Scrub Typhus in Kerala: Demographic, Clinical, and Laboratory Predictors of ICU Admission in a Tertiary Care Setting.

BACKGROUND: Scrub typhus, a mite-borne infection caused by Orientia tsutsugamushi, is endemic in South and Southeast Asia, including India. Although increasing awareness and improved healthcare access have reduced mortality, the disease remains a significant public health concern. Kerala, a southern Indian state, has reported scrub typhus cases for decades; however, comprehensive data on its clinical profile and severity indicators are limited. This study aimed to describe the clinical characteristics of scrub typhus and identify predictors of intensive care unit (ICU) admission. METHODS: A r…

Incidence of Scrub Typhus in Rural South India.

BACKGROUND: Hospital studies suggest that scrub typhus is a leading cause of severe undifferentiated fever in regions across Asia where the … ticipants underwent blood sampling to estimate the incidence of serologically confirmed Orientia tsutsugamushi infection. RESULTS: We systematically assessed 32,279 participants from 7619 households for acute febrile illness. During 54,588 person-years of follow-up, we observed 6175 episodes of fever. A blood sample was obtained in 4474 episodes (72.5% …

Cellulitis that was NOT Cellulitis.

Cellulitis is a common bacterial skin infection characterized by erythema, swelling, and pain of the affected skin and subcutaneous tissues. The lower limbs are a commonly affected area due to microtrauma and gravity-dependent stagnation of blood. We describe a case of a male patient with an extensive travel history who presented with left lower limb cellulitis, hyperbilirubinemia, and acute kidney injury. When workup for the usual causes of cellulitis did not provide a clearer picture, we had to consider the unusual. Considering his travel history, an exotic tropical zoonotic disease was cons

Scrub Saccadomania: An Enigma of Eye Movement in the Tropics.

BACKGROUND: Opsoclonus myoclonus syndrome (OMS) is a rare neuro-ophthalmic disorder characterized by chaotic, multidirectional saccadic eye movements. It is associated with various etiologies, including infectious and immune-mediated encephalitis. Scrub typhus, caused by Orientia tsutsugamushi, is an uncommon but important, treatable cause of OMS. Here, we present a case of scrub meningoencephalitis presenting with saccadomania as the initial clinical manifestation. OBJECTIVE: To highlight scrub meningoencephalitis as a potential cause of OMS and emphasize the importance of prompt recognition…

Temporal trends in diagnostic evolutions for rickettsial diseases including scrub typhus: a bibliometric study.

The bibliometric analysis aimed to assess the bibliometric characteristics, current status and temporal trends in diagnostic evolutions related to infectious diseases caused by the genus Orientia and Rickettsia. Relevant studies were retrieved from Scopus using pre-determined standard keywords. The analysis was performed using the R package 'Bibliometrix' through the 'Biblioshiny' interface. A total of 239 studies were identified for Rickettsia diagnostics, while 168 studies focussed on scrub typhus diagnostics. The annual growth rate of publications in scrub typhus (1.82%) was found to be hig

Patient-centric analysis of Orientia tsutsugamushi spatial diversity patterns across Hainan Island, China.

BACKGROUND: Scrub typhus, traditionally caused by Orientia tsutsugamushi, is a re-emerging public health concern within the Tsutsugamushi Triangle. Despite growing awareness, prevention strategies remain inadequate on Hainan Island, China, where scrub typhus poses a significant threat, especially in field-related environments. METHODOLOGY/PRINCIPAL FINDINGS: Gene flow analysis of the tsa56 gene and multilocus sequence typing (MLST) were conducted on 156 previously confirmed scrub typhus cases from 2018 to 2021 across Hainan Island. By integrating published datasets, we identified 12 major sub…

Scrub typhus masquerading as other conditions among the pediatric population in India: a case series.

India is an endemic country for scrub typhus, a common rickettsial infection caused by Orientia tsutsugamushi contracted through the bite of infected mites. Though relatively common in the pediatric population, early diagnosis sometimes becomes difficult due to its atypical presentation. We reported five febrile children who had varied presentations like convulsion, jaundice, generalized edema, or features like Kawasaki disease, and only one had pathognomonic eschar. They were diagnosed with acute disseminated encephalomyelitis, nephrotic syndrome, hepatitis, or Kawasaki phenotype hyperinflamm…

The second generation metagenomic sequencing of cerebrospinal fluid for diagnosis of non-eschar scrub typhus.

Tsutsugamushi disease, is an infectious disease transmitted by ticks and caused by the rickettsiella. It is characterized by eschar, fever, rash, and flu-like symptoms. However, diagnosing atypical cases without an eschar and with negative Weil-Felix test results poses a significant diagnostic challenge. This study presents a noteworthy case of non-eschar typhus which was effectively diagnosed using advanced next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) samples.

COVID-19 and Parasitic Co-Infection: A Hypothetical Link to Pulmonary Vascular Disease.

Background/Objectives: Before the Coronavirus disease 2019 (COVID-19) era, the global prevalence of pulmonary arterial hypertension (PAH) was between 0.4 and 1.4 per 100,000 people. The long-term effects of protracted COVID-19 associated with pulmonary vascular disease (PVD) risk factors may increase this prevalence. According to preliminary data, the exact prevalence of early estimates places the prevalence of PVD in patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection at 22%, although its predictive value remains unknown. PVD caused by COVID-19 co-infections i

Mathematical analysis of scrub typhus seasonal infection with re-scaled transmission rate considering Northeast India reported data from 2010 to 2022.

Healthcare reporting methods have seen a common problem with actual incidence of scrub typhus cases in Northeast India that were reported during post rainy season. We propose a Host-Vector model, a first of its kind, with a significant modification in the disease infection transmission rate. Our work aims to investigate a mathematical model by Atangana-Baleanu fractal-fractional operator that has seasonal pattern incorporating 2010-2022 data. The existence-uniqueness property is investigated using the fixed point theory, and also Ulam-Hyers stability is performed. Based on Lagrange's interpola

Validation of Clinical RGA Score in Diagnosis of Scrub Typhus at 2nd Tier Hospital of West Bengal, India.

Scrub typhus is caused by arthropod-borne gram-negative obligately intracellular bacillus Orientia tsutsugamushi bite by bitten by an infected vector, a Leptotrombidium mite. An eschar at the site of chigger feeding is a classic clinical feature of scrub typhus. Scrub typhus had severe complications including jaundice, acute renal failure, acute respiratory distress syndrome etc. Laboratory investigation is the gold standard test for the diagnosis but it is not available at all point of care centers. Rathi Goodman Score (RGA) was introduced and validated to diagnose the disease at the resource…

From eschar to diagnosis: A case report on scrub typhus causing multi-organ failure in a returning traveller.

Scrub typhus, caused by Orientia tsutsugamushi, is a rickettsial infection transmitted by mite bites, often underdiagnosed in travellers from endemic regions. This case report describes a 65-year-old female with hypertension who developed severe scrub typhus after a trip to Sri Lanka. She presented with fever, myalgia, headache, fatigue and a scabbed lesion. Initial tests showed neutrophilia, lymphocytopenia and elevated liver enzymes. Malaria was ruled out, and empiric treatment with intravenous Tazocin was initiated. On day 5, she developed Clostridium difficile infection, requiring a switch…

Development of molecular and immunological based diagnostic tools to detect Orientia tsutsugamushi (scrub typhus) infection in human.

Scrub typhus is a disease caused by a bacterial pathogen, Orientia tsutsugamushi which spreads through bites of infected chigger mite. Fever, headache, body aches and rash are common symptoms of scrub typhus. The absence of specific signs and symptoms of the disease makes diagnosis extremely difficult. In the present study, the blood samples from individuals with infections were obtained and analyzed for IgM and IgG antibodies using a commercial ELISA test kit. The present work was carried out to develop a molecular and immunological based technique which are more sensitive and accurate for th…

Bilateral Neuroretinitis Induced by Scrub Typhus in China.

A 51-year-old female sanitation worker in China developed bilateral neuroretinitis due to scrub typhus. Initially diagnosed with 'scrub typhus infected pneumonia', her symptoms subsided within seven days of chloramphenicol treatment. However, two weeks later, she experienced acute bilateral vision loss. Fundoscopy revealed swollen optic discs, exudates, and a small macular pigment epithelial detachment. Diagnosed with scrub typhus-related neuroretinitis, her vision improved after methylprednisolone treatment. This represents the first reported case of such in southern China, suggesting a delay

Clinical Characteristics and Evolution of Ocular Manifestations in Scrub Typhus.

Scrub typhus, caused by Orientia tsutsugamushi infection, manifests with fever, rash, eschar, pneumonia, hepatosplenomegaly, and lymphadenopathy. We conducted a review of the ocular manifestations post-infection and found a high incidence (67%) during the recovery phase (2-4 weeks), particularly prevalent in the tropical Tsutsugamushi triangle. Despite China, only three cases of optic neuritis had been reported, suggesting its rarity due to early antibiotic administration, we present the first documented case of binocular optic neuroretinitis due to Orientia tsutsugamushi infection in China. O…

Scrub Typhus Combined With Septic Shock Disseminated Intravascular Coagulation and Significant Hyperfibrinolysis: A Case Report and Review of the Literature.

Introduction: Scrub typhus is an acute infectious disease caused by Orientia tsutsugamushi, whose pathophysiology is characterized by systemic small-vessel vasculitis. Its high misdiagnosis rate stems from its nonspecific clinical features. If not diagnosed and treated in time, patients may rapidly progress to multiorgan dysfunction syndrome (MODS) or even disseminated intravascular coagulation (DIC), posing a severe threat to life. Case Presentation: The patient was a 68-year-old male with "recurrent fever and dry cough for six days." He was admitted to the hospital with a diagnosis of scrub …
