Towards the Development of an Integrative, Evidence-Based Suite of Indicators for the Prediction of Outcome Following Mild Traumatic Brain Injury: Results from a Pilot Study.

BACKGROUND: Persisting post-concussion symptoms (PPCS) is a complex, multifaceted condition in which individuals continue to experience the symptoms of mild traumatic brain injury (mTBI; concussion) beyond the timeframe that it typically takes to recover. Currently, there is no way of knowing which individuals may develop this condition. METHOD: Patients presenting to a hospital emergency department (ED) within 48 h of sustaining a mTBI underwent neuropsychological assessment and demographic, injury-related information and blood samples were collected. Concentrations of blood-based biomarkers

Use of high b value diffusion-weighted magnetic resonance imaging in acute encephalopathy/encephalitis during childhood.

AIM: To determine the use of high b value diffusion-weighted imaging (DWI) in the diagnosis and assessment of acute febrile encephalopathy/encephalitis in childhood. SUBJECTS AND METHODS: We enrolled 22 children, for whom we examined DWI with b=1000s/mm2, DWI with b=3000s/mm2, and apparent diffusion coefficient (ADC) map with b=1000 during the acute phase of febrile encephalopathy/encephalitis. Clinical diagnoses included acute encephalopathy with biphasic seizures and late reduced diffusion (AESD; n=6), clinically mild encephalopathy/encephalitis with a reversible splenial lesion (MERS; n=6)

Febrile status epilepticus and epileptogenesis: The FEBSTAT study.

The multicenter FEBSTAT study (Consequences of Prolonged Febrile Seizures in Childhood: https://grantome.com/grant/NIH/R37-NS043209-12; PI S. Shinnar) examined the outcome of febrile status epilepticus (FSE) in over 200 prospectively enrolled infants, with many followed for 10 years after FSE. Initial magnetic resonance images (MRIs), done within days after FSE, showed a unilateral hyperintense hippocampal T2 signal in 10% of subjects. Post-ictal EEGs showed focal slowing maximal in the temporal region in 24% and focal attenuation in 13% of all subjects. There was a significant association bet

TACKLE concussion study: an observational study of post-concussion biomarker and neuroimaging progression in adolescent male school sports-related concussion.

INTRODUCTION: Sports-related concussion (SRC) is an established research topic in the context of sport professionals suffering from mild traumatic brain injury (mTBI), but there is scant investigation of SRC arising in school-aged athletes. Effective management of SRC in adolescents is especially dependent on obtaining an understanding of its pathophysiology and the multifaceted nature of recovery. In this planned observational study, we shall investigate the associations among multimodal data comprising blood-based and saliva-based biomarkers, diffusion tensor imaging (DTI), quantified suscep

Inhibitory saccadic dysfunction is associated with cerebellar injury in multiple sclerosis.

Cognitive dysfunction is common in patients with multiple sclerosis (MS). Saccadic eye movement paradigms such as antisaccades (AS) can sensitively interrogate cognitive function, in particular, the executive and attentional processes of response selection and inhibition. Although we have previously demonstrated significant deficits in the generation of AS in MS patients, the neuropathological changes underlying these deficits were not elucidated. In this study, 24 patients with relapsing-remitting MS underwent testing using an AS paradigm. Rank correlation and multiple regression analyses wer

Epileptiform activity contralateral to unilateral hippocampal sclerosis does not cause the expression of brain damage markers.

OBJECTIVE: Patients with epilepsy often ask if recurrent seizures harm their brain and aggravate their epileptic condition. This crucial question has not been specifically addressed by dedicated experiments. We analyze here if intense bilateral seizure activity induced by local injection of kainic acid (KA) in the right hippocampus produces brain damage in the left hippocampus. METHODS: Adult guinea pigs were bilaterally implanted with hippocampal electrodes for continuous video-electroencephalography (EEG) monitoring. Unilateral injection of 1 μg KA in the dorsal CA1 area induced nonconvulsi

Noninvasive Monitoring of Changes in Cerebral Hemodynamics During Prolonged Field Care for Hemorrhagic Shock and Hypoxia-Induced Injuries With Portable Diffuse Optical Sensors.

INTRODUCTION: Achieving simultaneous cerebral blood flow (CBF) and oxygenation measures, specifically for point-of-care injury monitoring in prolonged field care, requires the implementation of appropriate methodologies and advanced medical device design, development, and evaluation. The near-infrared spectroscopy (NIRS) method measures the absorbance of light whose attenuation is related to cerebral blood volume and oxygenation. By contrast, diffuse correlation spectroscopy (DCS) allows continuous noninvasive monitoring of microvascular blood flow by directly measuring the degree of light sca

Adjunctive remote ischaemic postconditioning and hypothermia therapy in acute encephalopathy with biphasic seizures.

BACKGROUND: Acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) is a common form of acute infection-triggered encephalopathy in children. This study aimed to evaluate the safety and feasibility of remote ischaemic postconditioning (RIPoC) as an adjunct to therapeutic hypothermia in patients with AESD. METHODS: In this prospective, non-randomised study, patients treated in January 2021-July 2024 were compared with a historical control group treated in January 2017-December 2020. RIPoC (four cycles of 5-min inflation and 5-min deflation) was applied to the lower extrem

Utility of Diffusion Tensor Imaging in Preterm Infants with Germinal Matrix Hemorrhage and Intraventricular Hemorrhage: A Systematic Review.

OBJECTIVE: Intraventricular hemorrhage (IVH) and germinal matrix hemorrhage (GMH) are the most common brain injuries in preterm infants. Neonates with these injuries are at greater risk of impaired neurodevelopmental outcome. Current guidelines recommend screening infants with cranial ultrasound (CUS); however, this is prone to missing subtle injury patterns, particularly within the posterior fossa. The present report sought to discuss the utility of diffusion tensor imaging (DTI) in preterm infants. METHODS: A systematic review of PubMed, Ovid MEDLINE, and Ovid EMBASE was conducted following

Chronic functional deficits following a single closed head injury in mice are prevented by minocycline and N-acetyl cysteine.

Traumatic brain injury (TBI) can produce chronic limb coordination and gait deficits that are associated with ongoing white matter damage. In rodent TBI models, chronic motor deficits may be obscured by aging or motor compensation. In addition, there are no treatments for TBI. The murine closed head injury (CHI) model produces diffuse, chronic white matter injury that may underlie chronic white matter dysfunction and motor deficits. Evoked compound action potentials (CAP) assess corpus callosum function from 3 to 180-days post injury (DPI). CHI acutely decreases total CAP amplitudes that recov

The effects of sleep and light at night on melatonin in adolescents.

OBJECTIVE: There is increasing awareness among physicians of the risks of traumatic brain injury (TBI)-induced hypopituitarism. We have assessed the prevalence and risk factors of post-traumatic hypopituitarism by analyzing the TBI database of the University of Pecs. DESIGN: This consecutive analysis of 126 TBI survivors (mean age: 42.4 years, average follow-up time: 48 months) revealed that 60.3% had severe and 39.7% moderately severe trauma based on GCS score. Subdural hemorrhage (29.3%) and diffuse injury (27%) were the most common types of injury; 17.5% of patients suffered basal skull fr

Isolated brainstem death: case presentation demonstrating preserved supratentorial perfusion on 99mTc-bicisate brain scintigraphy.

BACKGROUND: The diagnosis of brain death (BD) is primarily clinical and usually straightforward when both supratentorial and infratentorial structures are irreversibly injured. In contrast, isolated catastrophic posterior fossa lesions-particularly those involving the brainstem-are uncommon and may demonstrate preserved supratentorial perfusion or cortical electrical activity on ancillary testing, even in the absence of brainstem function. This apparent dissociation is typically transient and invariably progresses to global cerebral circulatory arrest and death. Few reports have documented the

Brain tumour post-treatment imaging and treatment-related complications.

PURPOSE: The imaging of primary and metastatic brain tumours is very complex and relies heavily on advanced magnetic resonance imaging (MRI). Utilisation of these advanced imaging techniques is essential in helping clinicians determine tumour response after initiation of treatment. Many options are currently available to treat brain tumours, and each can significantly alter the brain tumour appearance on post-treatment imaging. In addition, there are several common and uncommon treatment-related complications that are important to identify on standard post-treatment imaging. METHODS: This art

What about guidelines for out-of-hospital cardiac arrest with unusual cardiac anatomy?

Conventional management of out-of-hospital cardiac arrest (OOHCA) in patients with unusual cardiac anatomy may result in inferior outcomes. It is important that appropriate consideration of these patients is made when developing OOHCA resuscitation guidelines. Initiatives to improve outcomes following OOHCA, such as community training of resuscitation techniques, should take patients with unusual cardiac anatomy into account. We present a male in his twenties with heterotaxy syndrome, a congenital defect with abnormally arranged intrathoracic and intra-abdominal organs. He experienced an unwit

From clinical observation to experimental validation: Investigating the neurotoxic impact of dimethylacetamide.

Dimethylacetamide (DMAC) is a polar organic solvent widely used in the production of synthetic fibers and other industrial applications. Its hepatotoxic potential has been well documented in laboratory animals and occupationally exposed workers, establishing DMAC as a recognized industrial hazard. However, evidence regarding its neurological effects remains scarce. We report a clinical case of accidental DMAC inhalation associated with diffuse cortical hyperintensity on brain magnetic resonance imaging, raising concern for direct neurotoxicity. To address this knowledge gap, we investigated th

Structural-Functional Dissociation in TBI Hemiplegia: Meridian-Sinew Therapy Promotes Motor Recovery Despite Corticospinal Tract Damage-A Case Report.

This study presents neuroimaging analysis of a 31-year-old male patient with hemiplegia secondary to severe traumatic brain injury who was treated with an external therapy of Traditional Chinese Medicine (TCM) called meridian-sinew therapy. The patient exhibited complete right-sided limb paralysis following trauma, with full muscle strength recovery after therapy. Serial diffusion tensor imaging evaluations revealed persistently reduced fractional anisotropy values in the left corticospinal tract (CST.L) below the normal range posttreatment, indicating a structure-function decoupling phenomeno
