RESULTS: Over 80% of pregnancies resulted after infertility treatment (21/26). Mean maternal age was 32. Rett syndrome is usually sporadic, but rare pedigrees with nonpenetrance in obligate carriers and possible anticipation suggest that it could be caused by a triplet repeat expansion (TRE). Rett probands and controls were systematically screened for expansions of any of the 10 possible triplet repeats by using a modified Repeat Expansion Detection (RED) assay that had been shown to detect expanded disease alleles in myotonic dystrophy and Huntington disease. In 1991 a triplet maternity of same-sex (FFF) occurred in a depopulated community of Argyll, Scotland, with a history of excessive, same-sex twinning. Due to a stillbirth and its confidentiality, the maternity was recorded as a twin birth. During the last decade, 89 sets of triplets were born in Denmark with a gestational age of more than 25 completed weeks. Obstetric complications of triplet pregnancies may make the delivery of extremely premature infants in these multiple gestations inevitable. To our knowledge, this case represents the first report of triplets delivered on three separate days, 16 days apart, using the technique of delayed interval delivery. One of the infants survived.

A survey of triplet pregnancies in Linköping, Sweden, 1973-1981, showed a low perinatal mortality (4%) and low morbidity possibly due to prophylactic bed rest and tocolytic treatment. All children were followed at child health clinics. In one child, a persistent growth retardation was recorded at 4 years of age. No psychomotor deviations were found in the group. An interview concerning social and economic factors after delivery was carried out. The answers revealed considerable strain in daily life for all families.

A retrospective analysis of 20 triplet pregnancies not stimulated by clomiphene or gonadotrophins revealed a perinatal mortality of 26.3%. There were greater losses recorded in the case of mothers who had received no antenatal care than for those who had been seen before the babies' births. A patient with a triplet pregnancy had one infant affected by anencephalus. This pregnancy resulted from ovulation stimulation by gonadotropins. Several cases of association between anencephalus and induction of ovulation by clomiphene have been reported. The association with gonadotropins, however, has been reported only once. The event was prenatally diagnosed by ultrasound studies in the first and second trimesters. The association of anencephalus with ovulation stimulation is discussed briefly.
