The Use of Alloderm® Coverage to Reinforce Tissues in Two-Stage Tissue Expansion Placement in the Subcutaneous (Prepectoral) Plane: A Prospective Pilot Study.

PURPOSE: Two-stage tissue expander (TE) to implant breast reconstruction is commonly performed by plastic surgeons. Prepectoral implant placement with acellular dermal matrix (ADM, e.g., AlloDerm®) reinforcement is evidenced by minimal postoperative pain. However, the same is not known for TE-based reconstruction. We performed this study to explore the use of complete AlloDerm® reinforcement of breast pocket tissues in women undergoing unilateral or bilateral mastectomies followed by immediate, two-stage tissue expansion in the prepectoral plane. METHODS: Patients (n = 20) aged 18-75 years we…

Highly Sensitive Spatial Host-Microbiome Transcriptomics in FFPE Tissues via Iterative Hydrogel Expansion.

Spatial transcriptomics (ST) of archival formalin-fixed paraffin-embedded (FFPE) tissues is fundamentally limited by a trade-off between spatial resolution and transcriptomic sensitivity due to severe molecular crowding and RNA degradation. Here, we present Ex-spRandom, a highly sensitive platform that overcomes this limitation by directly converting diverse, fragmented RNA biotypes into stably anchored cDNA through the integration of random-primed in situ cDNA synthesis chemistry with interpenetrating polymer network (IPN)-based tissue expansion chemistry. This physicochemical synergy physica…

A new strategy for total auricular reconstruction using prelamination of an extended retroauricular flap with tissue expansion.

BACKGROUND AND AIM: To accomplish total ear reconstruction with aesthetic appearance is a great challenge for plastic surgeons worldwide due to insufficient skin coverage. A retroauricular fascial flap and skin graft technique are commonly used, but the results obtained are not satisfactory due to color mismatch, severe edema, and donor-site morbidity. Here, we describe a novel strategy for total ear reconstruction, utilizing an extended retroauricular flap prelaminated with tissue expansion to obtain enough skin for ear reconstruction. METHODS: About 2 months before ear reconstruction, a kid…

Oriented cell divisions induce basal progenitors and regulate neural expansion across tissues and species.

Nervous system expansion relies on progenitor proliferation, yet its regional and evolutionary regulation is incompletely understood. While basally dividing progenitors are implicated in neocortical growth, their developmental origins and relevance beyond the cortex remain unclear. We show here that double inactivation of spindle orientation regulators GPSM2 and SAPCD2 in mice completely reorients progenitor divisions in both the neocortex and retina. This shift increases basal progenitors over sixfold in the neocortex and induces their ectopic emergence in the retina, resulting in extra cell

Filter-aided expansion proteomics for the spatial analysis of single cells and organelles in FFPE tissue samples.

Filter-aided expansion proteomics (FAXP) is a spatial proteomics approach designed for high-resolution analysis of formalin-fixed, paraffin-embedded (FFPE) tissues. Here we describe the integration of hydrogel-based tissue expansion with mass spectrometry, enabling isotropic expansion and robust protein retention while preserving spatial features. The FAXP workflow consists of several sequential steps, including tissue section dewaxing, in situ protein anchoring, hydrogel embedding, homogenization, staining, isotropic expansion, microdissection and filter-aided in-gel digestion to maximize pep…

The effects of soft-tissue expansion on lip repair and midfacial growth in a rabbit cleft lip model.

Midfacial hypoplasia following primary cleft lip repair is a common clinical correlate. Recent experimental work has suggested that increased lip pressure following an undermined lip repair may contribute to midfacial growth inhibition. Soft-tissue expansion has been used to generate additional soft tissue for reconstruction in the cranial region. The use of this technique in the labial region may allow lip repair to be performed with less tension (pressure) and thus facilitate midfacial growth. To test this hypothesis, 40 4-week-old rabbits were randomly divided into four groups. Two groups h…

Pretransfer tissue expansion of a scalp free flap for burn alopecia reconstruction in a child: a case report.

A nine-year-old boy who sustained severe grease-fire burns over his right scalp, face, neck, and trunk 14 months previously, presented with extensive alopecia over the right frontotemporoparietal scalp. Near complete coverage of the scalp defect was obtained with pretransfer expansion of the left temporoparietal scalp, followed by free-flap transfer of the expanded tissue to the defect. Pretransfer tissue expansion has the advantages of increasing flap surface area and minimizing donor site deformity. This case illustrates the applicability of pretransfer tissue expansion of free flaps, and de…

[MRI features of expansive processes of soft tissues. Contribution to etiologic diagnosis. Apropos of 32 cases].

This study concerns the investigation of 32 soft-tissue tumors by MRI, subsequently verified by biopsies or surgery (n = 28) or other investigations (n = 4). MRI can suggest the diagnosis in some cases: lipomas cysts, hematomas, neuromas, desmoid tumors, hemangiomas, by contrast analysis and morphologic aspects; sometimes, malignant lesions are suspected on particular anatomic criteria. MRI is excellent in the evaluation of local extension. MRI can therefore be performed when the staging of soft tissue tumor is incompletely provided by other investigations (CT or US).

Genetic modifiers of somatic expansion and clinical phenotypes in Huntington's disease highlight shared and tissue-specific effects.

An inherited, expanded CAG repeat in HTT undergoes further somatic expansion to cause Huntington's disease (HD). To gain insights into this molecular mechanism, we compared genome-wide association studies of somatic expansion in blood and somatic expansion-driven HD clinical phenotypes. Here, we show that somatic expansion is driven by a mismatch repair-related process whose genetic modification and consequences show unexpected complexity, including cell-type specificity. The HD clinical trajectory is further modified by non-DNA repair genes that differentially influence measures of cognitive

Genetic modifiers of somatic expansion and clinical phenotypes in Huntington's disease reveal shared and tissue-specific effects.

Huntington's disease (HD), due to expansion of a CAG repeat in HTT , is representative of a growing number of disorders involving somatically unstable short tandem repeats. We find that overlapping and distinct genetic modifiers of clinical landmarks and somatic expansion in blood DNA reveal an underlying complexity and cell-type specificity to the mismatch repair-related processes that influence disease timing. Differential capture of non-DNA-repair gene modifiers by multiple measures of cognitive and motor dysfunction argues additionally for cell-type specificity of pathogenic processes. Bey
