Subarachnoid hemorrhage as the initial presentation of cerebral venous thrombosis.

Cerebral venous thrombosis presenting as subarachnoid hemorrhage (SAH) is very rare. We present a woman with thrombosis of the superior sagittal, straight, transverse and sigmoid sinuses who presented with SAH in the right temporal sulcus and bilateral cerebellar sulci. Brain perfusion CT demonstrated a delay of the mean transit time and high cerebral blood volume around the right posterior temporal lobe and cerebellum. These findings were compatible with venous congestion and they suggest the possibility that extension of the dural sinus thrombosis into the superficial veins caused localized

Transverse sinus dominance predicts outcome after venous sinus stenting for pulsatile tinnitus.

OBJECTIVES: Venous sinus stenting (VSS) is highly effective for pulsatile tinnitus (PT), yet up to 20% of patients experience incomplete relief. We aimed to investigate whether transverse sinus dominance could serve as a non-invasive imaging biomarker for clinical risk stratification. MATERIALS AND METHODS: Preoperative transverse sinuses were classified into balanced or non-balanced dominance via qualitative visual assessment on CTV/MRV. The primary endpoint was an unfavorable outcome (residual tinnitus, defined as postoperative tinnitus handicap inventory (THI) > 16) at 3-month follow-up. M…

Length of Transverse Sinus Reinforcement Predicts Symptom Improvement After Venous Sinus Stenting for Idiopathic Intracranial Hypertension.

BACKGROUND AND OBJECTIVES: Chronically elevated venous sinus and intracranial cerebrospinal fluid (CSF) pressures in idiopathic intracranial hypertension (IIH) may weaken the dural walls covering these sinuses, increasing their compliance and susceptibility to compression by elevated CSF pressures. This may contribute to proximal or distal stent-adjacent stenosis after venous sinus stenting (VSS), explaining the persistent or recurrent IIH symptoms and elevated CSF pressures in some cohorts. We sought to assess whether increasing the length of transverse sinus (TS) reinforced by the stent (TS-

Pilot study of fractal dimension analysis of osteogenesis for bone substitute materials of Bio-Oss in lateral sinus augmentation.

BACKGROUND: Fractal dimension (FD) analysis has been proposed and validated in osseointegration-related research. The aim of this study was to evaluate the feasibility of FD analysis in the osteogenesis detection of bone substitute materials (BSMs) of Bio-Oss in maxillary lateral sinus augmentation. METHODS: Patients who received lateral maxillary sinus augmentation and underwent grafting with BSMs (Bio-Oss) were included in the study. The cross sections of the BSMs under cone-beam computed tomography (CBCT) at mesial, distal, and sagittal directions were obtained immediately after the graft

Radiographic outcomes of lateral sinus floor elevation with and without bone window repositioning: one-year results of a randomized controlled trial.

The objective of this study was to perform a comparative evaluation of the radiographic outcomes of lateral sinus floor elevation with and without bone window repositioning (BLSFE and LSFE, respectively) when applied concomitantly with implant placement. A randomized controlled clinical trial was conducted between February 1, 2016 and May 1, 2017 including 26 individuals with at least one missing tooth. Participants were randomized 1:1 to undergo BLSFE (10 participants, 16 implants) or LSFE (13 participants, 19 implants). Bovine-derived xenograft was used in both groups and the implants were i

Onyx embolization for dural arteriovenous fistulas: a multi-institutional study.

BACKGROUND: Although the liquid embolic agent, Onyx, is often the preferred embolic treatment for cerebral dural arteriovenous fistulas (DAVFs), there have only been a limited number of single-center studies to evaluate its performance. OBJECTIVE: To carry out a multicenter study to determine the predictors of complications, obliteration, and functional outcomes associated with primary Onyx embolization of DAVFs. METHODS: From the Consortium for Dural Arteriovenous Fistula Outcomes Research (CONDOR) database, we identified patients who were treated for DAVF with Onyx-only embolization as the

Dural arteriovenous fistula of the transverse-sigmoid sinus causing trigeminal neuralgia.

The authors analysed an unusual case of dural arteriovenous fistula (DAVF) of the transverse-sigmoid sinus causing trigeminal neuralgia is presented. Although progression to almost continuous facial pain has been reported, symptoms may be indistinguishable from typical trigeminal neuralgia. The patient had a 6-year history of right-sided trigeminal neuralgia initially well controlled by medical management. He was referred for surgical management after 10 months of progressively worsening of symptoms. At the time of consultation, the patient complained of pulsatile tinnitus in the right ear. Co

Occipital transtentorial approach selected based on venous anatomy for a vermian cavernous malformation: illustrative case.

BACKGROUND: Cavernous malformations of the superior cerebellar vermis beneath the tentorium are rare and surgically challenging because approach selection must account for lesion location and venous anatomy. OBSERVATIONS: A 69-year-old man presented with vertigo, nausea, and truncal ataxia caused by hemorrhage from a cavernous malformation of the superior cerebellar vermis. Repeated CT during the first 5 days after admission showed hematoma enlargement with worsening symptoms. Preoperative CT venography revealed bilateral hypoplastic transverse sinuses with dominant occipital and marginal sin…

Catastrophic Outcome Following Apparently Trivial Nondominant Transverse Sinus Injury During Resection of a Tentorial Meningioma: Case Report.

A 52-year-old man was admitted to our hospital with symptoms of raised intracranial pressure and cerebellar dysfunction caused by a medium-sized (4 cm in diameter) tentorial meningioma with an infratentorial extension. Preoperative magnetic resonance imaging showed that the tumor indented and possibly partially invaded the adjacent junction of the nondominant transverse and sigmoid sinuses. The contralateral dominant transverse sinus was fully patent. Total surgical removal of the lesion was done through the left retrosigmoid approach. During dissection of the meningioma, some bleeding from th

Hormone Concentration Measurement in Intracranial Dural Arteriovenous Fistulae.

OBJECTIVE: Intracranial dural arteriovenous fistulae (DAVFs) represent a subset of cerebral vascular malformations associated with significant morbidity and mortality. In Japan, DAVF exhibits sex-based differences in anatomical distribution, with female predominance in the cavernous sinus (CS) and male predominance in the transverse sinus (TS). Nevertheless, the pathophysiology of DAVF is not fully understood, and hormonal influences are hypothesized to play a role in its development. This study aimed to investigate changes in the concentrations of sex steroid hormones between intracranial and

Clinical and radiographic outcomes of reentry lateral sinus floor elevation after a complete membrane perforation.

BACKGROUND: Although small perforation of the maxillary sinus schneiderian membrane is a well-documented complication during lateral sinus floor elevation (LSFE), complete perforations larger than 10 mm often result in discontinuation of surgery. Reports on reentry LSFE and its long-term outcomes are sparse. PURPOSE: To evaluate the long-term outcomes of reentry LSFE following complete membrane perforation to elucidate the technical details of the reentry procedure. MATERIALS AND METHODS: We assessed the medical records of all patients receiving LSFE from 2008 to 2017 in the Department of Or

Empty Sella Is a Sign of Symptomatic Lateral Sinus Stenosis and Not Intracranial Hypertension.

BACKGROUND AND PURPOSE: Empty sella has been reported in patients with idiopathic intracranial hypertension and is thought to be a sign of elevation of intracranial pressure. However, it can also be found in patients with lateral sinus stenosis presenting with isolated pulsatile tinnitus without signs of intracranial hypertension. We hypothesized that the volume of the sella turcica would be similar in both groups of patients undergoing stent placement for lateral sinus stenosis. MATERIALS AND METHODS: Consecutive patients with idiopathic intracranial hypertension or isolated venous pulsatile
