Cholelithiasis, cholecystitis, and common bile duct stones.

Cholecystitis and cholelithiasis are being recognized with increasing frequency in infancy, childhood, and adolescence. Hematologic disorders account for a large proportion of cases; however, in most cases the etiology is uncertain. Infants and children are noted with stones in association with total parenteral nutrition, prolonged fasting, or ileal resection. Biliary dyskinesia, a disorder of impaired gallbladder contractility, is being recognized with increased frequency in late childhood and teenage years. Spontaneous stone resolution is frequently noted in infancy, and a period of observat

[Chronic pancreatitis in Cameroon. Analysis of etiological and clinical aspects].

In a collection of 3,571 patient's files admitted in the University Teaching Hospital and the Yaounde General Hospital, we studied 27 patients suspected of chronic pancreatitis. 20 patients with calcified chronic pancreatitis benefited from a detailed history, physical examination and a complete paraclinical work-up. From the data collected, chronic alcoholism seemed to have been the main aetiology. Industrial beer from barley alone and/or associated with other traditional liquors was most consumed. The majority of patients were heavy alcoholics and daily consumption varied from 75 to 124 g of

Biliary concrements: the endoscopic approach.

Since 1970, endoscopic retrograde cholangiopancreaticography (ERCP) has received much attention as a diagnostic method in pancreaticobiliary disease. The information given by ERCP is of great value in the management of patients with symptoms after biliary tract surgery and especially in patients with suspected obstructive jaundice. In patients with symptoms highly suggestive of cholelithiasis, ERCP examination should be considered for definitive diagnosis despite a negative oral cholecystogram and ultrasound finding. Endoscopic papillotomy is a major advance in the treatment of many elderly an

[Experiences with preoperative and palliative percutaneous transhepatic cholangio-drainage].

In obstructive jaundice PTCD plays an important role as a preoperative or a palliative therapeutic method inspite of the availability of biliary endoprostheses. This study was carried out to assess complications following PTCD, the effect on liver function (serum parameters) and the relationship between duration of preoperative drainage and postoperative complication rate. The results demonstrate that 11-hole pigtail-catheters are useful for intrahepatic location and 32-hole ringcatheters for extrahepatic location due to reduced dislocation rates. The effect of biliary drainage after 7 rsp. 25

New methods in gastroenterology.

Major advances in gastroenterology are due in part to the rapid development of fiberendoscopes. Originally intended to improve gastroenterological diagnostics, the field of application was broadened by a variety of therapeutic procedures which now concur with the corresponding surgical approach. Endoscopic electro- and photocoagulation has considerably improved the outcome of acute gastrointestinal hemorrhage: endoscopic polypectomy is the procedure of choice today in benign and occasionally in malignant bowel tumors. Biliary tract surgery was revolutionized by endoscopic sphincterotomy, offer

Neonatal hepatitis in premature infants simulating hereditary tyrosinosis.

Three premature infants are reported who developed obstructive jaundice in the first 3 months of life, with a chemical and histological picture consistent with neonatal hepatitis. The disease was complicated by gross generalized aminoaciduria, rickets, and anaemia. Serum tyrosine levels were raised to 0·28-1·64 μmole/ml in 2 infants, and did not fall in response to ascorbic acid given orally. The infants recovered over the next 3 to 5 months without specific therapy. Many features of the illness in the 3 infants are suggestive of hereditary tyrosinosis, but the transient nature of the illness

Treatment of Portal Biliopathy-Related Bile Duct Stricture Using a Two-Step Approach: Proximal Splenorenal Shunt and Roux-en-Y Hepaticojejunostomy.

Portal biliopathy, also referred to as portal cavernoma cholangiopathy, is a biliary complication of extrahepatic portal vein obstruction caused by cavernomatous transformation of the portal vein and resulting biliary compression. Although biliary abnormalities are common in long)standing portal hypertension, only a minority of patients become symptomatic and require intervention. Herein, we present a 13)year)old patient with long)standing extrahepatic portal vein obstruction who developed pruritus, jaundice, and a distal common bile duct stricture with proximal biliary dilatation secondary to

Heterotopic Gastric Mucosa in a Duplication Cyst of the Common Hepatic Duct Mimicking Cholangiocarcinoma.

Heterotopic gastric mucosa in biliary tract is a congenital anomaly that can prove significant clinical dilemmas. Here we report the case of a 28-year-old female patient presenting with jaundice, pruritus, and altered liver tests, with predominant cholestasis. Liver biopsy revealed histological changes suggesting large bile duct obstruction with advanced fibrosis. At imaging, common hepatic duct stricture due to an intraluminal enhancing mass was observed. Endoscopic retrograde cholangiopancreatography and upper echoendoscopy revealed a firm mass of the common hepatic duct with a complete obst

Trichotillomania +/- trichobezoar: revisited.

Trichotillomania is an intriguing psychosomatic entity in which there is an irresistible desire to pull out the hair from the scalp, eyelashes, eyebrows and other parts of the body. The process results in an instant release of tension, a sense of relief and security. However, non-scaring alopecia is its clinical presentation. The development of trichobezoar following ingestion of the pulled hair is its salient complication in a few cases. Subsequently, it may cause symptoms pertaining to the gastrointestinal tract culminating in intestinal obstruction, perforation, pancreatitis and obstructive

Inhibition of heat shock protein 90 alleviates cholestatic liver injury by decreasing IL-1β and IL-18 expression.

Severe cholestatic liver injury diseases, such as obstructive jaundice and the subsequent acute obstructive cholangitis, are induced by biliary tract occlusion. Heat shock protein 90 (HSP90) inhibitors have been demonstrated to be protective for various organs. The potential of HSP90 inhibitors in the treatment of cholestatic liver injury, however, remains unclear. In the present study, rat models of bile duct ligation (BDL) were established, the HSP90 inhibitor 17-dimethylamino-ethylamino-17-demethoxygeldanamycin (17-DMAG) was administered, and its ability to ameliorate the cholestasis-induce

The "sausage" abscess: abscess of the liagamentum teres hepatis.

An abscess of the ligamentum teres hepatis is a very rare cause of acute abdomen and can present a diagnostic dilemma. A 40-year-old diabetic male presented with obstructive jaundice and cholangitis. An ill-defined, sausage-shaped, tender parasagittal supraumbilical mass was palpable on the right side. Murphy's sign was negative. Laboratory investigations revealed polymorphonuclear leukocytosis (total leukocyte count 19,000 mm-3), elevated alkaline phosphatase (400 IU l-1), conjugated hyperbilirubinaemia (16 mg dl-1) and elevated blood glucose (240 mg dl-1). Early-onset severe voriconazole-induced liver injury with progressive cholestasis in pulmonary aspergillosis: A case report.

BACKGROUND: Voriconazole is the recommended first-line treatment for invasive pulmonary Aspergillosis. However, its nonlinear pharmacokinetics and marked interindividual variability increase the risk of drug-induced liver injury, particularly in older patients with multiple comorbidities. CASE PRESENTATION: A 76-year-old Asian man with chronic obstructive pulmonary disease, type 2 diabetes mellitus, hypertension, polycythemia, and glucocorticoid-induced adrenal insufficiency was admitted with an infective exacerbation of COPD. Sputum culture identified Aspergillus fumigatus, and oral voricona

21 years of evolution of primary hepatic neuroendocrine neoplasm in a patient with primary sclerosing cholangitis: A case report.

INTRODUCTION AND IMPORTANCE: Primary hepatic neuroendocrine neoplasms(PHNEN) are among the rarest primitive neuroendocrine neoplasms. Main prognostic factor is histological. We report an unusual case of a PHNEN with 21 years of evolution in the setting of a primary sclerosing cholangitis(PSC). CASE PRESENTATION: A 40 year old man presented in 2001 with clinical signs of obstructive jaundice. CT-scan and MRI showed a 4 cm large hypervascular proximal hepatic mass evoking hepatocellular carcinoma(HCC) or cholangiocarcinoma. Exploratory laparotomy found an aspect of advanced chronic liver diseas

Infant biliary cirrhosis secondary to a biliary inflammatory myofibroblastic tumor: A case report and review of literature.

BACKGROUND: A biliary inflammatory myofibroblastic tumor (IMT) is a rare type of mesenchymoma that, although it has a broad age spectrum, usually occurs in adults. Diagnosis is difficult because biliary IMTs often exhibit nonspecific clinical symptoms and imaging features, resulting in delayed or inappropriate treatment. Although most IMTs are benign, some show malignant properties such as infiltration, recurrence, and metastasis. CASE SUMMARY: Here, we retrospectively describe a 10-month-old infant who was admitted to our hospital due to stubborn jaundice. The patient responded poorly to rou

Endoscopic papillectomy and KRAS expression in the treatment of adenoma in the major duodenal papilla.

OBJECTIVE: The use of endoscopic papillectomy for resecting adenomas in the major duodenal papilla is increasing. This study focuses on the following three issues: Can endoscopic papillectomy be performed as a safe diagnostic and/or therapeutic procedure in biopsy-verified or suspected ampullary adenoma? Does expression of mutated KRAS in resected adenomatous tissue predict long-term outcome? What other factors may affect long-term outcome and should, therefore, be considered in decision making prior to endoscopic papillectomy? MATERIAL AND METHODS: Thirty-six prospectively collected patients

Surgical treatment of coledochal cyst associated with an aberrant posterior hepatic duct: report of a case and brief literature review.

Choledochal cysts (CCs) are rare congenital cystic or fusiform dilatations of the biliary tree that can involve the extrahepatic and/or intrahepatic biliary tree. We report a case of huge type I CC associated with an aberrant posterior hepatic duct. A 52-year-old man presented with a 3-week history of upper right abdominal pain and jaundice and serologic sign of obstructive jaundice. Ultrasonography (US), magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography were performed with the diagnosis of CC type I according to the classification of Alonso-Lej and

[Massive hemobilia caused by necrotic hemorrhagic cholecystitis. Report of a case].

The authors report a case of massive hematobilia due to hemorrhagic cholecystitis. Hematobilia is a rare pathology which affects the biliary tract and gallbladder. The first authors to describe hematobilia defined it as a hemorrhage of the gastroenteric tract due to the communication of blood vessels with the intra and extra-hepatic biliary tract and in some rare cases to the communication of the branches of the cystic artery within the gallbladder wall. Sandblom, in particular, specified that bleeding must be within the biliary tract and not secondary to an enterobiliary fistula. In 55% of ca

Radiofrequency ablation for cholangiocarcinoma - Safety and effectiveness in clinical practice.

Cholangiocarcinoma comprises a heterogeneous group of biliary tract neoplasms, and represents the second most common neoplasm of the liver (15 %) after hepatocellular carcinoma. Diagnosis is achieved through various imaging techniques and is generally made at an advanced stage, often rendering the tumor unresectable. Bipolar biliary radiofrequency ablation is a palliative treatment that, when applied endoscopically, is a safe and effective technique. It is indicated for inoperable cholangiocarcinomas presenting with jaundice, to recanalize the biliary tract and restore patency of obstructed me
