Whole-body PET/CT reveals systemic dissemination in spinal tuberculosis: findings from the Spinal TB X cohort.

INTRODUCTION: Spinal tuberculosis (STB) is a severe form of extrapulmonary tuberculosis associated with diagnostic delay and substantial morbidity. The extent of disease-isolated versus disseminated STB-remains poorly defined. We applied whole-body 18F-FDG-PET/CT to characterise the clinical and radiographic phenotype of STB. MATERIAL AND METHODS: The Spinal TB X cohort (NCT05610098) is a prospective study of HIV-infected and uninfected patients with suspected STB in Cape Town, South Africa. We analysed the first ten patients with microbiologically confirmed STB. Demographic, clinical, microb

Neutrophil-lymphocyte ratio as a potential marker for differential diagnosis between spinal tuberculosis and pyogenic spinal infection.

OBJECTIVE: Distinguishing spinal tuberculosis and pyogenic spinal infection is extremely important. The neutrophil-lymphocyte ratio (NLR), a simple indicator, has been shown to be a novel inflammatory marker. The objective of our study was to determine whether the NLR could be a potential indicator for discriminating spinal tuberculosis (STB) from pyogenic spinal infection (PSI). METHODS: We compared the clinical and laboratory characteristics of 146 patients diagnosed with STB and 60 participants with PSI from the First Affiliated Hospital of Nanjing Medical University. The NLR's diagnostic

Surgical Treatment for Spinal Tuberculosis without Elevation of Inflammatory Biomarkers at the Initial Visit Mimicking Spinal Metastasis.

Here, we report a case of spinal tuberculosis without elevation of C-reactive protein (CRP) at the initial visit mimicking spinal metastasis. A 70-year-old woman developed progressive paraplegia without a history of injury and came to our hospital for evaluation. Severe compression to the spinal cord with osteolytic destruction of the spinal vertebrae at T6-7 was observed without elevation of CRP. A T4-9 posterior decompression and fusion were performed. Although the pathology revealed no malignant tumor cells, a positron emission tomography-computed tomography (PET-CT) showed upregulation of

Spinal brucellosis - A mimicker of spinal tuberculosis: An analysis of 10 patients from a tertiary care center in India.

INTRODUCTION: Brucellosis of the spine has emerged as a significant health concern in developing countries. Spinal brucellosis shares identical clinical and radiological features with tubercular spondylodiscitis. A wide range of nonspecific symptoms, chronicity of this disease, and tissue culture which is mostly invasive and time-consuming, pose a challenge in definitive diagnosis. In this study, we aim to report the clinical, radiological, and microbiological features of this disease and analyze the outcome of patients treated for spinal brucellosis. MATERIALS AND METHODS: This retrospective

Metastatic Colorectal Cancer Presenting as Spinal Cord Compression and Mimicking Tuberculosis.

A 69-year-old male presented to the ED with a three-week history of back pain and progressive bilateral lower limb weakness. Initial imaging raised concerns for discitis and spinal cord compression (SCC). Further investigations, including MRI, CT, PET-CT, and biopsy, revealed metastatic adenocarcinoma of likely lower gastrointestinal origin, complicated by pulmonary embolism and multiple pulmonary nodules. The case emphasizes the diagnostic challenges when malignancy mimics infectious processes such as tuberculosis and highlights the critical role of imaging, tissue biopsy, and multidisciplina

Dual antituberculosis drugs-loaded gelatin hydrogel bioimplant for treating spinal tuberculosis.

Spinal tuberculosis (TB) represents around 1% of the recorded TB with a high mortality rate due to neurological complications and kyphosis. The current work aimed to develop a bioimplant scaffold to treat spinal TB disease. The scaffold is composed of a biocompatible semi-interpenetrating (semi-IPN) gelatin-based hydrogel incorporating mesoporous silica nanoparticles (MPS-NPs) loaded with rifampicin (RIF) and levofloxacin (LEV) to treat TB. The elastic modulus of the hydrogel was 7.18 ± 0.78 MPa. Minimum inhibitory concentrations (MIC) value against Mycobacterium bovis for LEV-loaded and RIF-l

Short-term outcome of lamina-sparing decompression in thoracolumbar spinal tuberculosis.

OBJECTIVE: The authors sought to assess the outcomes of lamina-sparing decompression using a posterior-only approach in patients with thoracolumbar spinal tuberculosis (TB). In patients with spinal TB with paraplegia, anterior decompression yields excellent results because it allows direct access to the diseased part of the vertebra, but the anterior approach has related morbidities. Posterior and posterolateral decompression mitigate approach-related morbidities; however, these approaches destabilize the already diseased segment. Lamina-sparing decompression through a posterior-only approach

Central nervous system and spinal cord tuberculosis: Revisiting an important disease.

Tuberculosis is a worldwide pandemic. Estimated that about 25% humans are colonized by Mycobacterium tuberculosis and about 1% are believed to develop the infection in the central nervous system (CNS-TB). Given the importance of this disease and its high levels of morbidity and mortality, it is imperative that every radiologist must be reminded of the most common findings of CNS-TB as there are several related differential diagnoses for this disease. The most common form CNS-TB is tuberculous meningitis (TBM), characterized mostly by basal meningitis, but infarction, hydrocephalus and tubercul

[Multinodular splenomegaly revealing multifocal, splenic and spinal tuberculosis: a case report].

Splenic tuberculosis and Pott´s disease are two rare entities, in particular in immunocompetent people. We here report the case of a 57-year-old immunocompetent woman presenting with atypical pain in the left hypochondriac region evolving over the last 3 months, associated with lower limb paraparesis with progressive onset. Laboratory data did not provide any specific information regarding diagnosis, except for positive response to the QuantiFERON Test. Abdominal and pelvic computed tomography (CT) scan showed splenomegaly with multiple nodular, hypodense lesions in the spleen. Spinal cord mag

Tuberculosis the great mimicker: 18F-fludeoxyglucose positron emission tomography/computed tomography in a case of atypical spinal tuberculosis.

Tuberculosis (TB) has aptly been called the great mimicker. A 14-year-old boy, who had been treated for Hodgkin's Lymphoma 2 years back and had been in remission, underwent a whole body 18F-fludeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) scan, owing to a recent onset of backache and difficulty in walking. The study showed an FDG avid destructive lesion of the posterior elements of the sixth, seventh and eighth dorsal vertebrae along with a hypermetabolic collection in the adjacent paraspinal muscles with extension into the spinal canal, causing encasement of t
