Influence of different palate repair protocols on facial growth in unilateral complete cleft lip and palate.

OBJECTIVE: To address the question of whether one- or two-stage palatal treatment protocol has fewer detrimental effects on craniofacial gro … maxillary sagittal length and position could be perturbed by both one- and two-stage palate repair. Vomer flap repair inhibited maxilla vertical growth. Delayed hard palate repair showed less detrimental effects on maxillary growth compared to early hard palate repair in UCCLP patients aged 5 years.

Transnasal endoscopic treatment of bilateral choanal atresia in newborns using balloon dilatation: own results and review of literature.

OBJECTIVES: Congenital choanal atresia is a complete obliteration of the posterior nasal aperture leading to life-threatening airway emergen … s were performed in transnasal endoscopic technique. In all patients the abnormally thick posterior vomer and the atretic bony plate were resected and the mucosa was perforated. A balloon dilator was used to dilate the neochoanae and prevent restenosis. All six patients were intraoperatively stented for at least six weeks. RESULTS: All six neonates wit …

Presurgical orthopedics has no effect on archform in unilateral cleft lip and palate.

OBJECTIVE: Evaluation of the effect of presurgical orthopedics on maxillary archform up to 6 months of age. DeSIGN: Retrospective, single-blinded, case-control study. PARTICIPANTS: Study model sets of 75 infants with consecutive, nonsyndromic, complete unilateral cleft lip and palate (excluding Simonart bands) from 1995 to 2005. INTERVENTIONS: All patients (PSO group, n  =  14; non-PSO group, n  =  61) received lip repair/vomer flap at 3 months and soft palate repair at 6 months by the same consultant surgeon. The two groups were comparable at birth (p > .01) in all archform variables. MAIN…

Dental arches and occlusion in bilateral cleft lip and palate patients after two different routines for palatal surgery.

BACKGROUND: Delayed hard palate repair (DHPR) is believed by many researchers to improve maxillary growth and facial appearance in patients … g the first year of life. These latter subjects had undergone more traditional palatal surgery with vomer flaps for repair of the anterior part and push-back closure for the posterior part of the cleft. Surgery was performed in both groups by the same surgical team at Sahlgrenska University Hospital, Göteborg, Sweden. Dental casts were used to analyze t …

Palatal subunits analysis in unilateral cleft lip and palate compared to controls: A comparative cohort study.

BACKGROUND: It is essential to objectively evaluate how a unilateral complete cleft lip and palate (UCLP) alters palatal anatomy. This study defines palatal subunits, to assess tissue changes in distinct anatomical areas of the palate. Additionally, we describe the changes after passive presurgical plate therapy. PATIENTS AND METHODS: The palates of 21 newborns with non-syndromic UCLP were analyzed using 3D scans at two time points: post-birth (T1) and before primary surgery (T2). Subunits were defined as the premaxilla, alveolar crests, palatal segments, vomer, and true cleft. A control grou…

Revision choanal atresia repair.

OBJECTIVE: Since the first description of choanal atresia (CA) in 1755, multiple surgical techniques have been proposed to repair the malformation. Today, the transnasal endoscopic method is the most widely accepted repair technique. The bony choanal plates are perforated, the lateral choanae are drilled, and a portion of the vomer removed. Unfortunately, the problem of re-stenosis is a significant issue, and as a result, revision surgery may be a recurring theme in some children. We describe a successful revision endoscopic technique utilizing a microdebrider with straight and 120 degrees tel…

The double-reverse Z-plasty (Furlow) repair of soft palate clefts has the advantages of lengthening the soft palate while reconstructing the … mplete cleft of the hard palate, which were repaired by a modified Von Langenbeck technique or with vomer flaps. Mean age of patients was 15 months, and mean operating time (including placement of pressure-equalizing tubes) was 162 minutes, with an average estimated blood loss of less than 25 ml. Extracts from salivary glands stimulate aggression and inositol-1, 4, 5-triphosphate (IP3) production in the vomeronasal organ of mice.

Mammals use chemical cues to coordinate social and reproductive behaviors. Chemical cues are detected by the VNO organ (VNO), which is a cartilage-encased elongated organ associated with the vomer bone in the rostral nasal cavity. The resident intruder paradigm was utilized to examine the ability of saliva and its feeder exocrine glands, the submaxillary, parotid, and sublingual glands to mediate aggression in mice. Saliva and extracts from submaxillary and parotid glands, but not extracts from sublingual glands of male CD-1 mice, induced a greater number of attacks and lower latencies to snif…
