Pyrithiamine was administered to newborn rats throughout the vulnerable period for myelinogenesis. A major metabolic defect was produced in the cerebral activities of the thiamine-dependent enzymes, transketolase and pyruvate decarboxylase. In spite of a defect in carbohydrate metabolism which is lethal in adult rats, overall development, and myelination as indicated by biochemical and morphological criteria, proceeded at an essentially normal rate. These findings indicate alternative metabolic pathways may be operational in newborn rat brain enabling it to circumvent major blockage in thiamin…

We report the case of a patient with terminal renal disease on chronic hemodialysis who developed acute thiamine deficiency as confirmed by erythrocyte transketolase determinations. The patient presented with a confusional state and severe memory disturbances, but other classical features of Wernicke's encephalopathy were absent. Almost all central nervous system symptoms rapidly disappeared after thiamine therapy. Therefore the possibility of thiamine deficiency must be considered in patients on chronic dialysis who present with central nervous system disturbances, even if all of the classica…

The thiamin status of early bronchial and breast carcinomas has been assessed using "thiamin pyrophosphate" (TPP) stimulating effect of transketolase enzyme activity. The TPP stimulating effect was found to be greater in a high proportion of the cancer patients than in normal subjects, suggesting that these patients may be at risk of thiamin deficiency. The urinary excretory level of thiamin was high in the cancer patients; it is, therefore, unlikely that the biochemical deficiency of the vitamin was due to lack of the vitamin intake. It is, however, possible that the cancer patients were unab…

Three experiments were performed to examine for causes of poor growth of young Merino sheep. Weekly testing of animals 42 weeks of age for 10 weeks revealed that 90% of clinically poor animals were excreting high levels of thiaminase in their faeces; low levels of activity were present in 20% of clinically normal animals. There were significant differences in the mean erythrocyte transketolase activity of the thiaminase excreting poor animals and the thiaminase free normal animals. Other known causes of poor growth could not be demonstrated. Weekly monitoring of thiaminase activity in the faec…

Thiamin status has been measured using the erythrocyte transketolase (EC 2.2.1.1) assay in twenty-eight normal children and in twenty-five severely-malnourished children throughout the course of recovery. Subclinical thiamin deficiency was found in 7% of the normal children and 36% of the malnourished children on admission. There was no significant association between thiamin status and oedema, stunting or wasting, history of breast-feeding, pattern of weaning, age or sex. Five malnourished children, who died, all had a normal thiamin status on admission; however, two developed biochemical evi…

Plasma concentrations of vitamins A and E, serum and erythrocyte folic acid, serum B12 and erythrocyte enzyme activations (to assess vitamins B1, B2 and B6 status) were measured at the start and finish of 39 courses of total parenteral nutrition (TPN). The daily regimen was standard. Plasma vitamin A, E, and folate concentrations and vitamin B6 status improved significantly during TPN. Three patients developed low levels of vitamin A and two patients developed high transketolase activations (B1 depletion) during therapy. The adequacy of vitamin replacement and the monitoring of vitamin status …

A prospective study of thiamine status in patients with severe injuries (injury severity scores greater than 12) showed that they all developed signs of severe thiamine deficiency on the basis of transketolase activity, the standard biochemical test for thiamine deficiency. This occurred within the first week after their injuries despite routine enteral or parenteral feeding including vitamin supplements. This may be due to increased requirements secondary to increased metabolic activity or increased cellular losses. An increased awareness of thiamine requirements and metabolism is essential t…

TKTL1 is a crucial regulatory enzyme in the pentose phosphate pathway (PPP) and plays a significant role in energy synthesis. It is expressed in various tumour tissues, with its expression level closely associated with tumour invasion, metastasis and prognosis. Recent studies utilising proteomic analysis and other methods have highlighted the noteworthy expression of the TKTL1 gene in germ cells, particularly in spermatogonia and ovarian cells. Consequently, this article reviews the molecular characteristics of TKTL1 and its expression in germ cells to provide a reference for research on TKTL1

Two cases of Wernicke's encephalopathy due to hyperemesis gravidarum are described. The first patient presented with bilateral papilloedema, altered sensorium and the second with bilateral retinal haemorrhages, ophthalmoplegia and nystagmus. Both patients were diagnosed with Wernicke's encephalopathy on clinical ground since there were no laboratory facilities to measure red cell transketolase and thiamine pyrophosphate levels. This is a rare but treatable complication of hyperemesis gravidarum (HG) and due to lack of diagnostic tools, there is often diagnostic uncertainty, delay in commencing…

The nature of the thiamine diphosphate binding proteins from rat liver hyaloplasm was studied. When [14C]thiamine was used as a marker, a [14C]thiamine diphosphate-containing electrophoretically homogeneous protein preparation was isolated from the liver soluble fraction and classified as transketolase. No other non-enzymatic proteins which bind thiamine diphosphate and can serve as substrates in the reaction of thiamine diphosphate synthesis in the hyaloplasm were found. It was shown that the phosphate group is transferred by rat liver thiamine diphosphate kinase to the free (but not to the p…

In 35 females and 22 males with acute alcoholic disorders and in 44 individuals of a control group the author studied the indices of thiamine balance--the content of thiamine and pyruvic acid in the blood and urine, concentration of citric acid in the blood, activity of transketolase of erythrocytes and TDP-effect. The results demonstrated that a thiamide deficiency in excretion and insufficiency in coenzyme functions of oxidative decarboxylation of pyruvic acid and in the pentose cycle is seen not only in alcoholic psychoses, but in the withdrawal syndrome. Sex differences of these indices an…

A 39-year-old man, with no history of alcohol intake, who had had an esophago-ileo-colo-gastroplasty with ileotransversostomy, developed diplopia, seizures, metabolic acidosis, and cardiac failure and finally refractory hyperdynamic shock. He died 20 h after admission to our intensive care unit from cardiocirculatory collapse. Postmortem results revealed low erythrocyte transketolase activity, which was increased by 22% by in vitro addition of thiamine diphosphate (TDP effect). Cerebral pathology showed the alterations of Wernicke's encephalopathy. We discuss the possible mechanisms of fatal c…
