Unexpected persistent left superior vena cava and absent right superior vena cava in situs solitus patient.

We, herein, report a patient with persistent left superior vena cava with enlarged coronary sinus and absent right superior vena cava. This anomaly, diagnosed intraoperatively during the third open-heart surgery in the course of transesophageal echocardiography examination, was not mentioned during the patient's previous two cardiac operations. Challenges in intraoperative management and implications for subsequent treatments are discussed.

Dynamic angio-MRI demonstration of persistent left superior vena cava in the absence of the right superior vena cava.

This condition is a rare anomaly. Combination with other congenital malformations including coarctation of the aorta, double aortic arc, transposition of great vessels and dextrocardia was reported. We performed an MRI evaluation. MRI was first obtained at cardiac level using a gated Black Blood and Cine technique, useful to confirm the enlarged coronary sinus, and then using a dynamic angiographic technique. By means of the latter modality, each frame was obtained in 5 s and was useful to demonstrate the contrast bolus through the venous, cardiac, pulmonary and systemic levels. It was possibl

Persistent left superior vena cava with absent right superior vena cava: a difficult cardiac pacemaker implantation.

An absent right superior vena cava associated with a persistent left superior vena cava (PLSVC) is a rare and generally asymptomatic congenital malformation. It is usually discovered incidentally during a pacemaker (PM) implantation. We report here a successful PM implantation using a left subclavian approach through the coronari sinus via a PLSVC, with a 2-year follow-up showing a still appropriate function of the device. We discuss the clinical usefulness of peripheral phlebography in the operating room for delineating precisely the venous anatomy, as well as the role of shaped stylets and a

[Segmental variants of the inferior vena cava--manifestations with embryological correlation in differentiation from secondary vena cava occlusion].

The vast variability of the overall rare congenital anomalies of the inferior vena cava (IVC) are mostly detected by accident through different imaging modalities. These cannot be classed as pathological findings, and should not be confused with lymphomas and has to differ from secondary collateral venous pathways. Based on 656 CT examinations 10.5% known forms of IVC anomalies were found. According to the embryological development three main groups of IVC variants could be nosologically classified: agenesis of the suprarenal IVC, anomalies of the pars renalis and anomalies of the infrarenal I

[The persistence of the left vena cava superior and the absence of the right vena cava superior: the implications in the implantation of a definitive pacemaker].

Persistent left superior vena cava and associated absence of the right superior vena cava is exceedingly rare. The authors present a case, diagnosed when a DDD pacemaker system was being implanted, and point out its rarity, possibility of diagnosis by non invasive methods and technical implications in transvenous electrode implantation.

[Temporary and permanent pacemaker provision in persistent left superior vena cava and atresia of the right superior vena cava].

HISTORY AND CLINICAL FINDINGS: An 80-year-old woman was admitted because of confusion episodes, difficulty of finding words and transitory clouding of consciousness on that day. There were moderately moist rales over both lung bases and mild pretibial oedema. The ECG showed atrial fibrillation with an irregular ventricular rate of 30/min. Attempted emergency implantation of a pacemaker electrode via the right subclavian vein failed because the catheter could not be advanced in the usual manner due to complete obstruction of unknown cause. INVESTIGATIONS: Fluoroscopy and venography of the uppe

Transvenous defibrillator implantation in patients with persistent left superior vena cava and right superior vena cava atresia.

In this report a transvenous cardioverter defibrillator implantation is described in two patients with a persistent left-sided superior vena cava and right SVC atresia. In the first case, manoeuvring of the guide wire inserted through the left subclavian vein into the SVC proved impossible, revealing a left SVC originating from the left brachiocephalic vein with an acute corner. Changing the side of implantation and inserting a CPI Endotak catheter through the right subclavian vein, the lead was easily advanced through the left SVC into the coronary sinus and then into the right atrium with th

Orthotopic heart transplantation with preoperative unsuspected left superior vena cava and absence of right superior vena cava.

We report the case history of a 47-year-old man who underwent orthotopic heart transplantation for ischemic cardiomyopathy. At the time of cardiectomy, the patient was found to have a persistent left superior vena cava draining into the coronary sinus and complete absence of his right superior vena cava. The donor heart had been removed without knowledge of this venous anomaly; consequently, the donor's superior vena cava and innominate vein were not harvested. The persistent left superior vena cava was cannulated for cardiopulmonary bypass. The recipient's heart was excised along the atrial v

Vena Tech vena cava filter: experience and early follow-up.

Vena caval filters, such as the Vena Tech filter, that employ low-profile introducer systems have provided physicians with a variety of options for percutaneous placement. From April 1989 to April 1990, 81 patients underwent percutaneous placement of the Vena Tech filter at the authors' institution. Follow-up has been obtained to evaluate the filter with regard to the prevention of pulmonary embolism, the maintenance of caval patency, and mechanical stability. Two cases of pulmonary embolism have been seen following filter placement. Three cases of caval thrombosis have occurred, with recanali

The use of the Zontik cava filter for temporary implantation to the inferior vena cava.

The authors review the potential for the use of the Zontik retrievable cava filter (CF) for temporary implantation. In the course of their observations, the CF was to be received by 68 patients for the time needed for the treatment of floating thrombi in the inferior vena cava and its tributaries using thrombectomy, thrombolysis or anticoagulants. In 10 patients, the CF was implanted in view of the risk of pulmonary thromboembolism (PTE) during and after obstetric-gynecological and orthopedic operations carried out in the presence of deep venous thrombosis of the lower limbs. After elimination

Superior vena cava-to-inferior vena cava bridging stent technique for cavo-atrial junction stenosis: a case series.

BACKGROUND: Endovascular bridging stents are relatively underreported but effective methods to increase the diameter of stenosed segments of the superior and inferior cavo-atrial junction. This case report describes the use of superior to inferior vena cava bridging stents to resolve cavo-atrial junction stenoses in three patients with distinct mediastinal masses. CASE SUMMARY: All patients presented symptomatically with shortness of breath, as well as lower limb oedema, facial and neck swelling, and/or a cough. SVC-to-IVC bridging stents were selected as the appropriate intervention for thes

Percutaneous stenting of malignant superior vena cava syndrome in a patient with persistent left and absent right superior vena cava.

Stent placement is the preferred means of managing malignant obstruction of the superior vena cava (SVC). Persistent left and absent right SVC is a very rare venous anomaly. We here report the case of a 58-year-old man who underwent percutaneous stenting for malignant persistent left and absent right SVC obstruction caused by advancement of adenocarcinoma of the upper lobe of the left lung. The patient became symptom-free one day after endovascular stenting through the right femoral vein. However, he experienced repeated supraventricular tachycardia during the procedure. To our knowledge, this

Liver cirrhosis and hepatocellular carcinoma in hepatic vena cava disease, a liver disease caused by obstruction of inferior vena cava.

PURPOSE: Hepatic vena cava disease (HVD), a form of Budd-Chiari syndrome, is caused by the obstruction of hepatic portion of the inferior vena cava. It is a chronic disease characterized by the development of liver cirrhosis (LC) and hepatocellular carcinoma (HCC). As HVD occurred in areas with high incidence of hepatitis B virus (HBV) infection and some patients tested HBsAg positive, it was thought to be the cause of LC and HCC. To assess the pathogenesis of LC or HCC in HVD, a long-term follow-up study was done. METHOD: Fifty-six patients with HVD diagnosed by ultrasound (US) and confirmed

Atrioventricular nodal tachycardia in a patient with anomalous inferior vena cava with azygos continuation and persistent left superior vena cava.

A 39-year-old female patient was referred for ablation of recurrent episodes of atrioventricular nodal reentrant tachycardia. A combination of an anomalous inferior vena cava with azygos continuation and a persistent left superior vena cava was discovered. A nonfluoroscopic navigation system was very useful for catheter ablation of the tachycardia in this unusual case of anomalous venous system of the heart.
