Neodymium:YAG laser coagulation prostatectomy for patients in urinary retention.

Urinary retention necessitating catheterization is the presenting complaint for a significant minority of men requiring surgical therapy for bladder outlet obstruction. A total of 67 men presented in acute or chronic urinary retention and underwent Nd:YAG laser prostatectomy. Their mean age was 71 years, the mean estimated excess transition zone tissue was 40 g, and the mean laser energy delivery was 48 kJ. The median postoperative catheterization time was 10 days. Five men required catheterization for > or = 6 weeks, and in five men, the catheter was removed within 48 hours. In four men (6%),…

Acute urinary retention: an unusual presentation of appendiceal abscess.

Emergency physicians frequently encounter patients with acute urinary retention. Although most common among the elderly, pediatric and young adult patients occasionally may present with this complaint. The differential diagnosis and emergency department work-up must be age specific. We discuss the case of a young man who presented with acute urinary retention, subsequently found to be caused by undiagnosed appendicitis with appendiceal abscess formation. Only nine such cases have been reported in the literature thus far, with only one occurring in the nonpediatric population. This case represe…

Posthernioplasty urinary retention: a noninvasive work-up for prediction.

The prognostic value of various subjective and objective noninvasive urological parameters was evaluated for their possible role in the prediction of post-hernioplasty urinary retention. In a prospective study, 35 patients aged 55-85 years without previous micturition complaints or urological surgery were evaluated 1 day prior to inguinal hernioplasty. This included medical history for modified Madsen subjective scoring profile, physical examination and uroflowmetry. Bladder capacity, residual urine and prostatic volume were measured by ultrasound. All patients underwent a similar method of he…

Ruptured abdominal aortic aneurysm presenting with acute urinary retention.

Ruptured abdominal aortic aneurysm classically presents with abdominal pain radiating to the back, a pulsatile abdominal mass and circulatory collapse. However, other symptoms may be the only presenting complaint. We report the case of a patient who presented with a history of acute retention of urine relieved by catheterisation, but who then developed clinical features more typical of abdominal aortic aneurysm rupture.

Appendiceal abscess masquerading as acute urinary retention in children.

Two boys with acute urinary retention were found to have a persistent pelvic mass after bladder decompression. Evaluation in each disclosed a large pelvic abscess secondary to a ruptured appendix. The diagnosis of appendicitis may be difficult and appendiceal abscess presenting with acute urinary retention in children has been reported previously in only 7 instances. History, physical examination, laboratory studies, and radiographic and ultrasonic evaluations should lead to the correct diagnosis, and surgical intervention restores normal voiding.

Urinary retention secondary to congenital bladder diverticula in infants.

Acute urinary retention in infants is uncommon. We present 2 patients in whom enormous bladder diverticula extended posteriorly, compressing the bladder outlet and causing obstruction. In 1 patient the diverticulum was solitary, periureteral and associated with ipsilateral ureteral reflux, while the other had bilateral paraureteral diverticula without reflux. Primary bladder diverticula in infants and children are the congenital result of localized deficient musculature allowing herniation of mucosa to occur at this site, even with normal voiding pressures. Giant bladder diverticula causing ou…

Adenomyosis-Induced Urinary Retention: Case Analysis and Clinical Implications.

BACKGROUND Acute urinary retention (AUR) is relatively rare in non-pregnant women and is usually associated with lower urinary tract dysfunction, neurological issues, or pelvic organ compression. Adenomyosis is a common gynecologic condition characterized by the invasion of endometrial glands and stroma into the myometrium, often accompanied by symptoms such as dysmenorrhea and heavy menstrual periods. Although adenomyosis is common, its involvement in causing urinary retention is rare but deserves clinical attention. CASE REPORT We report the case of a patient with recurrent urinary retention…

Delayed Urinary Retention After Holmium Laser Enucleation of Prostate.

OBJECTIVE: To identify modifiable perioperative risk factors associated with delayed urinary retention (DUR) that occurs after a successful same-day trial of void (SDTOV) in patients undergoing holmium laser enucleation of prostate (HoLEP). METHODS: Our institution prospectively maintains an institutional review board-approved single surgeon database of HoLEP procedures from which we identified patients with DUR after passing SDTOV. Exclusion criteria included the first 50 cases (due to the surgeon's learning curve), patients with failed SDTOV, and clot retention. Using SPSS 26 statistical so…

Tamsulosin to Prevent Urinary Retention After Vaginal Urogynecologic Surgery.

IMPORTANCE: Postoperative urinary retention (POUR) affects up to 51% of women undergoing surgery for pelvic organ prolapse (POP). Effective prevention strategies remain elusive, although alpha receptor antagonists show promise in recent literature. OBJECTIVES: This study sought to evaluate the effect of preoperative tamsulosin on POUR after vaginal surgery for POP. STUDY DESIGN: We conducted a retrospective cohort study of female patients undergoing vaginal surgery for POP, with or without concomitant midurethral sling. Patients who received preoperative tamsulosin were compared with control…

Hyponatraemia, AKI, and urinary retention following elective joint arthroplasty.

BACKGROUND: Acute kidney injury (AKI) and hyponatraemia are common postoperative complications following elective joint replacement surgery, often related to fluid and electrolyte imbalances. Dehydration can contribute to AKI, whereas excessive fluid administration may lead to hyponatraemia. Additionally, urinary retention can mimic AKI by reducing urine output, potentially resulting in unnecessary fluid administration and dilutional hyponatraemia. This study aims to improve postoperative fluid-electrolyte management by differentiating AKI from urinary retention and reducing the incidence of h…

Urinary Retention Evaluation and Catheterization Algorithm for Adult Inpatients.

IMPORTANCE: Acute urinary retention (UR) is common, yet variations in diagnosis and management can lead to inappropriate catheterization and harm. OBJECTIVE: To develop an algorithm for screening and management of UR among adult inpatients. DESIGN, SETTING, AND PARTICIPANTS: In this mixed-methods study using the RAND/UCLA Appropriateness Method and qualitative interviews, an 11-member multidisciplinary expert panel of nurses and physicians from across the US used a formal multi-round process from March to May 2015 to rate 107 clinical scenarios involving diagnosis and management of adult UR …

Bladder Leiomyoma presenting as urinary retention: A case report.

Leiomyoma of the bladder is a rare diagnosis and accounts for 0.43% of all bladder tumors. We herein present a case of a thirty-nine-year-old female diagnosed with bladder leiomyoma presenting with urinary retention. Workup of her urinary retention revealed a midline bladder neck mass. Transurethral resection of her tumor revealed pathology consistent with bladder leiomyoma. Surgical resection is generally the recommended management of this lesion to make a histopathological diagnosis despite low potential of malignant transformation. Although these tumors are benign, they can cause severe obs…

Covert postpartum urinary retention: causes and consequences (PAREZ study).

INTRODUCTION AND HYPOTHESIS: Increased post-voiding residual volume (PVRV), known as covert postpartum urinary retention (PUR), is an asymptomatic condition with unknown long-term adverse effects. The objectives were to determine the frequency of this phenomenon 3 days after delivery and to examine the associated risk factors and consequences of the increased residuum on women´s health 6 weeks postpartum. METHODS: We carried out a prospective observational study including a total of 926 primiparous women, giving birth to singletons. All participants underwent ultrasound determination of PVRV …

Urethral leiomyoma: A rare cause of acute urinary retention.

INTRODUCTION AND IMPORTANCE: Leiomyoma is a benign smooth muscle tumor, which is the most common seen in the uterine and is rarely found in the urethra. These are benign mesenchymal tumors that arise from the smooth muscle, which appear during the reproductive age of a female. Diagnosis and management of this case at a rare location were discussed. CASE PRESENTATION: A 24-year-old woman with complaints of urinary retention and a mass protruding out of the urethral external meatus presented to the obstetrics and gynecology clinic. On pelvic examination, a red like mass was seen into urethral e…

Urinary retention concomitant with methamphetamine use: a case report.

BACKGROUND: Urinary retention is a condition in which impaired emptying of the bladder results in postvoid residual urine. It can be acute or chronic urinary retention. There have been only scattered case studies that have described urinary retention resulting from methamphetamine use. This case report is aimed at raising awareness about methamphetamine abuse as an important factor in the aetiological considerations when evaluating cases of urinary retention among healthy younger age groups. CASE PRESENTATION: We report a patient who had acute urinary retention after brief amphetamine use. A …
