Coronectomy practice. Paper 2: complications and long term management.

Coronectomy was developed to reduce the incidence of iatrogenic injury to the inferior dental nerve, which can be a result of routine mandibular third molar surgery, but it is not widely accepted despite early studies that all showed positive results. This two-part paper looks at the technique in more detail to help clinicians gain a better understanding of the procedure. The first part dealt in detail with technical aspects. This one will consider the management of common complications.

An analysis of 58 traumatically intruded and surgically extruded permanent teeth.

Fifty-eight traumatically intruded and mainly surgically extruded permanent teeth were followed up for 3 years and 4 months (mid-term results: 29 teeth) and 9 months (short-term results: 29 teeth) on average. Statistically, the mid-term results showed more cases of severe crown discoloration (54%) than the short-term results (9%), but no difference in pulpal and periodontal healing. Three teeth (5%) were lost. Factors which positively influenced pulpal healing were shallow intrusion depth, intact crown and immaturity of the root. Factors which positively influenced periodontal healing were sha

Longitudinal post-eruptive mandibular tooth movements of males and females.

Unbiased estimates of post-eruptive eruption and migration of the mandibular teeth for large representative samples are presently unavailable. The purpose of this study was to evaluate pure tooth movements of untreated children and adolescents longitudinally. Lateral cephalograms of 214 French-Canadians, followed bi-annually between 8 and 15 years of age, were traced, and the positions of the mandibular permanent central incisors and first molars were digitized. Temporal changes in tooth position were evaluated relative to naturally stable mandibular reference structures, using the mandibular

Dose-effect analysis of late dental adverse effects following radiation therapy in survivors of paediatric head and neck rhabdomyosarcoma.

INTRODUCTION: Paediatric head and neck rhabdomyosarcomas (HNRMS) often require radiotherapy, causing dental complications during critical development. Limited literature suggests dose-dependent risks, but clear thresholds for late dental adverse effects are lacking. This study aims to characterize dose effects on developing dentition in paediatric HNRMS patients to identify dose-response relationships informing evidence-based treatment planning. MATERIALS AND METHODS: Orthopantomograms from survivors of HNRMS were evaluated using the validated DENTALE scoring system. Radiation doses from brac

Influence of the hyrax expander screw position on displacement and stress distribution in teeth: A study with finite elements.

INTRODUCTION: This study aimed to simulate the different positions of the hyrax appliance expander screw and evaluate tooth displacement and the stress distribution standard on the periodontal ligament using the finite element method. METHODS: Part of the maxilla with anchorage teeth, periodontal ligament, midpalatal suture, and the hyrax appliance was modeled, and finite element method models were created to simulate 6 different screw positions. There were 2 vertical positions at distances of 20 mm and 15 mm from the occlusal plane. Another position was anteroposterior, the center of the scr

Hereditary amelogenesis imperfecta. I. Oral health in children.

An epidemiological study of hereditary enamel defects comprising more than 400 000 children, 3-19 years of age, was performed in 1982-1983 in the middle of Sweden. 105 children were diagnosed as having Hereditary Amelogenesis Imperfecta (HAI). Based on clinical and genetic data a classification of HAI could be carried out based on the hypoplastic and the hypomineralized type. 99 out of the 105 children were available for a thorough clinical and radiographical examination concerning oral health and these children were analysed in terms of carious lesions, restorations, dental plaque, calculus,

Characteristic dental pattern with hypodontia and short roots in Fraser syndrome.

Fraser syndrome (FS) is a rare autosomal recessive multiple congenital malformation syndrome characterized by cryptophthalmos, cutaneous syndactyly, renal agenesis, ambiguous genitalia, and laryngotracheal anomalies. It is caused by biallelic mutations of FRAS1, FREM2, and GRIP1 genes, encoding components of a protein complex that mediates embryonic epithelial-mesenchymal interactions. Anecdotal reports have described abnormal orodental findings in FS, but no study has as yet addressed the orodental findings of FS systematically. We reviewed dental radiographs of 10 unrelated patients with FS

Cone-beam computerized tomography evaluation of the maxillary dentoskeletal complex after rapid palatal expansion.

INTRODUCTION: Rapid palatal expansion (RPE) is routinely used to correct transverse deficiencies in the maxilla, but its effects on the dentoalveolus are uncertain. The purpose of this study was to compare measurements made on cone-beam computerized tomography scans between patients with RPE treatment and controls to determine transverse dimension increases and the amounts of alveolar and dental tipping. METHODS: Twenty-five patients with posterior crossbite who required RPE treatment and 25 sex- and age-matched controls (no crossbite) were orthodontically treated and received cone-beam compu

Peg-shaped and small lateral incisors not at higher risk for root resorption.

The purpose of this study was to examine the pattern of external root resorption for peg-shaped and small lateral incisors according to various conditions and to compare it with that of normal-shaped lateral incisors. The periapical radiographs of 114 patients with 60 peg-shaped and 54 small lateral incisors were measured before and after treatment. Crown shape was assessed by examining pretreatment study casts. Findings showed that there was no significant difference in external root resorption between peg-shaped (1.09 mm) and normal lateral incisors (0.88 mm). However, a statistically signif

Adjustment of the occlusal vertical dimension in the bite-raised guinea pig.

In humans, the inappropriate occlusal vertical dimension (OVD) causes several orofacial disorders, such as bruxism and pain in the masticatory muscles and temporomandibular joint. However, it is still unclear how strictly the OVD is adjusted. To answer this question, we studied the temporal change of the OVD in bite-raised young guinea pigs. The OVD was raised by fixation of a bite-raising appliance to the lower incisors, and increased by either 3 or 1.5 mm at the first molars. After the space produced between the upper and lower molars was filled within 10 days due to eruption of the molars,

Dental implant reconstruction after endodontic failure: report of case.

Avulsion of natural teeth caused by trauma may present an unusual problem for the general practitioner. Endodontic therapy and replantation of the tooth into the socket have a relatively low long-term prognosis because of internal or external root resorption. Dental implants can provide proper support and function for missing teeth with excellent esthetic results without preparation of natural dentition. In this case report, an implant system is used to restore an anterior space created by the loss of endodontically treated central and lateral incisors.

Clinical performance of a conometric retention system for single implant restorations: 1-year analysis of a prospective multicenter study.

PURPOSE: Conventional screw- and cement-retained single implant restorations are associated with biological and technical complications. Friction-retained conometric concepts have been proposed as an alternative; however, clinical evidence for single crowns, particularly from prospective multicenter studies, remains limited. This study evaluated the 1-year clinical performance of a conometric retention system for implant-supported single crowns. METHODS: This prospective multicenter study was conducted at nine clinical centers in Europe and North America. Subjects requiring single-tooth repla

Longitudinal observational digital analysis of occlusion in Hall Technique.

OBJECTIVE: To evaluate volumetric changes over time in teeth treated with the Hall Technique (HT) and their opposing teeth. Secondary aims included assessing occluso-vertical dimension (OVD), temporomandibular joint (TMJ) function, and children's treatment perceptions. METHODS: Twenty-eight children (5-9 years-old) requiring HT treatment for one first primary molar were recruited. Dental impressions were taken. Mean volume differences were digitally calculated from scanned models (Rapidform2006) between HT-treated and opposing teeth over time. Canine overlap (to measure OVD) was assessed with

An investigator-blinded, 24-month, parallel-group, non-inferiority study to compare aesthetic restorations in primary anterior teeth in a paediatric dental clinic: study protocol for a randomised controlled trial.

INTRODUCTION: Children who suffer from severe caries in childhood may have negative impacts on the growth, development, nutritional problems and quality-of-life problems related to the oral health of the child and their family. There are no studies that have compared rehabilitative techniques of primary anterior teeth regarding patient-centred outcomes and even longevity of the restoration. Thus, this project aims to evaluate the effectiveness of restorative treatment of anterior primary teeth with monochromatic composite resin in single insertion through polyvinyl crowns, after selective remo

Long-term assessment of the periodontal health of removable partial denture wearers: A systematic review and meta-analysis.

STATEMENT OF PROBLEM: The long-term effects of wearing removable partial dentures (RPDs) remain unclear. PURPOSE: This systematic review addressed the question "Is the long-term use of RPDs deleterious to the remaining teeth?" MATERIAL AND METHODS: This review was guided by the preferred reporting items for systematic reviews and meta-analyses (PRISMA) checklist. Six databases and nonpeer-reviewed literature were searched in April 2024 without language or follow-up restrictions. Only clinical studies evaluating RPD long-term use were included. The risk of bias and evidence certainty were ass
