Dissociation between Caloric and Video Head Impulse Tests in Dizziness Clinics.

Vestibular assessment tests such as the video head impulse test (vHIT) for the horizontal semicircular canal, and caloric test (Cal), both evaluate horizontal canal function. One would assume that the outcomes for these tests should lead to concordant results, yet several studies have suggested that dissociation can occur in certain pathological conditions. As this topic remains inconclusive, this review aims to analyze the scientific evidence regarding the patterns of hypofunction observed in vHIT and Cal in different otoneurological diseases. A comprehensive review of the literature regardin

Discordance Between Subjective Visual Vertical and Vestibular Evoked Myogenic Potentials in Stroke.

BACKGROUND: While there has been a significant correlation between subjective visual vertical (SVV) and ocular vestibular myogenic potentials (oVEMPs) tests in acute vestibular neuritis (AVN), the association between these two tests in central vestibular lesions remains uncertain. METHODS: We prospectively recruited 58 patients with acute stroke involving the posterior circulation territory (PCS) between 2019 and 2022. For comparison, 32 patients with AVN served as a control. We evaluated sustained utricular function through SVV tests and transient utricular function through bone-conducted (B

The functional head impulse test: preliminary data.

The functional head impulse test is a new test of vestibular function based on the ability to recognize the orientation of a Landolt C optotype that briefly appears on a computer screen during passive head impulses imposed by the examiner over a range of head accelerations. Here, we compare its results with those of the video head impulse test on a population of vestibular neuritis patients recorded acutely and after 3 months from symptoms onset. The preliminary results presented here show that while both tests are able to identify the affected labyrinth and to show a recovery of vestibular fu

[Preoperative Examination:Neuro-Otological Examination and Its Significance].

Neuro-otologic examinations are essential to identify lesion sites in patients with hearing and balance disorders. These tests assess the functions from the peripheral sensory organs to the central auditory and vestibular pathways. They are valuable for evaluating inner ear function and detecting central nervous system involvement. Auditory tests, such as pure-tone audiometry and auditory brainstem response, help localize lesions along the auditory neural pathway. Vestibular function can be assessed using the caloric test, video head impulse test, and vestibular evoked myogenic potentials, whi

Vestibular function in children with vestibulocochlear nerve aplasia/hypoplasia.

BACKGROUND: Infants and young children with vestibulocochlear nerve (VCN) hypoplasia/aplasia present with severe hearing loss and are candidates for cochlear implantation (CI). It is unknown whether vestibular function is related to CI outcome and if vestibular tests can guide the operation decision. AIMS/OBJECTIVES: Our aim was to describe the vestibular function in patients with VCN hypoplasia/aplasia before a possible CI. MATERIALS AND METHODS: Forty-two ears in 23 patients were tested between 2019 and 2022 with bone-conducted cervical vestibular evoked myogenic potentials (BCcVEMP), vide

Vertigo and dizziness in childhood - update on diagnosis and treatment.

Vertigo and balance disorders are not uncommon in children. The prevalence of vestibular vertigo in 10-year-Dolds is estimated to be 5.7%. The most common cause is vestibular migraine which accounts for almost 40% of the diagnoses. In adolescents, the incidence of somatoform vertigo syndromes increases. Vestibular function can be reliably evaluated at the bedside by the head-impulse test for vestibulo-ocular reflex function, ocular motor testing of the central vestibular system, and balance tests for vestibulo-spinal function. Vestibular migraine is treated by behavioural and drug therapies. S

Behavioural disturbances and sensory pathology following allylnitrile exposure in rats.

Animals exposed to allylnitrile develop permanent abnormalities in motor behaviour, similar to those caused by 3,3'-iminodipropionitrile (IDPN) and crotononitrile. IDPN and crotononitrile effects have been attributed to vestibular hair cell degeneration, but allylnitrile has been suggested to modify behaviour through neuronal degeneration in the CNS. Adult male Long-Evans rats were exposed to allylnitrile (0, 20, 40, 60 mg/kg per day, for 3 days) and the changes in rearing activity and rating scores in tests of vestibular function were assessed. Surface preparations of the vestibular sensory e

A simple and time saving cold mini caloric test.

A simple and time-saving office procedure is presented. The test consists of placing a small piece of cotton-wool soaked in ice water against the tympanic membrane. We term this test the Cold mini caloric test. Our method has been compared quantitatively with the Bithermal caloric test in 20 normal controls, 50 unspecific dizzy patients with no vestibular or obvious neurological disorder, 22 Menière ears, 12 acoustic neuromas and 20 cases of vestibular neuronitis. The comparison between the two methods has shown the Cold mini caloric test to present consistently similar, though somewhat shorte

[Pseudocaloric nystagmus (author's transl)].

A pseudocaloric nystagmus is, in its strict sense, a spontaneous nystagmus activated by the unspecific stimulus of syringing the auditory canal. 40 patients with a unilateral, peripheral-vestibular defect were examined electronystagmographically. With most patients the 30 degrees caloric stimulation gave an incorrect result of the degree of hypoexcitability with all parameters (duration, maximum frequency and maximum intensity). This incorrect result was most pronounced with patients after neurectomy of the vestibular nerve, who frequently had a heavily reduced reaction of the healthy ear. For

Long-term outcome of vestibular function and hearing in children with congenital cytomegalovirus infection: a prospective cohort study.

PURPOSE: Congenital cytomegalovirus infection (cCMV) is the most frequent nonhereditary cause for sensorineural hearing loss (SNHL) in children. Data on vestibular function in children with cCMV are, however, scarce, although some evidence for cCMV-associated vestibular dysfunction exists. In this prospective cohort study, we evaluated long-term vestibular function and hearing outcomes in a cohort of children with cCMV. METHODS: Participants were 6-7-year-old children with cCMV from a large population-based screening study. Controls were age and gender matched healthy children, who were CMV-n

The role of vestibular cues in postural sway.

Controlling posture requires continuous sensory feedback about body motion and orientation, including from the vestibular organs. Little is known about the role of tilt vs. translation vs. rotation vestibular cues. We examined whether intersubject differences in vestibular function were correlated with intersubject differences in postural control. Vestibular function was assayed using vestibular direction-recognition perceptual thresholds, which determine the smallest motion that can be reliably perceived by a subject seated on a motorized platform in the dark. In study A, we measured threshol

Impact of vestibular dysfunction on short - term auditory outcome in severe to profound unilateral idiopathic sudden sensorineural hearing loss: a retrospective analysis.

BACKGROUND: Vestibular dysfunction is prevalent in severe/profound unilateral idiopathic sudden sensorineural hearing loss (ISSNHL) and may influence hearing recovery. This study investigates associations between vestibular function (cVEMP/oVEMP, vHIT, caloric tests) and short-term auditory outcomes. AIMS: To evaluate whether vestibular test abnormalities predict treatment response in severe/profound ISSNHL and identify independent prognostic factors. MATERIAL AND METHODS: Retrospective analysis of 133 patients (mean age 53.58 ± 18.56 years) treated with steroids/hyperbaric oxygen. Vestibula

Standing postural stability during galvanic vestibular stimulation is associated with the motor function of the hemiplegic lower extremity post-stroke.

Background: The vestibular system is profoundly involved in standing postural stability. Patients with post-stroke hemiparesis have poor postural control function; nevertheless, it is unclear as to how the vestibular system affects postural control after stroke. Objectives: The purpose of this study was to quantitatively evaluate the relationship between galvanic whole-body sway responses and motor function of the hemiplegic lower extremity post-stroke.Methods: Thirty stroke patients and 49 healthy controls underwent standing body sway tests to examine postural control function during vestibula

Vestibular-Evoked Myogenic Potential Testing in Vestibular Localization and Diagnosis.

Vestibular-evoked myogenic potentials (VEMPs) are short-latency, otolith-dependent reflexes recorded from the neck and eye muscles. They are widely used in neuro-otology clinics as tests of otolith function. Cervical VEMPs are recorded from the neck muscles and reflect predominantly saccular function, while ocular VEMPs are reflexes of the extraocular muscles and reflect utricular function. They have an important role in the diagnosis of superior canal dehiscence syndrome and provide complementary information about otolith function that is useful in the diagnosis of other vestibular disorders.

Vestibular diagnosis in Menière's disease.

Vestibular diagnosis of Menière's disease must be based on the results of an elaborate test battery. The latter should include a complete caloric test (stimulation with cold and warm water), carried out under nystagmographic control. Statistical analysis of caloric responses recorded in 100 normal test subjects by D.C. nystagmography show clearly that culmination frequency and culmination eye velocity are the most reliable parameters indicating an unilateral loss of function. With respect to the differential diagnosis - endorgan or neuronal lesion - only the galvanic test allows an unequivocal
