Risk factors for nonunion of bone fracture in pediatric patients: An inception cohort study of 237,033 fractures.

UNLABELLED: Adult fracture nonunion risk is related to injury severity and surgical technique, yet nonunion is not fully explained by these risk factors alone; biological risk factors are also important. We test a hypothesis that risk factors associated with pediatric fracture nonunion are similar to adult nonunion risk factors.Inception cohort study in a large payer database of pediatric fracture patients (0-17 years) in the United States in calendar year 2011. Continuous enrollment in the database was required for 12 months, to allow time to capture a nonunion diagnosis. The final database c

Diagnostic Use of Four-Dimensional Computed Tomography in Scapholunate Ligament Tear Detection.

Background: Wrist arthroscopy is widely used to assess the intactness of ligaments, including the scapholunate ligament (SLL), as it allows direct visualisation and palpation. However, arthroscopy is invasive and costly, while traditional imaging modalities, such as X-rays and MRI, often miss ligament injuries due to their static nature. A promising alternative is four-dimensional computed tomography (4DCT), which allows dynamic, non-invasive visualisation of wrist motion in real time. This study evaluates the diagnostic potential of 4DCT for detecting SLL tears by comparing its findings with

Does scaphoid fracture surface morphology correlate with fracture stability?

We examined the correlations between the surface morphology of acute scaphoid fractures and their stability. Our hypothesis was that a more irregular fracture surface would have a stabilizing effect on the fracture. A total of 75 isolated acute scaphoid fractures of adults were analysed according to location and displacement (as a sign of fracture instability) using a 3-D computed configuration space graph method. Displaced waist fractures had more irregularity than non-displaced fractures. The distal and proximal fractures exhibited a different pattern of displacement in comparison with the w

Stage-Based Surgical Strategies and Outcomes for Preiser Disease: A Single-Center Experience.

BACKGROUND: The optimal surgical approach for Preiser disease, a rare avascular necrosis of the scaphoid, remains unclear. Our institution employs a stage-based strategy: vascularized bone grafting (VBG) or closing radial wedge osteotomy (CRWO) for early disease (Herbert classification stages 1-2) and proximal row carpectomy (PRC) for advanced disease (stages 3-4). This study evaluates this stage-based surgical approach. METHODS: We retrospectively analyzed 8 patients (mean age: 60.6 years) who underwent surgical treatment for Preiser disease between 2000 and 2019. Preoperative evaluation inc

Recommendations Regarding Suspected Scaphoid Fractures Are Associated With Patient and Surgeon Comfort With Uncertainty.

AIMS: In a scenario-based survey experiment, we evaluated the contributions of patient and surgeon tolerance for uncertainty to variation in management strategies for suspected scaphoid fractures. We asked the following: (1) Are patient and surgeon factors, including comfort with uncertainty, associated with the choice of management strategy for a suspected scaphoid fracture? and (2) What patient and surgeon factors are associated with comfort letting the patient decide how to manage their suspected scaphoid fracture? METHODS: Surgeon members of the Science of Variation Group completed the sh

In vivo non-contact regions of proximal scaphoid in six extreme wrist positions.

INTRODUCTION: Fractures of the scaphoid are the most common carpal injuries, account for 80-90% of all carpal fractures. 5-15% nonunion of scaphoid fractures were reported even with adequate primary treatment, which probably progresses to osteoarthritic changes several decades later. Researches regarding to scaphoid physiological characteristic in vitro and in vivo and kinds of trials in clinical practice are being kept on going, which contribute much to our clinical practice. With the advancing wrist arthroscopy, 3D-print patient-specific drill guide, and intraoperative fluoroscopic guidance,

Management of scaphoid fractures with CT scanning and virtual fracture clinic pathway reduces need for face-to-face clinic appointments.

INTRODUCTION: Early diagnosis is key to managing scaphoid fractures effectively. Computed tomography (CT) imaging can be effective if plain radiographs are negative. With increasing pressure on face-to-face clinics, consultant-led virtual fracture clinics (VFCs) are becoming increasingly popular. This study evaluates the management of patients with suspected scaphoid fractures using a standardised treatment protocol involving CT imaging and VFC evaluation. METHODS: The study was conducted at a busy district general hospital. The pathway began in February 2018. Patients presenting to the emerg

Scaphoid fractures: has magnetic resonance imaging become a cost-effective alternative to definitive diagnosis?

BACKGROUND: A clinical examination finding of anatomical snuffbox tenderness (AST) with plain film radiographs alone yields low sensitivity and specificity in diagnosing scaphoid fractures. International guidelines suggest immediate referral for magnetic resonance imaging (MRI) following one non-diagnostic radiograph in such patients. Perceived cost, high demand and limited capacity of MRI scanners have resulted in few suspected fractures following this pathway in our institution. AIMS: Our study aimed to audit cost-effectiveness of immediate MRI referral following one non-diagnostic radiogra

6-week radiographs unsuitable for diagnosis of suspected scaphoid fractures.

INTRODUCTION: Six week follow-up radiographs are a common reference standard for the diagnosis of suspected scaphoid fractures. The main purpose of this study was to evaluate the interobserver reliability and diagnostic performance characteristics of 6-weeks radiographs for the detection of scaphoid fractures. In addition, two online techniques for evaluating radiographs were compared. MATERIALS AND METHODS: A total of 81 orthopedic surgeons affiliated with the Science of Variation Group assessed initial and 6-week scaphoid-specific radiographs of a consecutive series of 34 patients with susp

Biomechanical comparison of 3 methods of scapholunate ligament reconstruction.

PURPOSE: To compare the biomechanical characteristics of 3 methods of scapholunate (SL) ligament reconstruction, including 1 that provides a biological central axis tether. METHODS: Twelve fresh-frozen cadaver limbs were mounted on a jig that allowed for wrist and finger motion by tendon loading. The specimens were randomized to receive the SL axis method (SLAM) reconstruction, the Blatt capsulodesis (BC), or the modified Brunelli tenodesis (MBT). Fluoroscopic images were taken to measure the SL interval and SL angle in various positions. The specimens were evaluated in 4 states: intact, with

The use of economic evaluation in hand surgery.

This article explains the concepts of an economic evaluation relevant to evidence-based hand surgery. Cost-effectiveness analysis in hand surgery is increasingly important as health care resources become scarce in most jurisdictions. Hand surgeons need to incorporate the "manager of health care system" competency in their daily practice. Hand surgery literature may claim that "a novel hand technique" is more cost-effective than a prevailing one; it is important that hand surgeons and other users of clinical research appraise such innovation claims before adopting them in their practices. Clini

Scaphoid fractures: dorsal versus volar approach.

Twenty-six patients with scaphoid fractures were treated with internal fixation using a cannulated differential pitch compression screw. Sixteen patients underwent a dorsal approach (group 1) 10 patients a volar approach (group 2). Average time from injury to surgery was 6.6 months (range: 0.3-19 months) for group 1 and 8.3 months (range: 0.3-24 months) for group 2. The rate of union, determined by radiographs and clinical examination, was 81% in group 1 and 80% in group 2. No significant differences were noted between the groups for dorsiflexion/palmar flexion, radial deviation, grip strength

[Midcarpal arthrodesis with cortical bolting chip for treatment of grade II/III scaphoid non-union and scapholunate advanced collapse].

INTRODUCTION: These case reports describe a mediocarpal arthrodesis with excision of the scaphoid using a cortical bolting chip and screw fixation instead of the use of Kirschner wires or Spider plates. METHODS: Four patients with a symptomatic SLAC/SLAC-wrist >or= grade II were treated. The evaluation occurred six months later. For the evaluation the Cooney and Bussey as well as DASH scores were used (pain, strength, range of motion, functional status, possible limitations of quality of life, subjective perception of the patient). The radiological evaluation was performed by conventional wri

The impact of lunate type on scapho-trapezio-trapezoid arthritis in trapeziometacarpal osteoarthritis patients.

BACKGROUND: Type II lunate has an extra facet with the hamate. This difference alters the biomechanics of the midcarpal joint, influencing the development of scapho-trapezio-trapezoid (STT) arthritis. We aimed to investigate whether, in patients with trapeziometacarpal (TMC) osteoarthritis (OA), involvement of the STT joint is associated with the lunate type. METHODS: A retrospective review was performed on 121 patients (mean age of 69 years) diagnosed with TMC OA. The lunate type was classified based on the presence of the facet detected on computed tomography images. The incidence of STT OA

Traumatic axial dislocation of the carpus: a case report of transscaphoid pericapitate transhamate axial dislocation.

Traumatic axial dislocation of the carpus in a 20-year-old man is described. This injury was accompanied by a crushing injury to the hand. The disruption pattern was different from those of previously reported cases. Despite the restoration of painless wrist motion postoperatively, grip strength remained below normal. Early accurate reduction, fixation, and range of motion (ROM) exercise are the treatment of choice in such complex injuries.
