Identification of side of seizure onset in temporal lobe epilepsy using memory tests in the context of reading deficits.

Sixty patients with temporal lobe epilepsy were classified into reading deficient (RD; n = 21) and non-reading deficient (non-RD; n = 39) groups. Selective deficits in verbal or nonverbal memory, consistent with side of seizure onset, were evident in the non-RD patients. Both verbal and nonverbal memory performance were reduced equivalently in individuals with RD, regardless of side of seizure onset. As a result, memory tests that were accurate in identifying side of seizure onset in the non-RD group were not as accurate in the RD group. When individual cases were classified using a clinically…

Geophysical variables and behavior: LV. Predicting the details of visitor experiences and the personality of experients: the temporal lobe factor.

The visitor experience, a more intense form of the normal sense of presence, emphasizes the deep belief of personal contact with an extraterrestrial (or religious) entity. Phenomenological details of visitor experiences are expected to reflect the functions of deep temporal lobe structures; common details involve cosmic meaningfulness, vestibular experiences, flickering, complex visual sensations and alimentary references. After intense experiences, interictal-like behaviors similar to religious conversions (widening affect, sense of personal, desire to spread the word, concern about Man's des…

The ATLAS/NOA-29 study protocol: a phase III randomized controlled trial of anterior temporal lobectomy versus gross-total resection in newly-diagnosed temporal lobe glioblastoma.

BACKGROUND: The discovery of cellular tumor networks in glioblastoma, with routes of malignant communication extending far beyond the detectable tumor margins, has highlighted the potential of supramarginal resection strategies. Retrospective data suggest that these approaches may improve long-term disease control. However, their application is limited by the proximity of critical brain regions and vasculature, posing challenges for validation in randomized trials. Anterior temporal lobectomy (ATL) is a standardized surgical procedure commonly performed in patients with pharmacoresistant tempo…

Effect and mechanism of lateralization of cerebellar-cerebral network abnormalities on language in patients with unilateral temporal lobe epilepsy.

OBJECTIVE: Since epilepsy is associated with cerebral dysfunction, there may be a recombination of language circuits in the course of epilepsy. We explored the differences in speech impairment patterns in patients with temporal lobe epilepsy (TLE) and different hemiencephalic attacks from the perspective of the cerebellum. METHODS: Fifty-eight patients with TLE and 30 healthy controls (HCs) were recruited. including 28 patients with left temporal lobe epilepsy (LTLE) and 30 patients with right temporal lobe epilepsy (RTLE). The resting state functional connection method was used to investigat…

Unveiling Molecular Dynamics of MeCp2, CDKL5 and BDNF in the Hippocampus of Individuals With Intractable Mesial Temporal Lobe Epilepsy.

Mutations occurring in the MeCp2, CDKL5 and BDNF genes have been linked to epileptogenesis in various epilepsy syndromes. This study employed bioinformatics analysis of transcriptomic data to examine the interrelationship among these genes in both epileptic and healthy individuals. Moreover, we assessed the expression of MeCp2, CDKL5 and BDNF at both mRNA and protein levels in human hippocampal tissues obtained from 22 patients undergoing epilepsy surgery for mesial temporal lobe epilepsy (MTLE) as well as from 25 autopsied specimens. Bioinformatics findings suggest that MeCp2, CDKL5 and BDNF …

The Metabolic Profile of Plasma During Epileptogenesis in a Rat Model of Lithium-Pilocarpine-Induced Temporal Lobe Epilepsy.

Temporal lobe epilepsy (TLE) arises mostly because of an initial injury. Certain stimuli can make a normal brain prone to repeated, spontaneous seizures via a process called epileptogenesis. This study examined the plasma metabolomics profile in rats with the induced TLE to identify feasible biomarkers that can distinguish progression of epileptogenesis in three different time points and reveal the underlying mechanisms of epileptogenesis. Status epilepticus (SE) was induced by repetitive intraperitoneal injections of low-dose lithium chloride-pilocarpine hydrocholoride. Blood samples were col…

Activation of the Reelin/GSK-3β/p-Tau Signaling Pathway in the Hippocampus of Patients with Temporal Lobe Epilepsy.

PURPOSE: In this study, we evaluated the presence of tau deposition and protein expression of Reelin/GSK-3β/p-Tau signaling pathway in the hippocampus of patients with temporal lobe epilepsy (TLE). METHODS: A total of 37 cases of TLE with and without hippocampal sclerosis (HS) were selected histopathologically for our study with 5 autopsy cases as the control group. Immunohistochemistry and the histelide assay, a novel technique quantifying antigens in paraffin section, were used to confirm the distribution of protein within Reelin/GSK-3β/p-Tau signaling pathway and validate the expression of…

Tau Pathology in the Medial Temporal Lobe and Neocortex: Implications for Cognitive Unimpaired in Cognitively Unimpaired Older Adults.

BACKGROUND: The accumulation of proteins such as amyloid-beta and tau, which disrupt normal cellular processes, characterizes Alzheimer's disease (AD). Cognitive decline is strongly linked to tau pathology, which initially manifests in the medial temporal lobe (MTL). AIMS: To investigated the association between cognitive performance and regional tau accumulation, as measured by positron emission tomography (PET) imaging, in cognitively normal older adults. Understanding this relationship is critical for early intervention before noticeable cognitive decline emerges. STUDY DESIGN: Retrospect…

[3H]T-401 binding to monoacylglycerol lipase (MAGL) in the brains of patients and mice with temporal lobe epilepsy.

Monoacylglycerol lipase (MAGL) inhibitors are considered as drug candidates for epilepsy. In order to determine the level of MAGL and evaluate changes in the epileptic brain, we have validated and used autoradiography and the MAGL radiotracer [3H]T-401 on resected temporal neocortex specimens obtained from patients with temporal lobe epilepsy and in brains from mice with chronic reoccurring hippocampal seizures. Saturation experiments revealed a KD around 4 nM for the human temporal cortex and 7 nM for the mouse brain. In the human brain, binding of [3H]T-401 was detected mostly in the grey ma…

Association of Medial Temporal Lobe Cerebrovascular Reactivity and Memory Function in Older Adults With and Without Cognitive Impairment.

BACKGROUND AND OBJECTIVES: Cerebrovascular reactivity (CVR) represents the ability of cerebral blood vessels to regulate blood flow in response to vasoactive stimuli and is related to cognition in cerebrovascular and neurodegenerative conditions. However, few studies have examined CVR in the medial temporal lobe, known to be affected early in Alzheimer disease and to influence memory function. We aimed to examine whether medial temporal CVR is associated with memory function in older adults with and without mild cognitive impairment (MCI). METHODS: In this observational study, independently l…

Allostatic load of immune cells in the spleen and brain of the Kcna1-null mouse model of temporal lobe epilepsy.

Human temporal lobe epilepsy (TLE), particularly drug-resistant TLE, is associated with chronic peripheral immune activation. Here, we determined whether a similar association was detectable in a genetic mouse model of TLE with spontaneous recurrent seizures (SRS), Kcna1-null mice. Flow cytometry was used with fluorescence-activated cell sorting to determine the presence of lymphocytes, macrophages and granulocytes in isolated brain and spleen of wildtype (WT) and Kcna1-null mice. Splenic analysis revealed uniformly elevated Mac-1+MHC-II+ macrophages across all epileptic mice, whereas CD8+ cyt…

Impaired degree centrality and effective connectivity contributed to deficits in cognition and depression in patients with temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) leads to severe neuropsychiatric symptoms and cognitive impairment; however, the underlying mechanism is still not fully understood. Using degree centrality (DC), effective connectivity (EC), and multivariate pattern analysis (MVPA), we aimed to reveal alterations in brain networks and investigate neurofunctional symptoms in TLE patients. A total of fifty TLE patients and forty-seven healthy controls (HCs) were enrolled and underwent resting-state functional magnetic resonance imaging (rsfMRI) and neuropsychological testing. Cerebral functional activity and neuroim…

Exploring the role of epileptic focus lateralization on facial emotion recognition in the spectrum of mesial temporal lobe epilepsy.

INTRODUCTION: Evidence increasingly shows that facial emotion recognition (FER) is impaired in refractory mesial temporal lobe epilepsy (rMTLE), especially in patients with a right focus. This study explores FER in both mild (mMTLE) and refractory forms, examining the influence of epileptic focus lateralization on FER. METHODS: 50 MTLE patients, categorized by epilepsy severity and focus lateralization, were compared with healthy controls. FER was assessed using the Ekman Faces Test (EFT), which evaluates recognition of six basic emotions, alongside a battery of cognitive and mood tests. RES…

Verbal semantic expertise is associated with reduced functional connectivity between left and right anterior temporal lobes.

The left and right anterior temporal lobes (ATLs) encode semantic representations. They show graded hemispheric specialization in function, with the left ATL contributing preferentially to verbal semantic processing. We investigated the cognitive correlates of this organization, using resting-state functional connectivity as a measure of functional segregation between ATLs. We analyzed two independent resting-state fMRI datasets (n = 86 and n = 642) in which participants' verbal semantic expertise was measured using vocabulary tests. In both datasets, people with more advanced verbal semantic …

Associations of Cerebral Blood Flow Patterns With Gray and White Matter Structure in Patients With Temporal Lobe Epilepsy.

BACKGROUND AND OBJECTIVES: Neuroimaging studies in patients with temporal lobe epilepsy (TLE) show widespread brain network alterations beyond the mesiotemporal lobe. Despite the critical role of the cerebrovascular system in maintaining whole-brain structure and function, changes in cerebral blood flow (CBF) remain incompletely understood in the disease. Here, we studied whole-brain perfusion and vascular network alterations in TLE and assessed its associations with gray and white matter compromises and various clinical variables. METHODS: We included individuals with and without pharmaco-re…
