformity after thoracotomy in children with pulmonary hydatid disease.

BACKGROUND: Pulmonary hydatid cyst disease is a common zoonotic infection, especially in agricultural and livestock communities. Thoracotomy is an important surgical procedure in the treatment of pulmonary hydatid cysts in children. However, the development of a spinal deformity is one of the long-term musculoskeletal complications of this procedure. The aim of this study was to evaluate the incidence, risk factors and clinical outcomes of spinal deformity in pediatric patients after thoracotomy. METHODS: Between 2008 and 2022, 116 pediatric patients who underwent thoracotomy for pulmonary hy

Right mini-thoracotomy approach for grown-up congenital heart disease.

BACKGROUND: Right mini-thoracotomy cardiac surgery has been recognized as a safe and effective procedure, with remarkable early and long-terms outcomes. However, most of the literature is focused on mitral valve surgery and few studies report on the minimally invasive approach applied to congenital disease. Aim of this study was to review our experience on patients with grown-up congenital heart (GUCH) undergoing right mini-thoracotomy cardiac surgery. METHODS: Data of patients with GUCH undergoing right mini-thoracotomy cardiac surgery from 2006 to 2019 were retrospectively analyzed. Inclusi

Scoliosis after thoracotomy repair of esophageal atresia: a systematic review.

Standard surgical repair of esophageal atresia/tracheoesophageal fistula (EA/TEF) is via a right posterolateral thoracotomy. A recognized complication is the later development of scoliosis. The prevalence and pathogenesis of secondary scoliosis are poorly understood. We, therefore, conducted a systematic review on this topic. All English language articles reporting incidence, outcomes and/or interventions for scoliosis in children after EA repair via thoracotomy were identified. Fourteen relevant articles published between 1969 and 2019 reporting 1338 children were included in the analysis. Th

Winged scapula following axillary thoracotomy with long thoracic nerve preservation.

Winged scapula is a rare condition caused by injuries to the long thoracic nerve (LTN) and accessory nerves. A 69-year-old man underwent surgery for right lung cancer. Video-assisted thoracic surgery was converted to axillary thoracotomy at the fourth intercostal space. The latissimus dorsi was protected, and the serratus anterior was divided on the side anterior to the LTN. Two months after discharge, he presented with difficulty in elevating his right arm and protrusion of the scapula from his back. Active forward flexion of the right shoulder was limited to 110° and abduction to 130°. He wa

Surgical wound-site inflammation: video-assisted thoracic surgery versus thoracotomy.

OBJECTIVES: Mechanical trauma occurring during pulmonary resection through both video-assisted thoracic surgery (VATS) or thoracotomy causes profound alterations in cytokines and the cellular network. The aim of this study was to analyse biological changes occurring in both the microenvironment (wound site) and macroenvironment (systemic circulation) following pulmonary lobectomy via the VATS or thoracotomic approach. METHODS: From October 2016 to July 2017, 30 patients with clinical Stage I lung cancer were recruited. In 12 cases (the VATS group), surgery was performed through a video-assist

Huge mediastinal liposarcoma resected by clamshell thoracotomy: a case report.

BACKGROUND: Liposarcoma is the single most common soft tissue sarcoma. Because mediastinal liposarcomas often grow rapidly and frequently recur locally despite adjuvant chemotherapy and radiotherapy, they require complete excision. Therefore, the feasibility of achieving complete surgical excision must be carefully considered. We here report a case of a huge mediastinal liposarcoma resected via clamshell thoracotomy. CASE PRESENTATION: A 64-year-old man presented with dyspnea on effort. Cardiomegaly had been diagnosed 6 years previously, but had been left untreated. A computed tomography scan

Giant thymoma successfully resected via anterolateral thoracotomy: a case report.

The appropriate surgical approach for a large mediastinal tumor is controversial. Median sternotomy is the standard approach for thymomas. We herein report the case of a giant thymoma, 13 cm in diameter, surgically resected via anterolateral incision. Subsequent thymectomy was performed via thoracoscopy. The resected specimen was a WHO type AB thymoma, Masaoka stage I, without capsular invasion. The anterolateral incision was less invasive and more versatile in the present case, as the incision could be extended to a hemiclamshell or posterolateral incision depending on exposure and relationsh
