The acute presentation of nitrous oxide-induced vitamin B-12 deficiency in a postoperative lumbar discectomy patient: illustrative case.

BACKGROUND: Vitamin B-12 deficiency can cause serious hematological and neurological complications. While most cases are chronic, acute presentations may occur. In autoimmune diseases, pernicious anemia is a common cause of vitamin B-12 deficiency. The authors present a case of acute vitamin B-12 deficiency following nitrous oxide anesthesia in a patient with Graves' disease, highlighting the impact of preexisting autoimmune conditions on perioperative management. OBSERVATIONS: A 44-year-old African American male with Graves' disease presented with symptoms of an L5-S1 disc herniation and und

Physiologically Based Pharmacokinetic Modeling of Vitamin B-12 Incorporating Mechanistic Absorption: An Example Application for Intake Estimation During Pregnancy.

BACKGROUND: Vitamin B-12 (cobalamin) is an essential micronutrient required for neurologic function, hematopoiesis, and DNA synthesis. Oral absorption of vitamin B-12 occurs through an intrinsic factor (IF)-dependent pathway at physiological intake amounts supplemented by passive diffusion at pharmacologic doses. Vitamin B-12 deficiency is a global public health concern, with certain populations such as older adults and pregnant women at higher risk. Despite adherence to intake recommendations, a substantial proportion of pregnant women exhibit suboptimal vitamin B-12 status. A pharmacokinetic

Multiple micronutrient-fortified rice affects physical performance and plasma vitamin B-12 and homocysteine concentrations of Indian school children.

Fortifying rice with multiple micronutrients could be a promising strategy for combat micronutrient deficiencies in developing countries. We determined the efficacy of extruded rice grains fortified with multiple micronutrients on the prevalence of anemia, micronutrient status, and physical and cognitive performance in 6- to 12-y-old, low-income school children in Bangalore, India. In a randomized, double-blind, controlled trial, 258 children were assigned to 1 of 3 intervention groups to receive rice-based lunch meals fortified with multiple micronutrients with either low-iron (6.25 mg) or hi

Low serum vitamin B-12 concentrations are associated with faster human immunodeficiency virus type 1 (HIV-1) disease progression.

We conducted a nonconcurrent prospective cohort study to examine associations between serum concentrations of vitamin B-6, vitamin B-12 and folate and the risk of progression to first acquired immunodeficiency syndrome (AIDS) diagnosis and CD4+ cell decline to < 2 x 10(8) cells/L. The study population was drawn from a cohort of homosexual and bisexual men in the Baltimore-Washington, DC, area. Eligible subjects were human immunodeficiency virus type 1 (HIV-1)-seropositive at study entry and had serum available in the serum repository from their 1984 baseline study visit. Serum micronutrient le

Role of vitamin B(12), folate, and thyroid stimulating hormone in dementia: A hospital-based study in north Indian population.

BACKGROUND: Vitamin B(12) and folate represent modifiable risk factors for dementia. They may increase the risk of Alzheimer's dementia (AD) and vascular dementia (VaD) as their deficiency can increase the homocysteine level due to slowed methylation reaction. Homocysteine has a neurotoxic effect that could lead to neurologic disturbances. Hence, it is important to explore the status of serum B(12) and folate in AD and VaD to evolve the treatment strategies for the same. OBJECTIVES: A retrospective study was conducted to assess the levels of vitamin B(12), folate, and thyroid stimulating horm

Zinc, gravida, infection, and iron, but not vitamin B-12 or folate status, predict hemoglobin during pregnancy in Southern Ethiopia.

The etiology of anemia during pregnancy in rural Southern Ethiopia is uncertain. Intakes of animal-source foods are low and infections and bacterial overgrowth probably coexist. We therefore measured the dietary intakes of a convenience sample of Sidama women in late pregnancy who consumed either maize (n = 68) or fermented enset (Enset ventricosum) (n = 31) as their major energy source. Blood samples were analyzed for a complete blood count, vitamin B-12 and folate status, plasma ferritin, retinol, zinc, albumin, and C-reactive protein (CRP). The role of infection and gravida was also examine

Maternal Folate and Vitamin B 12 Concentrations During Pregnancy Influence Neonatal Nutritional Status and Adiposity: Results from the OBESO Cohort.

BACKGROUND/OBJECTIVES: Folate and B12, among other B vitamins, are methyl donors and contribute to multiple DNA methylation processes. Maternal deficiency of these nutrients may be associated with impaired fetal growth, affecting the nutritional status and adiposity of the newborn. This study aimed to describe maternal folate and B12 status throughout pregnancy and evaluate its association with neonatal nutritional status. METHODS: We studied 90 healthy pregnant women and their babies from the prospective OBESO cohort (Mexico City). Serum folate and B12 concentrations were measured (ELISA) in

Folate and vitamin B-12 deficiencies additively impaire memory function and disturb the gut microbiota in amyloid-β infused rats.

Folate and vitamin B12(V-B12) deficiencies are associated with metabolic diseases that may impair memory function. We hypothesized that folate and V-B12 may differently alter mild cognitive impairment, glucose metabolism, and inflammation by modulating the gut microbiome in rats with Alzheimer's disease (AD)-like dementia. The hypothesis was examined in hippocampal amyloid-β infused rats, and its mechanism was explored. Rats that received an amyloid-β(25-35) infusion into the CA1 region of the hippocampus were fed either control(2.5 mg folate plus 25 μg V-B12/kg diet; AD-CON, n = 10), no folat

Efficacy of oral compared with intramuscular vitamin B-12 supplementation after Roux-en-Y gastric bypass: a randomized controlled trial.

BACKGROUND: After Roux-en-Y gastric bypass (RYGB), patients often develop a vitamin B-12 deficiency. OBJECTIVE: Our objective was to investigate whether oral supplementation increases and normalizes low vitamin B-12 concentrations (vitamin B-12 > 200 pmol/L) in RYGB patients as compared to intramuscular injections. DESIGN: A randomized controlled trial in RYGB patients with subnormal serum B-12 concentrations was performed. One group (IM B-12) received bimonthly intramuscular hydroxocobalamin injections (2000 µg as loading dose and 1000 µg at follow-up) for 6 mo. The second group (oral B-12)

Serum asymmetric dimethylarginine (ADMA), homocysteine, vitamin B(12), folate levels, and lipid profiles in epileptic children treated with valproic acid.

Recent reports have demonstrated elevated serum homocysteine (Hcy) levels in children receiving valproic acid (VPA) therapy. Elevated Hcy levels might play a potential role in the resistance to antiepileptic drugs, and might lead to an increased risk for a vascular disease. It has been reported that elevated total homocysteine (tHcy) levels are associated with elevated asymmetric dimethylarginine (ADMA) levels, which are factors that may be better indicators of endothelial dysfunction compared to serum homocysteine levels, because they are less sensitive to changes, such as fasting status, phy

Examining the Clinical Usefulness of Urine Methylmalonic Acid for Diagnosis of Vitamin B-12 Deficiency in Older Adults: A Pilot Study.

AIM: Vitamin B-12 deficiency is common among older adults and can lead to irreversible neurological damage and severe complications if not treated early. Despite its clinical significance, effective management remains challenging due to the absence of simple and sensitive biomarkers for early detection and for monitoring treatment response. PURPOSE: This pilot study evaluated the feasibility and potential of the urinary methylmalonic acid (uMMA)/creatinine ratio as a biomarker of vitamin B-12 deficiency in older adults, while exploring clinical responses following supplementation. PARTICIPAN

Vitamin B-12 status is associated with socioeconomic level and adherence to an animal food dietary pattern in Colombian school children.

Vitamin B-12 is related to neurocognitive function in school-age children, yet sociodemographic and dietary correlates of vitamin B-12 status in this age group are not well characterized. The prevalences of vitamin B-12 or folate deficiencies in Colombia are unknown. We conducted a cross-sectional study in a representative sample of 2800 low- and middle-income children aged 5-12 y from Bogotá's public schools. Plasma vitamin B-12 and erythrocyte folate concentrations (mean +/- SD) were 327 +/- 106 pmol/L and 858 +/- 256 nmol/L, respectively. The prevalence of vitamin B-12 deficiency (<148 pmol

Branched-chain and odd-numbered fatty acids and aldehydes in the nervous system of a patient with deranged vitamin B 12 metabolism.

A mixture of isomers of methylhexadecanoic acid was isolated from glycerolipids of brain, spinal cord, and sciatic nerve of a patient who died from methylmalonic aciduria, a disease in which vitamin B(12) is not converted to deoxyadenosyl B(12). The isomers were identified by gas-liquid chromatographic-mass spectrometric analyses, and the data indicated that the points of methyl branching are located predominantly on the even-numbered carbon atoms. The concentration of these branched-chain acids among the glycerolipid fatty acids in the patient's nervous system was at least 0.3-0.9%, while the
