The predictive value of obstructive sleep apnoea severity on clinical outcomes following maxillomandibular advancement surgery.

We aimed to evaluate whether the severity of preoperative obstructive sleep apnoea (OSA) has potential predictive value for the clinician assessing patients referred for maxillomandibular advancment surgery. We performed a retrospective analysis of consecutive patients who underwent maxillofacial operations for OSA at our institution. We stratified them into 2 groups according to apnoea/hypopnoea index (AHI) scores calculated from a preoperative sleep study: mild-moderate OSA (AHI less than 30) and severe OSA (AHI 30 and above). Both groups were matched for baseline demographic and clinical ch

A modified retromaxillary approach to the infratemporal fossa: three case studies.

The infratemporal fossa (ITF) is an anatomically complex region with multiple neural and vascular structures entering and exiting through foramina in the skull base. The main obstacles to approaching the ITF are the zygomatic arch, the parotid gland, the facial nerve, and the ascending ramus AND condylar head of the mandible. Different surgical approaches to the ITF exist and the best approach should provide optimal visibility, minimal impairment of temporomandibular joint function, and preservation of motor and sensory nerve integrity. This report describes a modified Obwegeser retromaxillary

Computer-assisted condylar reconstruction in bilateral ankylosis of the temporomandibular joint using autogenous coronoid process.

During the past 10 years, more than 20 patients a year have been treated at our centre for bilateral ankylosis of the temporomandibular joint (TMJ). Here we describe the use of computer-assisted three-dimensional surgical planning and its clinical effects in condylar reconstruction for such patients using autogenous coronoid process. Sixteen patients with bilateral bony ankylosis of the TMJ from March 2006 to March 2009 were randomly divided into 2 groups and treated by bilateral osteoarthrectomy and condylar reconstruction by free grafting of autogenous coronoid process with and without three

Surgical treatment and reconstruction for central giant cell granuloma of the jaws: a review of 18 cases.

Central giant cell granuloma (CGCG) is an uncommon benign bony lesion that occurs in the mandible and maxilla. The clinical behaviour of CGCG ranges from a slow-growing asymptomatic swelling to an aggressive lesion that presents pain, local bone destruction, root resorption and tooth displacement. Therapeutic options have varied greatly over the years. Non-surgical treatments with alpha interferon (alpha-IFN), calcitonin and corticosteroids have been described and their benefits may be worthy of consideration. Surgery is considered the traditional treatment and it is still the most accepted on

Functional outcomes of temporomandibular joint reconstruction with vascularized tissue.

OBJECTIVE: To determine how current temporomandibular joint (TMJ) reconstruction methods affect functional outcomes. METHODS: Retrospective review from January 2006 to July 2017 at a single tertiary care center. All patients who underwent mandibulectomy with subsequent reconstruction with vascularized free tissue were included in the study. Condylar segments were reconstructed with vascularized free tissue flap in conjunction with autologous tissue or allograft in the joint space. Preoperative, 3 month, 1 year, and 2 year postoperative records were assessed for trismus, need for tube feeds, a

Does a patient's Mallampati score predict outcome after maxillomandibular advancement for obstructive sleep apnoea?

The Mallampati airway classification has been used to estimate the success of uvulopalatopharyngoplasty in patients with obstructive sleep apnoea (OSA) but its predictive value in maxillomandibular advancement has not been proved. We aimed to explore the association between preoperative Mallampati scores and surgical outcome after bimaxillary advancement for OSA. We retrospectively analysed data on 50 patients who had maxillofacial operations for OSA at our hospital and stratified them into two groups based on Mallampati scores: high (class III/IV) and low (class I/II). We compared pre- and po

Solitary plasmocytoma of the mandible--a combined approach for treatment and reconstruction.

A 44-year-old man presented with a solitary plasmocytoma in the left mandible. After neoadjuvant radiation therapy and chemotherapy, a bone marrow biopsy was taken for cryopreservation. In simulation surgery, tumour resection and primary reconstruction were planned on an individual stereolithographic 3-D skull model. Radical tumour resection and primary reconstruction using a microvascular iliac crest bone graft was performed. In case of extensive osteolysis and/or mandibular asymmetry due to tumourous bone enlargement, preoperative model planning has proved to be a useful tool for primary rec

Three-dimensional courses of zygomaticofacial and zygomaticotemporal canals using micro-computed tomography in Korean.

The zygomatic nerve (ZN), which originates from the maxillary nerve at the pterygopalatine fossa, enters the orbit through the inferior orbital fissure. Within the lateral region of the orbit, the ZN divides into the zygomaticofacial (ZF) and zygomaticotemporal (ZT) nerves. The ZF and ZT nerves then pass on to the face and temporal region through the zygomaticoorbital foramen and enter their own bony canals within the zygomatic bone. However, multiple zygomaticofacial and zygomaticotemporal canals (ZFCs and ZTCs, respectively) can be observed, and their detailed intrabony courses are unknown.

A novel surgical approach to impacted mandibular third molars to reduce the risk of paresthesia: a case series.

PURPOSE: Extraction of impacted mandibular third molars (M3s) may cause temporary or permanent neurosensorial disturbances of the inferior alveolar nerve (IAN). Although the incidence of this complication is low, a great range of variability has been reported in the literature. Several methods to reduce or eliminate this complication have been proposed, such as orthodontic-assisted extraction, extraction of the second molar, or intentional odontoectomy. The purpose of this series of cases is to present a novel approach for a riskless extraction of impacted mandibular M3s in contact with the IA

Three-dimensional analysis system for orthognathic surgery patients with jaw deformities.

INTRODUCTION: Traditionally, lateral and frontal cephalograms are used with facial photographs to evaluate a patient's maxillofacial skeletal and facial soft-tissue morphology. However, the enlargement and distortion of 2-dimensional radiography made it difficult to accurately conceptualize the patient's anatomy. The purpose of this article was to introduce a new method for comparing 3-dimensional (3D) standard values of the maxillofacial skeletal and facial soft-tissue morphology before and after orthognathic surgery. METHODS: Normative 3D standard values of the maxillofacial skeletal and fa

Worsening of pre-existing TMJ dysfunction following sagittal split osteotomy: a study of three cases.

When patients seeking treatment for malocclusion also suffer from temporomandibular joint (TMJ) disorders, it is hard to predict the result of simultaneous treatment of both conditions, or to plan for its different goals, because of unpredictable changes in the relationship between the disk, the fossa and the condylar head. Prediction is harder in cases of presurgical TMJ hypomobility, especially those with adhesion in the upper TMJ compartment. Authors differ widely on the likely effect of orthognathic surgery on TMJ disorders. This paper reports three cases in which TMJ disorders worsened af

Global research trends and thematic evolution in robotic craniofacial and facial reconstruction and its enabling digital technologies: a bibliometric and visualisation study.

Robotic craniofacial and facial reconstruction is an emerging interdisciplinary field that increasingly intersects with navigation-guided surgery, computer-assisted planning, virtual surgical planning, CAD/CAM workflows, image-guided surgery, augmented reality, and other enabling digital technologies. However, the global research landscape and thematic evolution of this field remain insufficiently characterised. This study aimed to map publication activity, collaboration patterns, citation structures, and research themes in robotic craniofacial and facial reconstruction and its enabling digita

Case Report: transcavernous sinus resection of parotid carcinoma with perineural invasion to Meckel's cave and cavernous sinus.

BACKGROUND: Adenoid cystic carcinoma (ACC) of the salivary glands is a rare but aggressive malignancy known for perineural invasion (PNI), allowing tumor spread along cranial nerves to the skull base, Meckel's cave, and cavernous sinus. Cavernous sinus involvement has traditionally been considered inoperable due to the density of neurovascular structures. Recent advancements in skull base microsurgery and anatomical landmark-guided navigation have enabled radical tumor resection in carefully selected patients. OBJECTIVE: To describe a tailored pretemporal extradural transcavernous skull base

Localized deferoxamine injection augments vascularity and improves bony union in pathologic fracture healing after radiotherapy.

BACKGROUND: Medically based efforts and alternative treatment strategies to prevent or remediate the corrosive effects of radiotherapy on pathologic fracture healing have failed to produce clear and convincing evidence of success. Establishing an effective pharmacologic option to prevent or treat the development of non-unions in this setting could have immense therapeutic potential. Experimental studies have shown that deferoxamine (DFO), an iron-chelating agent, bolsters vascularity and subsequently enhances normal fracture healing when injected locally into a fracture callus in long bone ani

[Pathogenic situations after orthodontic treatment. Defects in skeleton and soft tissues that should be avoided after orthodontic treatment].

The perfect correction of dental alignment anomalies found in dento facial dysmorphosis should not be the only criterium of success in dento facial orthopedics. One must insure, lest severe T.M.J. disorders and sleep apnea problems subsequently occur, that : (1) the condyles and mandibular vertical branches are correctly positioned relative to the basilar apophysis and cervical vertebrae, (2) the hyoid bone must not descend too low relative to the C3-C4 intervertebral disk.
