[Controversies in the selection of donors in heart transplants].

The shortage of ideal donor hearts has led to an increasing number of would-be heart transplant recipients who die while on the waiting list: an extension of the classical donor criteria seems necessary to expand the donor pool. In this paper, the major controversies on donor selection are presented, along with the clinical results obtained in cardiac transplantation with donor grafts not complying with standard criteria. The extension of donor age over 35 years is supported by the decreased incidence of coronary artery disease (CAD) in the Western world. In our experience, the number of donor

Dysplastic epidermal change in immunosuppressed patients with renal transplants.

The prevalence of non-melanoma skin cancer has been studied in a group of 85 patients who have undergone renal transplantation. We also investigated the relationship between the development of neoplastic lesions and the duration of immunosuppression, previous sun exposure and infection with human papilloma virus. The overall prevalence of neoplastic and pre-neoplastic epidermal lesions in the group was 25 per cent, higher than that previously reported in studies from the United Kingdom. In patients who had survived for more than 80 months after transplantation the prevalence of these lesions w

Percutaneous nephropyelostomy in the management of obstructed renal transplants.

Five percutaneous nephropyelostomies were performed on 4 patients with obstructed renal allografts. In all instances the procedure significantly influenced further management of these patients. It allowed pressure measurement within the obstructed collecting system, assessment of renal function after prolonged drainage, and anatomical delineation of the site of the obstruction using antegrade pyelography. Percutaneous pyelonephrostomy and drainage is a useful means to determine whether obstruction or rejection is the major contributor to renal failure following renal transplantation.

Rejection and repair of endothelium in major vessel transplants.

Fresh saphenous vein homografts are gaining popularity as conduits for femoral-popliteal or distal bypass grafts. Aside from major blood gro … t endothelial cell proliferation detected by H3-thymidine uptake. This response peaked at 5 days in transplants and ever-expanding islands of proliferating endothelium were present in aortas exposed to 20 percent sodium chloride, leading to healing in 3 to 4 months. In the transplanted vessels, total endothelial destruction occurred at 11 to 28 days, and cell …

Kidney preservation by surface cooling: analysis of 130 transplants.

An analysis of 130 consecutive cadaveric renal transplants has shown the effectiveness of simple surface cooling of organs before transplantation. The primary graft failure rate was 10%, and kidneys could be stored for periods of up to 12 hours including warm ischaemia times of 150 minutes. Forty-eight patients received haemodialysis before transplantation and 78 patients received peritoneal dialysis. There were significant differences in the incidence of tubular necrosis in the two groups: these may have been linked to the differences in fluid and osmotic load presented to the transplants.

Microvascular inflammation in kidney transplants: finally an effective treatment?

Microvascular inflammation-glomerulitis and peritubular capillaritis-is the hallmark histologic lesion of antibody-mediated allograft rejection and may also occur in the absence of donor-specific antibodies. In either case, microvascular inflammation leads to graft loss, and it has proven difficult to treat. As demonstrated in a study in this issue of Kidney International, targeting CD38, a glycoprotein expressed on both plasma cells and natural killer cells, shows great promise in treating microvascular inflammation in kidney transplants.

Heme disposal inside heart transplants: a trigger for rejection?

Cardiac allograft vasculopathy (CAV) is a fibroproliferative form of transplant rejection with limited treatment options other than retransplantation. In this issue, See and colleagues examined human explanted allografts with CAV. They found that a high proportion of intragraft plasma cells produce antibodies that recognize the heme catabolic end product, bilirubin. Clonotypic profiling revealed that bilirubin-reactive antibody-producing plasma cells develop from graft-infiltrating innate-like B cells, a subset often characterized by their rapid production of polyreactive natural antibodies as

Organ Donation and Transplants During Major US Motorcycle Rallies.

IMPORTANCE: Large-scale motorcycle rallies attract thousands of attendees and are associated with increased trauma-related morbidity and mortality. OBJECTIVE: To examine the association of major US motorcycle rallies with the incidence of organ donation and transplants. DESIGN, SETTING, AND PARTICIPANTS: This population-based, retrospective cross-sectional study used data from the Scientific Registry of Transplant Recipients for deceased organ donors aged 16 years or older involved in a motor vehicle crash and recipients of organs from these donors from March 2005 to September 2021. EXPOSUR…

Vascular Complications in Pediatric Liver Transplants and Their Management.

OBJECTIVES: The blood supply of the liver occurs through the hepatic artery and portal vein. Outflow of blood circulation in the liver is via the hepatic veins. Any disruption in this blood circulation results in deterioration of liver functions. In this study, we evaluated early vascular complications in pediatric liver transplant recipients seen at our center. MATERIALS AND METHODS: From November 1988 to December 2021, our team has performed 701 liver transplant procedures, which have included 334 pediatric liver transplants. Among these pediatric liver transplant recipients (mean age of 7.…

Worse Outcomes Associated With Liver Transplants: An Increasing Trend.

Background and aim Since individuals in the early stages of liver cirrhosis are typically asymptomatic, the prevalence of liver cirrhosis may be underestimated. Liver cirrhosis has a significant morbidity and mortality rate, with 1.03 million deaths worldwide each year. For end-stage liver disease, liver transplantation is a potential therapeutic option. The goal of our research was to examine the current trend in liver transplants using data from a national database. Methods Using the International Classification of Diseases (ICD)-9 codes, we identified individuals who had a liver transplant …

Autogenous transplants of adrenal fragments in an animal model.

UNLABELLED: Introduction Adrenal insufficiency is a typical complication after surgical treatment of adrenal tumors, especially after the removal of both adrenal glands. Human beings are not able to survive without adrenal glands and without proper hormonal substitution. Autotransplantation of a fragment of the adrenal gland may prevent this complication. This can be done by transplanting the entire adrenal glands or its fragment, such as the adrenal cortex cells. In the case of adrenal tumors, the entire adrenal gland can not be transplanted. However, it is possible to transplant cells from t

Intestinal transplants: review of normal imaging appearance and complications.

Intestinal transplant (IT) is one of the least common forms of organ transplant but is increasing both in volume of cases and number of centers performing intestinal transplants, with the busiest centers in North America and Europe. IT can be performed in isolation or as part of a multivisceral transplant (MVT). Intestinal failure either in the form of short gut syndrome or functional bowel problems is the primary indication for IT. The normal post-surgical anatomy can be variable due to both recipient anatomy in regard to amount of residual bowel and status of native vasculature as well as wh…

Lymphatic vessels identified in failed corneal transplants with neovascularisation.

BACKGROUND: Corneal transplant failure with neovascularisation is a leading indication for full-thickness grafts in patients. Lymphangiogenesis is implicated in the pathology of graft failure, and here we systematically evaluate failed human corneal transplants with neovascularisation for the presence of lymphatic vessels. METHODS: Nine failed grafts with neovascularisation, based on H&E staining with subsequent immunoperoxidase staining for CD31, a blood vessel marker, were selected. Lymphatics were investigated by immunohistochemical and immunofluorescence approaches using podoplanin as a l…

Identifying Subphenotypes of Antibody-Mediated Rejection in Kidney Transplants.

The key lesions in antibody-mediated kidney transplant rejection (ABMR) are microcirculation inflammation (peritubular capillaritis and/or glomerulitis lesions, abbreviated "pg") and glomerular double contours (cg lesions). We used these features to explore subphenotypes in 164 indication biopsies with ABMR-related diagnoses: 137 ABMR (109 pure and 28 mixed with T cell-mediated rejection [TCMR]) and 27 transplant glomerulopathy (TG), identified from prospective multicenter studies. The lesions indicated three ABMR subphenotypes: pgABMR, cgABMR, and pgcgABMR. Principal component analysis confir
