Pneumatosis cystoides intestinalis in a trauma patient presenting with pneumoperitoneum: a case report.

BACKGROUND: Pneumatosis cystoides intestinalis is a rare and usually benign condition in which multiple thin-walled cysts develop in the submucosa or subserosa of the gastrointestinal tract. While usually asymptomatic, severe cases can result in pneumoperitoneum, which can be managed surgically or medically depending on circumstances. We present a case of a patient with pneumatosis cystoides intestinalis, which presented as pneumoperitoneum following trauma. To our knowledge, there are no other published cases in which a trauma patient with pneumoperitoneum was found to have radiologic evidenc

Use and Misuse of Opioids After Gynecologic Surgical Procedures.

OBJECTIVE: To examine the rate of opioid use for gynecologic surgical procedures and to investigate persistent opioid use among those women who received an initial opioid prescription. METHODS: A retrospective cohort study using the MarketScan database was performed. MarketScan is a claims-based data source that captures claims from more than 50 million privately insured patients and 6 million Medicaid enrollees from 12 states. We identified women who underwent major and minor gynecologic surgery from 2009 to 2016. Among women who received an opioid prescription, new persistent opioid use was

Reoperation rates after anterior cervical discectomy and fusion versus posterior cervical foraminotomy: a propensity-matched analysis.

BACKGROUND CONTEXT: Anterior cervical discectomy and fusion (ACDF) and posterior cervical foraminotomy (PCF) are both used to surgically treat patients with cervical radiculopathy and have been shown to have similar outcomes. Nonetheless, ACDF has become increasingly more commonplace compared with PCF, in part because of a pervasive belief that PCF has a higher incidence of required reoperations. PURPOSE: To determine the reoperation rate at the index level of ACDF versus PCF 2 years postoperatively. STUDY DESIGN: A retrospective case-control. PATIENT SAMPLE: All patients that underwent ACD

A Decade of Craniosynostosis Surgery in the United States: Trends in Management, Payer Mix, and Cost.

BACKGROUND: Cranial vault remodeling (CVR) and endoscopic strip craniectomy (SC) are principal operations for nonsyndromic craniosynostosis. Over the past decade, evolving minimally invasive techniques and shifting payer landscapes may have influenced national practice patterns. We evaluated trends in surgical utilization, payer mix, and cost across US children's hospitals from 2016 to 2025. METHODS: A retrospective analysis of the Pediatric Health Information System identified infants with craniosynostosis who underwent CVR or SC between 2016 and 2025 using ICD and CPT codes. Demographics, p

Impact of In-Hospital Osteoporosis Treatment on Post-discharge Medication Continuity in Osteoporotic Fracture Patients: Implications for Care Continuity.

BACKGROUND: Despite the well-established benefits of osteoporosis medication in reducing the risk of secondary fractures and mortality, post-fracture treatment rates remain suboptimal. The hospitalization period represents a critical opportunity to initiate therapy, yet the impact of in-hospital osteoporosis treatment on long-term adherence has not been fully evaluated in the Korean population. METHODS: This retrospective cohort study used data from the National Health Insurance Service sample cohort in South Korea. Patients aged ≥ 50 years who were hospitalized for osteoporotic fractures, in

Does the quality of pain relief after major surgery influence the risk of postoperative complications? A prospective observational study.

OBJECTIVES: Effective postoperative acute pain management continues to be a challenge. It remains uncertain whether poorly controlled postoperative pain influences the risk of postoperative complications. Therefore, we aimed to investigate whether indicators of poor pain control increase the likelihood of cardiac, pulmonary, infectious, thromboembolic, and surgical complications, as well as of prolonged use of analgesics. METHODS: This prospective observational study combines treatment data from the German net-ra registry and claims data from the second-largest public health insurer BARMER (M

Trends in utilization, timing, and outcomes of thoracic endovascular repair for type B aortic dissection in the United States.

BACKGROUND: Aortic dissection is the most common acute aortic syndrome in the United States. Type B aortic dissection (TBAD) can be managed medically, through open surgical repair, or with thoracic endovascular repair (TEVAR). The present study sought to assess contemporary trends in the use and timing of TEVAR. METHODS: Adult nonelective TBAD admissions were identified in the 2010 to 2020 Nationwide Readmissions Database. Patients were categorized as medical management (Medical Management), TEVAR at initial hospitalization (Early), or TEVAR during readmission (Delayed). Multivariable models

Sick leave and disability pension among TMD patients with musculoskeletal diseases, mental and behavioural disorders - a SWEREG-TMD population-based cohort study.

BACKGROUND: Temporomandibular disorders (TMD) are associated with musculoskeletal diseases (MSD), mental and behavioural disorders (MBD), and patients with TMD have been shown to have 2-3 times more days of sick leave (SL) and disability pension (DP) than the general population. MSD and MBD are two of the most common causes for SL and DP, and the association between TMD and the influence of comorbidities on the need for SL and DP among TMD patients need further clarification. This study investigates the impact of MSD and MBD comorbidity on SL and DP among TMD patients diagnosed in a hospital s

Dorsal Root Ganglion Stimulation as a Salvage Treatment for Complex Regional Pain Syndrome Refractory to Dorsal Column Spinal Cord Stimulation: A Case Series.

OBJECTIVE: The efficacy of traditional spinal cord stimulation (t-SCS) tends to decay over time in patients with complex regional pain syndrome (CRPS). While it has been shown that dorsal root ganglion (DRG) stimulation is extremely effective in t-SCS-naïve patients with CRPS, its efficacy in patients who had previously failed t-SCS is unknown. Given that DRG-SCS and t-SCS target different spinal pathways, a failure with t-SCS should not automatically preclude a patient from attempting DRG-SCS. MATERIALS AND METHODS: Two patients with lower extremity CRPS, previously implanted with t-SCS syst

Quality of life after breast reduction surgery: a 10-year retrospective analysis using the Breast Q questionnaire: does breast size matter?

BACKGROUND: Patient reported outcome studies are extremely valuable. We hypothesize that breast reduction surgery has a high satisfaction rate, and that body mass index (BMI), and the amount of breast tissue removed would not alter this outcome. The purpose of this study was to determine if there is an improvement in the quality of life in patients undergoing breast reduction surgery, and whether breast volume or patient size influences this outcome. METHODS: A retrospective chart review was performed on 600 consecutive patients at a single institution from 2000 to 2010. Patients were mailed

The use of radial vessel stump in free radial forearm flap as flap monitor in head and neck reconstruction.

The versatile use of the free radial forearm flap for the reconstruction of pharyngoesophageal defects has been proven ideal in previous studies. However, the monitoring of flap viability when buried underneath the skin in head and neck surgery still posed technical difficulties. An innovative monitoring method for the buried free radial forearm flap by using distal radial vessel stump elevated over the skin is to be presented here. Eighteen patients received free radial forearm flap reconstruction for their pharyngoesophageal defects after tumor ablation during June 2003 to March 2005. All pa

GPT-4o-Powered Preanesthetic AI: Development and Validation Study.

BACKGROUND: Accurate preanesthetic assessment is essential for perioperative risk stratification, but conventional tools such as the American Society of Anesthesiologists (ASA) physical status classification and postoperative nausea and vomiting (PONV) risk scores may be affected by subjective judgment, incomplete documentation, and fragmented clinical data. Large language models may support preanesthetic assessment by integrating structured and unstructured clinical information. OBJECTIVE: This study aimed to develop and retrospectively validate a GPT-4o (OpenAI)-powered AI system for preane

Development and Validation of a Bedside Risk Assessment for Sustained Prescription Opioid Use After Surgery.

IMPORTANCE: The increased use of prescription opioid medications has contributed to an epidemic of sustained opioid use, misuse, and addiction. Adults of working age are thought to be at greatest risk for prescription opioid dependence. OBJECTIVE: To develop a risk score (the Stopping Opioids After Surgery score) for sustained prescription opioid use after surgery in a working-age population using readily available clinical information. DESIGN, SETTING, AND PARTICIPANTS: In this case-control study, claims from TRICARE (the insurance program of the US Department of Defense) for working-age ad

Mapping the gendered dynamics of cesarean section: A scoping review.

Cesarean section (CS) is a lifesaving obstetric intervention when medically indicated, yet global rates have risen sharply over recent decades, with substantial variation across and within countries. While underuse persists in many low-resource settings, overuse is increasingly documented in higher-income and private-sector facilities, raising concerns about inequitable access, unnecessary surgical intervention, and divergent quality of maternity care. Existing explanations have largely focused on clinical indications and biomedical risk profiles, often neglecting the social and gendered struc

Factors Associated with Presentation of Severe Adolescent Idiopathic Scoliosis.

UNLABELLED: Background: Adolescent idiopathic scoliosis (AIS) is a common referral to pediatric orthopaedic surgeons. Timely treatment with appropriate bracing decreases the risk of curve progression and need for surgical intervention. Despite pediatrician screening, patients still present to orthopaedic surgeons with curve magnitudes too large for nonoperative bracing to prevent progression to surgical curve magnitude. The aim of this study was to identify the sociodemographic differences between patients who are initially diagnosed with mild versus moderate/severe AIS.Methods: This retrospec
