Skin condition associated with intensive use of alcoholic gels for hand disinfection: a combination of biophysical and sensorial data.

Although hand hygiene is an important and inexpensive measure to prevent nosocomial infections in clinical settings, the compliance of healthcare workers remains low. In Europe, alcoholic hand disinfection is first choice, but there exists a limited user acceptability due to estimated adverse effects on skin condition. This study was designed to investigate skin tolerance to alcohol-based disinfecting gels and changes in skin condition depending on humectant concentration, alcohol grades, as well as type of alcohol used. A comparison of 6 alcohol-based gels was made based on a randomized doubl

[Anesthesia for hand surgery in patients with bullous epidermolysis].

Epidermolysis bullosa is a rare genetic disease, characterized by the formation of bullae in the skin and squamous epithelium following minimal trauma. The majority of surgical indications specific to this disease concern autosomal recessive dystrophic epidermolysis bullosa (RDEB). Hand surgery is one of the fields of surgery involved. The cicatricial course of the lesions results in retraction and pseudosyndactylization of the fingers, sometimes leading to complete destruction of the hand. Since 1988, we have treated 23 patients, including 11 children under the age of 8 years and weighing les

Isothiazolinones in Disposable Rubber Gloves-Results of Chemical Analysis.

BACKGROUND: Contact allergy to benzisothiazolinone (BIT) has increased in recent years, but exposure to it is not always found. It's important to know whether the gloves might be an isothiazolinone source especially in patients with symptoms from gloves and isothiazolinone contact allergy with or without allergy to rubber chemicals. OBJECTIVES: To present results of chemical analysis of isothiazolinones in disposable rubber gloves of patients in an occupational dermatology clinic. METHODS: We went through our chemical analysis record from 2018-2025 and identified isothiazolinone analyses don

Thresholds for Contact Allergy to a Rubber Accelerator: TETD-Determined by Serial Dilution Patch Testing and a Simulated Glove-Use Model.

BACKGROUND: Rubber accelerators found in rubber gloves may cause contact allergy and thus allergic contact dermatitis. OBJECTIVES: To investigate which doses of tetraethylthiuram disulfide (TETD) elicit allergic contact dermatitis by patch testing and in a repeated occluded application test simulating glove use. PATIENTS AND METHODS: A multicentre study was conducted. Eight patients with contact allergy to TETD and eight controls comprised the study group. Doses of TETD 1% (400 μg/cm2), 0.33% (130 μg/cm2), 0.11% (44 μg/cm2), 0.037% (15 μg/cm2), 0.012% (4.9 μg/cm2), 0.0041% (1.6 μg/cm2) and p

[Cohort study for long-term follow-up of patients in whom the so-called "molecular classifier" is used to distinguish eczema from psoriasis : Background and implementation].

A cohort of occupational dermatology patients will be set up at the University Hospital Heidelberg, Germany. In a 3-year prospective cohort study, the so-called molecular classifier will be applied in 262 patients to better differentiate between eczema and psoriasis. A retrospective cohort of 282 occupational health patients with the same suspected diagnosis but no molecular classifier designation was used as a control group. In 5 follow-up visits during 3 years, data will be obtained on diagnoses, disease course and severity, absence from work, occupation and quality of life. The research que

Occupational hand dermatitis web survey in a university hospital during COVID-19 pandemic: the SHIELD study.

BACKGROUND: Occupational hand dermatitis (OHD) is a skin disease occurring on employees' hands in certain jobs. Little is known about prevalence, incidence and characteristics of this adverse skin reaction and its associated risk factors during COVID-19 pandemic. To evaluate both prevalence and incidence of OHD and associated risk factors in Italian clinicians. METHODS: A cross-sectional study was performed using a self-report questionnaire. RESULTS: Two hundred and thirty clinicians responded to the survey and 82% of responders did not report previous OHD history before the COVID-19 pandemi

Onychodystrophy as the only sign of congenital candidiasis.

Congenital candidiasis infection often presents as a skin rash with variable involvement of nails and mucous membranes. Isolated nail involvement is rare, may present late, and can often be managed with topical antifungal medication. We report a case of congenital candidiasis limited to the fingernails that resolved completely within 3 months with topical treatment.

Hand dermatitis--differential diagnoses, diagnostics, and treatment options.

The pathogenesis of hand dermatitis is multifactorial, and includes factors such as genetic predisposition and exposure. A high incidence rate is associated with female gender, contact allergy, atopic dermatitis, and wet work. The most important risk factors for the persistence of hand dermatitis include its extent, contact allergic or atopic etiology, childhood dermatitis, and early onset (before the age of 20). The cost of illness of hand dermatitis corresponds to this seen in moderate to severe psoriasis. The diagnostic workup of hand dermatitis and its differential diagnoses requires a det

Abnormal wound healing related to high-dose systemic corticosteroid therapy in a patient with Ehlers-Danlos syndrome benign hypermobility Type.

The adverse impact of chronic corticosteroid therapy on wound healing has been well characterized, as has abnormal wound healing known to occur in the classic type of Ehlers-Danlos syndrome (EDS). In contrast, abnormal wound healing is absent in cases of EDS benign hypermobility type (EDS-BHT). We present the case of a patient with EDS-BHT with no history of abnormal wound healing who developed large nonhealing ulcers to sites of minor trauma after initiating therapy with high-dose systemic corticosteroids for dermatomyositis. This case provides a dramatic illustration of the effects of chroni

Hand eczema among hairdressing apprentices in Denmark following a nationwide prospective intervention programme: 6-year follow-up.

BACKGROUND: Hand eczema is the commonest occupational skin disease in Denmark, and hairdressing is a high-risk profession. In 2008-2010, a clinically controlled, prospective intervention study aimed at reducing the development of hand eczema was conducted at hairdressing schools in Denmark. The findings showed that significantly fewer apprentices in the intervention group developed hand eczema over a period of 18 months. OBJECTIVES: To investigate the long-term effect of the intervention. METHODS: Two hundred and eighty-four participants were identified from the original dataset, and were se

Guidelines for diagnosis, prevention and treatment of hand eczema.

The guidelines aim to provide advice on the management of hand eczema (HE), using an evidence- and consensus-based approach. The guidelines consider a systematic Cochrane review on interventions for HE, which is based on a systematic search of the published literature (including hand-searching). In addition to the evidence- and consensus-based recommendation on the treatment of HE, the guidelines cover mainly consensus-based diagnostic aspects and preventive measures (primary and secondary prevention). Treatment recommendations include non-pharmacological interventions, topical, physical and s

Antisynthetase syndrome: not just an inflammatory myopathy.

In recent years, antisynthetase syndrome has been recognized as an important cause of autoimmune inflammatory myopathy in a subset of patients with polymyositis and dermatomyositis. It is associated with serum antibodies to aminoacyl-transfer RNA synthetases and is characterized by a constellation of manifestations, including fever, myositis, interstitial lung disease, "mechanic's hands," Raynaud phenomenon, and polyarthritis. Physicians should be familiar with its variety of clinical presentations and should include it in the differential diagnosis in patients presenting with unexplained inte
