[Reconstructive surgery of the upper limbs in the rehabilitation of tetraplegia--case report].

As part of the rehabilitation of tetraplegic patients, movement improving operations have been carried out for more than 30 years. The scant results of the early 70's caused such a standstill and division among professionals, that operations on the upper limbs on tetraplegic patients became questionable. The authors started operating upper limb reconstructions on tetraplegia patients for achieving a basic hand function i.e. key pinch, grasping etc. in 2001. Three cases are quoted and one of them, operated ten months ago is described in details. The postoperative treatment of the other two pati

[Anatomic reconstruction of the lateral ligaments of the ankle using a plantaris tendon graft in the treatment of chronic ankle joint instability].

The purpose of this work was to present our technique of anatomic reconstruction of the lateral ankle ligaments using a free plantaris tendon graft. Between 1988 and 1997, 52 ankles (48 patients) were treated for chronic ankle instability resisting to a training program of minimally 3 months. The average age was 28.6 years (16 to 46 years) at the time of surgery. There were 30 ankles in men and 22 ankles in females. 4 patients were treated on both ankles. 50 ankles were available for a follow-up investigation after a mean of 3.5 years (1 to 10 years). Based on the AOFAS-Hindfoot-Scale an avera

Zanoli's procedure for overlapping fifth toe: retrospective study of 18 cases followed for 4-17 years.

Digitus quintus superductus pedis (overlapping fifth toe) is a congenital deformity, often bilateral, in which the fifth toe is medially directed, rotated and hyperextended. There is no consensus regarding the best treatment. We describe our experience using Zanoli's procedure, which is a tenodesis procedure using the extensor tendon of the fifth toe. Between 1979 and 1993, we operated on 16 patients (7 bilateral, total 23 feet). 13 (10 women) patients (18 feet), with a mean age of 26 (19-40) years, were seen on follow-up. The mean follow-up time was 8 (4-17) years. Pain relief was achieved in

[Surgical treatment of habitual patella dislocations by combined soft tissue surgery].

Since January 1988 21 operations using a modified Madigan and Goldthwait method have been carried out on 18 patients suffering from recurrent dislocation of the patella. This soft tissue procedure is presented here in its individual operational steps. The average age of the patients was 22 years and the majority were female (6:1). The mean follow-up time was 19.3 months. One patient who had both knees operated on reported recurrent luxation 6 months after the operation in one knee and subluxation in the other. None of the other patients suffered further dislocations. In the opinion of the pati

The postoperative knee.

Magnetic resonance (MR) has established itself as the gold standard for noninvasive imaging of the knee. As in arthroscopy, assessment of injuries to the menisci, articular surfaces, synovium, and cruciate ligaments can be made. Additionally, MR has an advantage over arthroscopy in being able to assess the para articular soft tissues and cortical and medullary bony compartments. Until recently, the role of MR in the postoperative knee has been less clear. New information, however, suggests that MR is capable of assessing the status of the partially resected meniscus, the adjacent articular sur

Modification of the L'Episcopo procedure for brachial plexus birth palsies.

We reviewed 19 children who had undergone a new modification of the L'Episcopo procedure for obstetric brachial plexus palsy. Through an axillary approach the latissimus dorsi tendon was re-routed anteriorly to the humerus and then anastomosed to the teres major tendon routed posteriorly. At an average follow-up of four years two months, the mean increase in shoulder abduction was 26 degrees and the mean increase in external rotation was 29 degrees. No neurovascular injury or postoperative infection occurred. Two patients had complications, and five did not gain from the procedure. The modifie

Sever-L'Episcopo transfers in obstetrical palsy: a retrospective review of twenty cases.

Since 1963, 25 patients have undergone Sever-L'Episcopo transfers for obstetrical birth palsy at the St. Louis Unit of the Shriners Hospital for Crippled Children. A follow-up of 2-6 years was possible with a retrospective review of 16 patients. Substantial improvement in shoulder external rotation as well as subjective functional improvement was obtained by all patients. Three transient and one permanent axillary nerve palsies resulting from this procedure are reported.

Foot surgery in Charcot-Marie-Tooth disease.

The preliminary study dealing with surgical treatment of foot deformity associated with CMT shows that the peroneal muscle may be functional in CMT but not measurable because of the deformity that it may have helped to create. The transfer of peroneus longus is designed to decrease deforming forces in conjunction with the Jones suspension that elevates the first ray. It assists the weak anterior muscle group. This, in addition to other surgical procedures as referenced, may be of help in correcting the foot deformity of CMT. Prognosis can be affected by the progression of the disease.

High Median Nerve Reconstruction with Vascularized Ulnar Nerve and Extensor Carpi Radialis Brevis Motor Branch to Anterior Interosseous Nerve Transfers.

Combined high median and ulnar nerve injuries present significant challenges due to the complexity of restoring sensory and motor functions with limited donor sites for tendon or nerve transfers. Traditional techniques such as cable grafting often fail to achieve satisfactory functional recovery, particularly in cases involving proximal injuries and large nerve gaps. We report a case of a 21-year-old man who sustained a gunshot wound to the right medial upper arm, injuring the median and ulnar nerves. Three months postinjury, the patient exhibited upper extremity weakness, with 0 of 5 finger a

Finite element simulation on posterior tibial tendinopathy: Load transfer alteration and implications to the onset of pes planus.

BACKGROUND: Posterior tibial tendinopathy is a challenging foot condition resulting in pes planus, which is difficult to diagnose in the early stage. Prior to the deformity, abnormal internal load transfer and soft tissue attenuation are anticipated. The objective of this study was to investigate the internal load transfer and strain of the ligaments with posterior tibial tendinopathy, and the implications to pes planus and other deformities. METHODS: A three-dimensional finite element model of the foot and ankle was reconstructed from magnetic resonance images of a 28-year-old normal female.

[Pattern of paralysis and reconstructive operations after traumatic brachial plexus lesions].

The aim of this study was to evaluate persistent patterns of paralysis after traumatic brachial plexus lesions. As a result, consecutive reconstructive operations according to our differential therapy concept are presented. Between 04/1994 and 12/2000 in 104 patients with brachial plexus palsy, the grade of muscle power of the affected upper extremities was evaluated prospectively. The neuromuscular patterns of defect showed, in most cases, insufficient muscle power grades of 0-2 for the deltoid muscle (90%), supraspinatus muscle (82%), infraspinatus muscle (93%), elbow flexors (67% to 77%), h

[Expert consensus on reconstruction treatment for irreparable posterosuperior massive rotator cuff tears].

Irreparable posterosuperior massive rotator cuff tears represent a common and challenging condition in shoulder surgery. Reconstruction procedures can effectively restore the anatomical structure and function of the shoulder joint and, as an important joint-preserving treatment option for irreparable posterosuperior massive rotator cuff tears, have garnered increasing attention and application among shoulder surgeons. However, a lack of consensus currently exists regarding its definition, diagnostic criteria, and treatment strategies, leading to inconveniences in clinical decision-making and a

Evaluation of foot biomechanics following surgical interventions, including SPOTT and SPLATT procedures in the management of equinovarus deformity in cerebral palsy hemiplegia.

BACKGROUND: Cerebral Palsy (CP) is a group of disorders affecting movement and posture and is the most common physical disability in children. Surgical interventions, such as split anterior and posterior tibial tendon transfers (SPLATT and SPOTT), can be necessary to correct foot and ankle deformities in CP. This is a retrospective pre-/post-operative cohort study that aims to specifically investigate effects on lower limb and foot biomechanics in children with CP. METHODS: Fifteen children aged 8-16 years at initial assessment (10 girls, 5 boys) underwent SPLATT and SPOTT procedures as part …

The radio-radial nerve transfer for elbow extension restoration in C5 to C7 nerve root injury.

Extension of the elbow is required to oppose gravity; however, activation of the triceps brachii is frequently underestimated during the surgical planning for brachial plexus injuries. This report aims to describe a novel technique of distal nerve transfer designed for elbow extension reconstruction in patients sustaining a C5-C7 nerve root injury. We report a patient sustaining a brachial plexus injury with triceps palsy and preserved finger extension motion; after careful intraneural dissection of the radial nerve, a fascicle innervating the extensor digitorum communis muscle was sectioned,

Irreparable rotator cuff tears: what to do and when to do it; the surgeon's dilemma.

Irreparable rotator cuff tears have been defined as those tears that because of their size and retraction cannot be repaired primarily. Patients with an irreparable tear present with a variety of symptoms and physical findings, and their management depends on the clinical situation and the specific location of the tear. Most of these tears occur in the older, less active patient and many of these patients are best treated without surgery. For those in whom surgery is indicated, the best procedure should be tailored to the tear.
