OBJECTIVE: To describe clinical and radiologic features of cranial nerve (CN) involvement in patients with myelin oligodendrocyte glycoprote … atients with radiologic and/or clinical CN involvement. Two patients displayed either trigeminal or vestibulocochlear nerve lesions at the root level, and the remaining patient had an oculomotor nerve involvement at the root exit and at the cisternal level. Additional CNS involvement was found in all 3 patients. None of the 3 patients' sera recognized MOG expression in C …

Burkitt lymphoma is a late complication of HIV, and bilateral cranial nerve palsies are extremely rare in patients with AIDS. A twenty year old Caucasian male with known congenital HIV who had been non-adherent with anti-retroviral therapy presented with multiple cranial nerve palsies and was eventually diagnosed with Burkitt lymphoma. Before chemotherapy, he was started on radiation therapy to the brain, meninges, and base of skull with the intent of improving cranial nerve palsies and preventing further neurological sequelae since the cranial nerve palsies were dense and there was concern th

The highly variable anatomy of the anterior inferior cerebellar artery is one of the causative factors of neurovascular compression of the facial and vestibulocochlear nerve resulting in hemifacial spasms associated with hearing loss, vertigo, and tinnitus. Our patient a 64-year-old male presented with such complaints for the past few months. He was investigated thoroughly for the cause and on Magnetic resonance imaging of the brain with Fast imaging employing steady-state acquisition sequence it was evident that the anterior inferior cerebellar artery loop was compressing the seventh and eigh…

Malformations in either the inner ear, vestibulocochlear nerve (VIIIth) or auditory cortex of the brain can lead to congenital sensorineural hearing loss (SNHL). In most cases the underlying disorders involve the membranous labyrinth at a microscopic level and therefore radiological examinations are entirely normal. In a significant proportion however (up to 20%), there are abnormalities visualized in the inner ear and/or the VIIIth nerve; the type of abnormality is relevant for the surgical planning of a cochlear implant. Imaging and the accurate radiological identification of the affected in…

The Schwannoma is a benign growth of the nerve sheath cells most commonly seen in the vestibulocochlear nerve. Its prevalence in the adrenal gland is 1-3%. Here we discuss a case that presented as an incidentaloma of the right adrenal gland in a young male patient who had vague abdominal symptoms and a normal hormonal profile. He underwent an excisional biopsy of the right adrenal gland due to the large size of the lesion (more than 4cm). The histopathology report helped to establish the diagnosis of Schwannoma. Incidentaloma is defined as a lesion of the adrenal gland encountered on any radio…

We report a previously undescribed hamartoma of the VIIIth nerve, consisting of adipose tissue, Schwann cells associated with myelinated nerve fibers, well-differentiated smooth and striated muscle fibers, and rare ganglion cells. The tumor was found in a 35-year-old Caucasian female who presented with right-sided hearing loss. The mass, which we designate an "ectomesenchymal" hamartoma, most likely developed from pluripotent neural crest cells ("ectomesenchyme"), which are capable of differentiating into a variety of neuroectodermal and mesenchymal cell types. The development of the neural cr

A vestibular schwannoma, often termed an acoustic neuroma, is a type of benign primary intracranial tumor of the myelin-forming cells of the vestibulocochlear nerve. The typical clinical presentation often includes ipsilateral sensorineural hearing loss/deafness, vertigo and tinnitus. In the present study, the case of a young male patient who presented with recurrent unilateral facial palsy without hearing impairment is presented. The patient was diagnosed with vestibular schwannoma and received steroidal treatment with prednisolone for two weeks. The patient's facial weakness recovered three …

QUESTION 1: Does intraoperative facial nerve monitoring during vestibular schwannoma surgery lead to better long-term facial nerve function?
TARGET POPULATION: This recommendation applies to adult patients undergoing vestibular schwannoma surgery regardless of tumor characteristics.
RECOMMENDATION: Level 3: It is recommended that intraoperative facial nerve monitoring be routinely utilized during vestibular schwannoma surgery to improve long-term facial nerve function.
QUESTION 2: Can intraoperative facial nerve monitoring be used to accurately predict favorable long-term facial nerve funct

A narrow internal auditory canal with duplication is a rare congenital disorder of temporal bone. High-resolution CT of the temporal bone is a preferred modality for assessing the bony framework of the temporal bone and inner and middle ear structures and is thus commonly used. Of late, due to rapid advancement in technology, MRI can assess the neural structures, including the vestibulocochlear and facial nerves. Here, we present the case of a 29-year-old male with congenital left-sided sensorineural hearing loss and left-sided facial palsy.

INTRODUCTION: Approximately 10% of patients with peripheral facial nerve palsy are children. Facial nerve palsy is usually idiopathic. An uncommon cause is herpes zoster oticus (Ramsay Hunt syndrome). This syndrome usually affects adults and is rare in children.
METHODS: We reviewed the literature and describe the cause, clinical manifestations, and treatment of Ramsay Hunt syndrome. We also report a case of the syndrome in a 12-year-old boy.
CONCLUSION: Ramsay Hunt syndrome is characterized by peripheral facial paralysis associated with herpetic eruption on the auricula and external ear, an
