We selected once more from about 15000 abstracts published in 2011 a couple of papers focused on general practice. Regarding the ear, we will discuss the role of nasal corticoids in Eustachian tube dysfunction, the treatment of vestibular neuronitis, the role of the bone-anchored hearing aid, or the vestibular implant. Regarding the nose, the influence of tobacco on sinus surgery, the treatment of staphylococcus cases, and the role of neuronavigation. Regarding the throat, head and neck region, our focus was on potential new treatments of the future that might change our approach, and on the c

Otoneurological test results analyzed by means of a quantitative statistical method.

This report is a presentation of an approach to the prediction of cerebellar-pontine (C-P) angle tumors and four peripheral vestibular diseases from a mathematical statistical point of view. The present study was performed in an attempt to quantify multidimensional, quantitative data. The data analyzed were from a total of 143 patients with C-P angle tumors, Menière's disease, sudden deafness, vestibular neuronitis, and benign paroxysmal positional vertigo (BPPV). Tests were carried out mainly on otoneurological parameters. By means of discriminant analysis as employed in the quantification me

[A role of vascular risk factors in the development of peripheral vestibulopathy].

The development of peripheral vestibular disorders are often thought to be associated with vascular mechanisms, taking into account terminal type of inner ear blood supply and other predisposing factors. A number of studies indicates a high frequency of vascular risk factors in the patients with vestibular neuronitis and benign paroxysmal positional vertigo (BPPV). According to other results, migraine is widely spread among patients with Meniere's disease and BPPV. However currently there is no evidence for casual relationship between vascular factors and development of peripheral vestibulopat

A simple and time saving cold mini caloric test.

A simple and time-saving office procedure is presented. The test consists of placing a small piece of cotton-wool soaked in ice water against the tympanic membrane. We term this test the Cold mini caloric test. Our method has been compared quantitatively with the Bithermal caloric test in 20 normal controls, 50 unspecific dizzy patients with no vestibular or obvious neurological disorder, 22 Menière ears, 12 acoustic neuromas and 20 cases of vestibular neuronitis. The comparison between the two methods has shown the Cold mini caloric test to present consistently similar, though somewhat shorte

Surgery for vertigo (you don't have to live with it).

Surgery for vertigo is recommended where there is objective evidence of a unilateral inner ear disorder, when medical therapy fails, and when the patient demands definitive therapy. Common surgical procedures are the endolymphatic subarachnoid shunt, retrolabyrinthine vestibular neurectomy, cochleovestibular neurectomy, and the post ampullary nerve section (singular neurectomy). Vertiginous disorders which can be surgically cured are Ménière's disease, vestibular neuronitis, chronic labyrinthitis, and benign positional vertigo. Due to the recent advances in microsurgery and the development of

Modification of compensatory saccades after aVOR gain recovery.

The recruitment of extra-vestibular mechanisms to assist a deficient angular vestibulo-ocular reflex (aVOR) during ipsilesional head rotations is well established and includes saccades of reduced latency that occur in the direction of the lesioned aVOR, termed compensatory saccades (CS). Less well known is the functional relevance of these unique saccades. Here we report a 42 y.o. male diagnosed with right unilateral vestibular hypofunction due to vestibular neuronitis who underwent a vestibular rehabilitation program including gaze stabilization exercises. After three weeks, he had a signific

Pediatric neurotology.

Pediatric neurotology is a subspecialty encompassing the medical and surgical treatment of conditions involving the middle ear, inner ear and lateral skull base. Multiple otologic and neurotologic diseases exist. In the pediatric patient the most common neurotologic conditions are vertigo, complications of otitis media, cholesteatoma, hearing loss and facial paralysis. Vertigo is differentiated into peripheral and central vestibular abnormalities. Peripheral vestibular causes include Meniere's disease, benign paroxysmal positional vertigo, vestibular neuronitis, otitis media and labyrinthitis.

Current diagnosis and treatment of vestibular neuritis: a narrative review.

Vertigo is the sensation of self-motion of the head or body when no self-motion is occurring or the sensation of distorted self-motion during an otherwise normal head movement. Representative peripheral vertigo disorders include benign paroxysmal positional vertigo, Ménière disease, and vestibular neuritis. Vestibular neuritis, also known as vestibular neuronitis, is the third most common peripheral vestibular disorder after benign paroxysmal positional vertigo and Ménière disease. The cause of vestibular neuritis remains unclear. However, a viral infection of the vestibular nerve or ischemia

Vestibular-evoked myogenic potentials.

PURPOSE OF REVIEW: This article reviews the literature on vestibular-evoked myogenic potential testing, a short latency electromyogram evoked by high acoustic stimuli and recorded via surface electrodes over the sternocleidomastoid muscle. Applications and refinements of this technique are described for different pathologies and in adults and children. RECENT FINDINGS: Various techniques for electrode placement have been described to elicit a vestibular-evoked myogenic potential response, which has been clinically investigated in normal individuals, under pathological conditions, and in adult

The use of antiserotonin drugs in the nucleoreticular vestibular syndrome: preliminary observations.

Vestibular neuronitis was described in 1949 and 1952 by Dix and Hallpike. Two groups of patients were described, those with sudden seizures and sensations of blackout (since identified as having vestibular neuritis) and a second group with symptoms of disequilibrium and feelings of top-heaviness or imbalance. The pathology was believed to be central to the inner ear. Arslan labeled these groups as having nucleoreticular vestibular syndrome. Using a suprathreshold stapedial reflex test, Bosatra localized the pathophysiology in the brainstem, an area rich in serotonergic neurons. This author has

Spatial disorientation and dysfunction of orientation/equilibrium reflexes: aeromedical evaluation and considerations.

Loss of spatial awareness has been implicated as a direct causal factor in 4-10% of serious aircraft mishaps and 10-20% of fatal aircraft mishaps (7). Spatial disorientation in flight usually results from misperception of visual, vestibular, or proprioceptive cues. Pathologic causes have rarely been implicated. A student naval aviator with recurrent loss of spatial awareness due to a defective vestibulo-ocular reflex (VOR), presumably from vestibular neuronitis in adolescence, is reported. His chief complaint, an inability to focus on the instrument panel during turbulent instrument meteorolog

[Equilibrium disorders in childhood].

Dizziness and imbalance are uncommon in children, but often alarming for their families. A detailed interview and otoneurological examination are important for reaching a specific diagnosis and treatment. The purpose of our study was to obtain our own etiological, clinical and diagnostic statistics, and to review the major related pathologies. We report a series of 76 children with imbalance or dizziness as the presenting symptom. Paroxysmal benign vertigo of childhood was the most common pathology in our study, followed by vestibular neuronitis. Our findings were compared to those of other au

Neurotrophin-4/5, brain-derived neurotrophic factor, and neurotrophin-3 promote survival of cultured vestibular ganglion neurons and protect them against neurotoxicity of ototoxins.

The ability of neurotrophin-4/5 (NT-4/5), brain-derived neurotrophic factor (BDNF), neurotrophin-3 (NT-3), and nerve growth factor (NGF) to promote survival of postnatal rat vestibular ganglion neurons (VGNs) was examined in dissociated cell cultures. Of the four neurotrophins, NT-4/5 and BDNF were equally effective but more potent than NT-3 in promoting the survival of VGNs. In contrast, NGF showed no detectable effects. As expected, TrkB-IgG (a fusion protein of extracellular domain of TrkB and Fc domain of human immunoglobulin G) specifically inhibited the survival-promoting, effects by NT-

[Vertigo in children].

The main difference between childhood and adulthood vertigo is that the evaluation of symptoms and examination pose special challenges in pediatric patients, depending on age. Vertigo in pre-teenage children is similar to vertigo in adults. In contrast, although benign paroxysmal vertigo, vestibular neuronitis, and Ménière's vertigo occur in children, the distribution of these conditions is different from that seen in adults. After vestibular and cochlear investigations as well as a CT scan or MRI study to outrule a tumor of the posterior cerebral fossa, the diagnosis of vestibular vertigo is

Auditory brain stem responses in patients with vertigo.

Sixty-two patients with vertigo were examined with ABR. Eight of the 30 patients with vestibular neuronitis had abnormal ABR indicating brain stem dysfunction. Most of the patients with abnormal ABR showed continued ABR abnormality after recovery from the disease. Only one patient with epidemic vertigo had abnormal ABR, the remaining 15 patients had no signs of brain stem involvement. All 16 patients with benign positional vertigo had normal ABR.

Bilateral latero-pontine infarction: a rare syndrome, an unusual imaging.

The authors present one case of bilateral infarction of the brain stem, in the territory of the antero-inferior cerebellar artery. Besides the fact that such bilateral ischaemic lesions are rare, the initial clinical presentation mimicking a vestibular neuronitis indicates that this latero-pontine syndrome may appear as a peripheral disorder. It also reminds clinicians that careful and repeated examinations should always be performed in all patients suffering from vertigo. An unusual iconography is also presented.

[The peripheral vestibular disorders encountered in the routine out-patient practical work].

The objective of the present study was to elucidate the frequency of causes underlying the development of dizziness and vertigo in the patients seeking advice and care at an outpatient healthcare facility. A total of 590 patients with complaints of dizziness and vertigo were examined. It was found out that the peripheral disorders of the vestibular analyzer were the most frequent causes of dizziness and vertigo in the majority of the examined patients. These conditions were documented to occur in more than 65% of the patients. The most common forms of peripheral vestibular disorders were benig

Efferent system degeneration in the human temporal bone.

Sense organ deposits have been described in temporal bones from patients with vestibular neuronitis, Meniere's disease, and benign paroxysmal positional vertigo that are not found in a comparable series of temporal bones without vestibulopathy. Because the recurrent vestibulopathies are caused by vestibular ganglionitis and the vestibulocochlear anastomosis was degenerated in these temporal bones, the deposits may represent the end buds of regenerating efferent axons injured in passage through the vestibular ganglion. Such neural buds have been described with transmission electron microscopy i

Vertigo with a Vestibular Dysfunction in Children During Respiratory Tract Infections.

Sudden balance disorders with violent vegetative symptoms (nausea and vomiting) pose a diagnostic and therapeutic problem. In children vertigo/dizziness with symptoms of vestibular dysfunction is rare, but as vascular etiology is unlikely in children such symptoms arouse concern. This article presents two cases of this type of vertigo. The patients were two boys (6 and 9 years old). They came down with similar symptoms: sudden dizziness, disabled walking, nausea and vomiting, spontaneous nystagmus, and a positive Romberg test. The onset of the balance disorder was preceded by respiratory infec
