Clinical problems in the mixed dentition: traumatized teeth--evaluation, treatment and prognosis.

Injuries to anterior teeth of children seem to be increasing. After conducting a thorough examination it is useful to classify the injury as a guide to treatment. Injury can be classified as crown fracture, crown-root fracture, root fracture, subluxation or loosening of the tooth, luxation or dislocation and, finally, exarticulation. The appropriate form of treatment for each class of injury is discussed and an indication of the expected success rate is given. Absence of a positive response to vitality tests is not by itself sufficient to warrant a diagnosis of pulp necrosis. It must be suppor

Treatment of horizontal root fractures using a triple antibiotic paste and mineral trioxide aggregate: a case report.

This case report describes the treatment of a horizontal root fracture in a maxillary central incisor (tooth #8) using a triple antibiotic paste and mineral trioxide aggregate. A nonsurgical endodontic treatment was performed to a coronal root canal fragment of tooth #8. During the treatment procedure, 1% sodium hypochlorite solution was used for irrigation and a triple antibiotic paste was used as an intracanal medicament. The coronal part of the canal was obturated with mineral trioxide aggregate totally. At follow-up examination after 12 months, the tooth was asymptomatic and radiographical

Rationale and methods for crown lengthening.

Surgical crown lengthening should not be attempted when tooth fractures extend into the middle third of the root. Excessive bone reduction on the fractured tooth and necessary blending of osseous contours over adjacent teeth may result in mobility or furcation involvement. In addition, the necessary osseous surgery may create precipitous changes in the bony architecture that will be bridged by soft tissues. This results in increased pocket depth and an unhealthy situation. If the tooth in question has a hopeless prognosis or the osseous surgical procedure would create a poor crown-root ratio, …

Comparative Evaluation of Two Different Preparation Designs and Materials on the Fracture Resistance of Laminate Veneers: An In Vitro Study.

Introduction Laminate veneers are thin porcelain or composite resin shells bonded to the front of teeth for cosmetic enhancement. The design of the tooth preparation is crucial; it involves minimal enamel removal to ensure a proper fit and strong bond. This careful preparation increases the veneers' fracture resistance by preserving tooth structure and evenly distributing stress, ensuring durability and natural appearance. Aim Our aim was to determine the fracture resistance of two preparation designs with two different materials for the fabrication of laminate veneers. Materials and methods A

Delayed reimplantation: a case report.

A 12-year-old boy reported to the clinic with a history of trauma, whereas playing 1 day before. On examination it was noted that the patient had an avulsed tooth in relation to 11, Class III fracture in relation to 12 and Class II fracture in relation to 21. The avulsed tooth was brought by the patient wrapped in a newspaper. Although the tooth was in highly unfavorable storage condition, it was decided to reimplant the teeth since the patient was young and to relieve him from psychological, cosmetic and functional trauma. Now after 24 months, the tooth is, functional, firm and free of sympto

Calcium hydroxide induced apical barrier in fractured nonvital immature permanent incisors.

Management of trauma to an immature permanent incisor is a challenge, especially when the pulp is necrotic. The aim of this study is to discuss the management of fractured maxillary right and left central incisors with immature root apex. Radiographic evaluation revealed that the tooth 11 had convergent apical walls, whereas tooth 21 had parallel apical walls; both the incisors were treated with pure calcium hydroxide paste to induce apical development. Follow-up clinical and radiographic examinations confirmed apical barrier at 3 months. Radiograph of tooth 11 with convergent roots showed con

Vertical Root Fracture in Buccal Roots of Bifurcated Maxillary Premolars from Condensation of Gutta-percha.

INTRODUCTION: Maxillary premolars are among the teeth most susceptible to vertical root fracture (VRF) from lateral condensation of gutta-percha. These teeth are distinguished by a complex anatomy of the buccal root including a large depression in the dentin wall facing the bifurcation. It is hypothesized that tooth sectioning coupled with 2-dimensional fracture analysis is instrumental in understanding VRF in such teeth. VRF was examined by tooth sectioning following the development of a fracture mechanics analysis to predict VRF in such roots. METHODS: The fracture morphology in teeth extra

The fate of a mid-root fracture: a case report.

The present report describes a case of a mid-root fracture in a maxillary central incisor of a 19-year-old patient. The fractured tooth was splinted with composite that was removed only 3 years later, as the patient did not appear for follow-up examinations. At this time, the radiographs revealed a normal periodontal ligament, rounding of the borders of the fragments and pulp obliteration of both fragments. Eight years later, the tooth was clinically normal and blurred calcification of the root canal was disclosed radiographically. After 13.5 years the patient complained of tooth mobility and

Impact fracture characteristics of intact and crowned human central incisors.

Dynamic fracture energies and patterns of fracture in extracted human central incisors were determined for groups of intact controls, groups with Vita Dur N crowns, Vita Hi Ceram crowns, Dicor crowns and porcelain veneers. Teeth were struck on their middle labial surfaces by a pendulum impact device. The mean fracture energy for teeth with Dicor crowns was significantly lower than for all other groups (P < 0.05). Control tooth crowns fractured obliquely in an apical-direction. Vita Dur N and Dicor crowns shattered, the underlying tooth usually fracturing in the plane of the impact force. Vita

Multidisciplinary approach for the treatment of a complicated crown-root fracture in a young patient: a case report.

Crown-root fractures in permanent teeth cause esthetic and functional problems. This paper reports the case of a complicated crown-root fracture in the maxillary right central incisor of a young patient who was treated with a multidisciplinary approach in two phases. A modified Widman flap, root canal therapy, glass fiber post cementation, and adhesive tooth fragment reattachment were performed shortly after an accident. Satisfactory esthetic and functional outcomes were obtained. However, the patient did not attend follow-up visits and returned after 7 years. During this second phase, the cli

Effect of Pulp Chamber Access, Instrumentation, Obturation, and Restoration on the Fracture Resistance of Endodontically Treated Canine Teeth in Dogs.

Veterinary studies documenting the effect of endodontic treatment on tooth fracture resistance are scarce. The objective of this ex vivo study was to evaluate the effects of mesial access preparation and restoration, as well as pulp chamber access, instrumentation, obturation, and restoration, on the fracture resistance and characteristics of canine teeth in dogs. Sixty-five dog canine teeth were divided into 4 groups: 1. Standard endodontic treatment through a mesial access only; 2. Treatment as per group 1, adding an incisal access, instrumentation and obturation of the pulp chamber, and res

Socket shield technique for immediate implant placement after dental trauma: a case report.

Anterior teeth are often extracted due to dental trauma, periodontal disease, and other factors. During the healing period of sockets, the loose bundle bone in the buccal side of the anterior teeth usually produces vertical and horizontal bone absorption. How to remain and reconstruct alveolar crest contour in the esthetic zone is a challenge to dentists. In this case, a 26-year-old man reported with the chief complaint of a traumatic fracture in upper front teeth region, accompanied by pain. After examination, he was diagnosed with tooth fracture (11,12 crown-root fracture). The socket shield

Endoscopically assisted removal of ectopic teeth in the floor of the orbit. Case report and latest literature review.

Ectopic sinonasal third molar is a rare condition characterized by the aberrant position of the third molar in the maxillar sinus. The etiology of the teeth in the maxillary sinus is commonly associated to trauma and iatrogenic dental procedures. We present the clinical case of a 33-year-old man who presents an ectopic tooth in the right maxillary sinus, located in the orbital floor, who requires endoscopic control through a maxillary approach when performing the extraction. The endoscopic technique is being increasingly used in the maxillofacial field. Thanks to the endoscopic control for com

Force eruption of mandibular second incisor in an 11- year old boy: a technical report.

There is a great challenge in the treatment of deeply fractured and un-restorable teeth among dentists. Orthodontic force eruption is a method of treatment for these teeth to preserve natural root system and periodontal structures. This technical report is a new modification of this procedure presented in an 11- year old boy with deeply fractured left second mandibular incisor. The fractured teeth were treated with root canal therapy and a file #80 was modified to become a hook cemented into the fractured tooth. Anterior teeth were splinted and used as anchorage to help the root extrusion. 1-y
