Outcome of wrist injuries with clinical suspicion of scaphoid injury after normal computed tomography.

OBJECTIVES: Wrist injuries are a common presentation to emergency departments. A clinical pathway has recently been introduced in our institution for patients with clinical suspicion of a scaphoid injury, using computed tomography (CT) at 2 weeks postinjury to rule out scaphoid fracture. However, it is not known what happens to patients who have a normal CT after such an injury. The aim of this pilot study was to assess disability after CT-negative wrist injuries. METHODS: A prospective questionnaire study was undertaken to assess disability in patients who had a normal CT after clinical susp…

MR imaging of traumatic and overuse injuries of the wrist and hand in athletes.

Traumatic and overuse injuries of the hand and wrist are common in athletes. Increasingly, MR imaging is being used to complement clinical and radiographic assessment in the diagnosis and management of these injuries. MR imaging is able to image accurately the bones, tendons, ligaments, nerves, and other small structures of the hand and wrist. This article provides an overview of traumatic and overuse injuries of the hand and wrist in athletes and a review of the MR imaging appearances.

Early active wrist mobilization in extensor tendon injuries in zones 5, 6, or 7.

Historically, flexor tendon injuries have been the focus of multiple treatment techniques, splinting approaches, and protocols. However, extensor tendon injuries can also have significant problems, such as extensor lag and wrist tenodesis. Because these deficits seem to have less of an effect on our daily functions, when compared to the flexors, less emphasis is seen in the literature. These authors have set up a splinting and exercise program that could help elevate our dealing with problems associated with healing extensor tendons.

Current concepts in the diagnosis and treatment of hand and wrist injuries in sports.

The hand is the most commonly injured part in sport. Fortunately, most injuries are minor and do not interfere with the sport participation, but even these leave residual functional deficits that may increase with time. More severe injuries of the hand are appreciated instantly by the player and health care personnel and generally receive better care. The goal is to maintain the best functioning hands now for the sport and in the future for the life's work of the hand. A program is herein described outlining the care of minor and major sports injuries to the hand that tries to return the injur

Analysis of Severity of Finger, Hand, and Wrist Injuries Among Department of Air Force Workers.

OBJECTIVE: This study examined US Department of the Air Force (DAF) work-related finger, hand, and/or wrist (FHW) injuries and focused on injury characteristics, severity, and outcomes. METHODS: Characteristics, details, and severity of work-related, noncombat FHW injuries (2008-2018) were examined. Rates, patterns of severity, and identification of at-risk populations within DAF were calculated. RESULTS: Nearly a quarter of DAF injuries were FHW-related and decreased from 2008 to 2018. The highest rates were among males, younger workers, and enlisted workers. Odds of severe injury increased

Hand and Wrist Dog-Leash Injuries in the Outpatient Setting: A Review of 443 Cases.

BACKGROUND: Injuries sustained from dog leashes often result in visits to hand surgeons. This study presents an analysis of dog-leash-related upper-extremity injuries at a single orthopedic practice, including treatment options for these injuries, and guidance for prevention. METHODS: We identified all outpatients who presented to a hand surgeon at a single institution from 2016 to 2021 following an upper-extremity injury related to a dog leash. Charts were reviewed and demographics, injury description and mechanism, and details of treatment were recorded. RESULTS: We identified 443 patients

Combined nerve, vessel, and tendon injuries of the volar wrist: Multidisciplinary treatment and functional outcomes.

The superficial location of critical structures, including tendons, nerves and vessels, in the volar surface of the wrist makes them vulnerable to penetrating trauma. Extensive injuries to these structures are described as "spaghetti wrist". The main objective of this study was to report functional outcome in spaghetti-wrist injuries. The records of patients presenting to our clinic with extensive volar wrist injuries between January 2016 and January 2019 were reviewed retrospectively. Age, gender, comorbidities, date of injury, injury mechanism, affected hand and transected structures were no…

[Results of primary repair of injuries to the median and ulnar nerves at the wrist].

INTRODUCTION: Wounds on the palmar side of the wrist affecting the median or ulnar nerves are responsible for motor and sensory sequelae, severe pain and cold intolerance. MATERIALS AND METHODS: Thirty-nine patients with 40 nerve sections were retrospectively reviewed with a mean follow up of 23 months. The median nerve alone was affected 20 times, the ulnar nerve seven times and both nerves simultaneously 13 times. In 75% of the cases, there was an associated vascular injury (radial artery and/or ulnar artery). The average number of tendons cut was 4.25. RESULTS: After repair of the median

The Enduro motorcyclist's wrist and other overuse injuries in competitive Enduro motorcyclists: a prospective study.

According to public opinion, Enduro motorcycling is a dangerous sport. Little is known about overuse injuries in connection with this sport. The study consisted of two phases. Phase 1 was planned as a prospective feasibility study for phase 2. The latter was initiated as a prospective investigation, comprising a random sample of the participants of phase 1. Enduro motorcyclists were interviewed with a prepared questionnaire. Clinical examinations were performed immediately before, and after, a major Enduro race, including pain assessment in the hand/wrist and the forearm using the visual analo

Treatment of injuries to the ulnar side of the wrist occurring with distal radial fractures.

We still do not know how to best treat the DRUJ condition that was recognized 200 years ago by Abraham Colles and later addressed in Frykman's classic thesis. To improve the outcome, we must recognize the differences be-tween osteoporotic and other fractures and understand the importance of ligament injuries, especially in patients under the osteoporotic age. However, our current problem is that neither the initial ligament injury nor the posttraumatic laxity is detectable with radiographic methods, which creates future challenges regarding diagnosis and treatment. We therefore have to criticall

Self-Inflicted Wrist Injuries in Flexor Tendon Zone V: Clinical Patterns, Management Strategies, and Outcomes.

Self-inflicted wrist injuries, particularly those involving flexor tendon zone V, constitute a distinct and clinically challenging subset of upper-extremity trauma with profound surgical, functional, and psychosocial implications. For every suicide, many involve wrist lacerations capable of compromising tendinous, neural, and vascular structures and resulting in long-term disability. Despite their clinical relevance, the literature remains fragmented, with no universally accepted definition of "spaghetti wrist," a term originally described in early surgical series of extensive volar wrist lace…

Epidemiology of work-related hand and wrist injuries in a referral center: A descriptive study.

BACKGROUND: Occupational hand and wrist injuries (OHWIs) account for 25% of work-related accidents in low- and middle-income countries. In Colombia, more than 500000 occupational accidents occurred in 2021, and although the rate declined to less than 5% in 2020 and 2021, at least one in four accidents involved a hand or wrist injury. AIM: To describe the OHWIs in workers seen at the emergency room at a second-level hospital in Colombia. METHODS: An observational study was performed using data from workers who experienced OHWIs and attended a second-level hospital, between June, 2020 and May,…

Dorsal Impaction Injuries of the Distal Radius: Operative Technique Through Dorsal Approach to the Wrist.

Distal radius fractures vary widely in fracture pattern and displacement. Impaction injuries involving the dorsal articular surface of the distal radius can present challenges when anatomic reduction and fixation is attempted through a standard volar approach. Dorsal approach to the distal radius can provide direct visualization of these fracture patterns, greatly facilitating anatomic reduction and stabilization. In this technique video, surgical approach, fracture reduction, and operative fixation of a dorsally impacted, intra-articular distal radius fracture through a dorsal approach is pre

Time heals all wounds - Outcomes of deep palmar wrist injuries may improve long after reinnervation.

Despite the clinical importance of deep wrist injuries (DWIs), data on the timeframe of possible improvements in hand function are scarce. We tested the hypotheses that a) the length of follow-up is positively correlated with the outcome, and b) this correlation is tempered by nerve involvement. All patients admitted to the Clinic of Plastic Surgery with an acute DWI between 2008 and 2016 were contacted for a follow-up examination including two-point discrimination, range of motion, grip, and pinch strength, as well as DASH and MMWS questionnaires and employment status. Possible confounders su…

Epidemiology of hand and wrist injuries treated in a reference specialty center over a year.

INTRODUCTION: Hand injuries constitute 6.6% to 28.6% of all injuries in the musculoskeletal system. Little information has been reported on the non-laboring, non-insured population with no social security. We describe the epidemiology of hand and wrist injuries treated over a year in the emergency department of a reference hospital in Mexico City that treats patients with no social security. OBJECTIVE: To describe the epidemiology of hand and wrist injuries treated in the ED of a high specialty reference medical facility over a year. MATERIAL AND METHODS: Medical records for patients treated…

VIDEO: Dynamic Ultrasound Evaluation for Soft-Tissue Injuries of the Extremities: Hand, Wrist, and Ankle.

OBJECTIVE. The purpose of this article is to show the sonographic anatomy of the extremities relevant to various ligamentous, tendinous, and articular injuries occurring in the hand, wrist, and ankle. A brief discussion and depiction of the specific elements relevant to the pathophysiologic mechanism of these entities is followed by demonstrations of the dynamic ultrasound techniques that can be used to diagnose these injuries. The schematics and video clips illustrate the normal and pathologic features of these injuries. The first two videos discuss soft-tissue injuries to the hand and wrist,
