A female infant born at 28 weeks gestational age, weighing 570 g, developed retinopathy of prematurity (ROP) which progressed to threshold disease in one eye. Results are presented from a case material of 22 prematurely born infants (43 eyes) who received cryotherapy for retinopathy of prematurity (ROP); 20 eyes were treated at stage 3, 14 at stage 3 plus, and nine at stage 4A. Follow-up examinations at the age of 26 to 52 months (average 38 months) showed regression of ROP in all eyes and visual acuities of 6/60 or better. The timing of cryotherapy is discussed. In a prospective, randomized clinical trial comparing transscleral cryotherapy with laser photocoagulation in the treatment of "threshold" stage 3+ retinopathy of prematurity, 32 infants were treated with diode laser photocoagulation in one eye. Fifty eyes of 25 preterm infants with stage 3+ retinopathy of prematurity (ROP) and 180 degrees to 360 degrees of preretinal neovascularization were treated with cryotherapy to ablate the peripheral zone of avascular retina. All neovascularization regressed. Eleven eyes developed traction detachments, eight of which were successfully reattached with scleral buckling surgery. The clinical findings of 20 patients with an isolated peripheral retinal mass secondary to retinal telangiectasis were reviewed. The important clinical features were subretinal exudation, vitreous haemorrhage and retinal detachment. No associated systemic abnormalities were identified. Early results suggest that cryotherapy is effective for the reduction of subretinal exudation but not for macular oedema. Patients with extensive retinal detachment complicated by vitreous haemorrhage and epiretinal membrane formation were treated with vitreoretinal surgery.

A total of 114 children (age range 4 months to 18 years) underwent definitive operation for life-threatening or incessant tachydysrhythmias resulting from accessory conduction pathways (Kent bundle) (79), atrial ectopic foci (18), or ventricular ectopic foci (17). Various models of macular edema have been studied; however, frank development of a prototypical cystoid macular edema has not been observed. We treated two cases of endophthalmitis caused by strabismus surgery. In one patient, a scleral perforation was recognized during surgery and treated with cryopexy. In the other, a scleral perforation was suspected but never confirmed. Despite intensive antibiotic and corticosteroid therapy, visual acuity decreased to light perception in Case 1 and to counting fingers in Case 2. The second eye was enucleated eight years later after it had become blind and painful.

Three cases of perforated Pseudomonas corneal ulcers with scleral extension were treated with keratoplasty and cryotherapy to the remaining cornea and sclera. All three cases showed dramatic improvement. Cryotherapy of Pseudomonas keratitis in guinea pigs has been shown to be effective. Pseudomonas organisms seem to be susceptible to cryotherapy in vivo; there is no effect in vitro. Cryotherapy may prove to be a new tool in the treatment of Pseudomonas keratitis.
