Morphological and Morphometric Analysis of the Pterion and Its Neurosurgical Implications.

Background The pterion is an important craniometrical landmark located on the lateral aspect of the skull, formed by the articulation of the frontal, parietal, squamous temporal, and greater wing of the sphenoid bones. Owing to its close relationship with vital neurovascular structures, such as the anterior division of the middle meningeal artery and the Sylvian fissure, precise knowledge of its morphology and location is essential for neurosurgical interventions, anthropological assessment, and forensic evaluation. This study aimed to investigate the morphological types of pterion and to dete…

[Distant metastases to nasal cavities and paranasal sinuses, from the organs outside the head and neck].

INTRODUCTION: The metastases from below the clavicles, to the head and neck region, are relatively uncommon, and usually demonstrate during the progression of the primary disease. At that time, the correct diagnosis requires only to compare the pathology report from the primary biopsy, with the biopsy from the lump in the head and neck. The majority of distant metastases to the head and neck region are localized within the lymph nodes. The metastases to nasal cavity and paranasal sinuses are very rare and usually localized within the maxillary sinus. MATERIAL: 4 cases, out of 46 all distant m

Outcome Squares Integrating Efficacy and Safety, as Applied to Functioning Pituitary Adenoma Surgery.

CONTEXT: Transsphenoidal surgery is standard care in the treatment of hormone-secreting pituitary adenomas. Current clinician-reported surgical outcome measures are one-dimensional, typically focusing primarily on complete or partial resection, and secondarily on complication rates. However, outcomes are best reflected by the delicate balance of efficacy and complications at patient level. OBJECTIVE: This study proposes a novel way to classify and report outcomes, integrating efficacy and safety at the patient level. METHODS: Retrospective chart review of all pure endoscopic transsphenoidal

Skull base CT: normative values for size and symmetry of the facial nerve canal, foramen ovale, pterygoid canal, and foramen rotundum.

PURPOSE: Contrast-enhanced MRI is the mainstay for detecting pathology in the skull base foramina and nerve canals, through demonstration of abnormal enhancement. When MRI is contraindicated, or unable to differentiate tumor from non-neoplastic pathology, high-resolution skull base CT is indicated to assess for nerve canal or foramen widening, which is currently determined subjectively. The purpose of this study is to provide objective CT criteria that may help distinguish between normal asymmetry and pathologic nerve canal or foramen widening. METHODS: Temporal bone CTs of 50 consecutive adu

Extended transsphenoidal approach for pituitary adenomas invading the anterior cranial base, cavernous sinus, and clivus: a single-center experience with 126 consecutive cases.

OBJECT: The standard transsphenoidal approach has been successfully used to resect most pituitary adenomas. However, as a result of the limited exposure provided by this procedure, complete surgical removal of pituitary adenomas with parasellar or retrosellar extension remains problematic. By additional bone removal of the cranial base, the extended transsphenoidal approach provides better exposure to the parasellar and clival region compared with the standard approach. The authors describe their surgical experience with the extended transsphenoidal approach to remove pituitary adenomas invadi

Provocation testing to assist craniomandibular pain diagnosis.

Patients with TMD often present with complex pain symptoms, which can make it difficult to reach a diagnosis. Usually palpation of the masticatory muscles and TM joints, range of motion testing and imaging are used in the diagnostic process. Sometimes it is necessary to evaluate the jaw moving muscles from a functional prospective because they cannot be palpated due to inaccessibility or because they have other structures that are more superficial to them. In these instances, provocation testing can be a helpful adjunct in providing some insight into what is occurring in the area being tested

Endoscopic transnasal resection of ectopic esthesioneuroblastoma in the pterygopalatine fossa: technical case report.

OBJECTIVE: Esthesioneuroblastoma is a rare, malignant neoplasm arising from the olfactory neuroepithelium in the upper nasal cavity. Even more rare is ectopic esthesioneuroblastoma developing from the region outside the olfactory epithelium. In addition, tumors occurring in the pterygopalatine fossa (PPF) are uncommon, and the endoscopic transnasal approach for the resection of malignant tumors in this region is also uncommon. CLINICAL PRESENTATION: We describe an esthesioneuroblastoma arising from the left maxillary sinus and PPF. The tumor was resected using the endoscopic transnasal approa

Traumatic chiasmal syndrome: a feature photograph.

Traumatic chiasmal neuropathy results from injury to the face, sphenoid and clivus. Its pathogenesis remains enigmatic. Because of its close relationship to the pituitary gland, hypothalamus and internal carotid artery, a neuro-ophthalmic evaluation and imaging is needed in such cases. We present a patient who developed traumatic chiasmal syndrome after an automobile accident. Computed tomographic scan showed fracture of the sella turcica. A carotid angiography showed a traumatic pseudoaneurysm of the internal carotid at the base of the skull.

Impact of extent of resection and adjuvant radiation therapy in the progression free survival in patients with spheno-orbital meningioma.

BACKGROUND: Spheno-orbital meningiomas (SOM) are known to invaded critical skull base areas. The authors report a series of WHO I SOM, propose a subclassification of this tumor according to its extension to critical positions and analyze the impact of extent of resection and the role of stereotactic radiotherapy in tumor recurrence. METHODS: A prospective maintained university medical center registry was utilized to undertake a retrospective review of patients operated with WHO I SOM. Details related to critical skull base region's extension (superior orbital fissure, cavernous sinus, orbital

Disruption of Rich-Club Connectivity in Cushing Disease.

OBJECTIVE: Cushing disease (CD) is a rare clinical disease in which brain structural and function are impaired as the result of excessive cortisol. However, little is known whether rich-club organization changes in patients with CD, as visualized on resting-state magnetic resonance imaging (fMRI), can reverse to normal conditions after transsphenoidal surgery (TSS). In this study, we aimed to investigate whether the functional connectivity of rich-club organization is affected and whether any abnormal changes may reverse after TSS. METHODS: In this study, 38 patients with active CD, 33 with p

Prognostic factors for visual recovery after transsphenoidal pituitary adenectomy.

OBJECTIVES: Pituitary adenomas often cause deficits in visual acuity (VA) and visual field (VF) due to compression of the optic chiasm. This study is to identify factors most likely to predict the prognosis of VA and VF after transsphenoidal pituitary adenectomy. MATERIALS AND METHODS: Clinical records of patients who underwent transsphenoidal pituitary adenectomy were retrospectively reviewed. Data analysed included systemic and visual symptom duration, tumour size, presence of suprasellar tumour extension, histological classification of tumour, VA, and VF. VFs were determined using the Gold

Endoscopic transsphenoidal surgery for cushing disease: techniques, outcomes, and predictors of remission.

BACKGROUND: The efficacy of endoscopic transsphenoidal surgery (ETS) for Cushing disease has not been clearly established. OBJECTIVE: To assess efficacy of a pure endoscopic approach for treatment of Cushing disease and determine predictors of remission. METHODS: A prospectively acquired database of 61 patients undergoing ETS was reviewed. Remission was defined as postoperative morning serum cortisol of <5 μg/dL or normal or decreased 24-hour urine-free cortisol level in follow-up. RESULTS: Overall, hypercortisolemia resolved in 58 of 61 patients (95%) by discharge. Tumor size did not predi

The extended direct endonasal transsphenoidal approach for nonadenomatous suprasellar tumors.

OBJECT: The extended transsphenoidal approach, which requires a bone and dural opening through the tuberculum sellae and posterior planum sphenoidale, is increasingly used for the treatment of nonadenomatous suprasellar tumors. The authors present their experiences in using the direct endonasal approach in patients with nonadenomatous suprasellar tumors. METHODS: Surgery was performed with the aid of an operating microscope and angled endoscopes were used to assess the completeness of resection. Bone and dural defects were repaired using abdominal fat, collagen sponge, titanium mesh, and, in

Large sphenocavernous meningiomas: is there still a role for the intradural approach via the pterional-transsylvian route?

BACKGROUND: Large-sized sphenocavernous meningiomas represent a surgical challenge. Although the role of skull base techniques with combined extra- and intradural steps has been recently emphasized, pure intradural resection tactics via the pterional route constitute the traditional microsurgical approach for resection of such tumours. METHOD: We report the application of the pterional-transsylvian approach in 13 patients with sphenocavernous meningiomas. This series is unique because it includes only patients with tumours exceeding 5 cm in their greatest dimension. FINDINGS: A gross total r
