Ruptured hydatid cyst with an unusual presentation.

Ruptured pulmonary hydatid cyst may sometimes cause complications like empyema, bronchopleural fistula, and collapsed lung. These complications may mislead the diagnosis and treatment if prior evidence of cyst has not been documented before rupture. We present a case of a young male who presented with complete collapse of left lung with pyopneumothorax and bronchopleural fistula which was misdiagnosed as pulmonary tuberculosis. He was referred to us from peripheral hospital for pneumonectomy when his condition did not improve after six months of antitubercular chemotherapy and intercostals dra

Thorax CT findings in symptomatic hemodialysis patients.

The goal of the present study was to discuss thoracic problems in symptomatic hemodialysis patients based on the CT findings among 64 uremic patients including 34 females and 30 males of age range 14 to 83 years (mean = 61 years). We retrospectively documented complaints of cough, dyspnea, low-grade pyrexia, malaise, and weight loss. Atelectasis (79.7%), cardiomegaly (59.3%), parenchymal fibrosis-scar (50%), pleural effusion (45.3%), and ground-glass opacity (45.3%) were the most common findings. Pulmonary artery caliber was greater than 32 mm in 19 (29.7%) patients. Staphylococcus aureus (26.

Tuberculosis in patients with systemic lupus erythematosus.

In a retrospective analysis of 146 systemic lupus erythematosus (SLE) patients seen over a 5 year period, 17 patients of tuberculosis (TB) were identified yielding a prevalence rate of 11.6%. The median duration of SLE was 12 months (range 2-96 months) and 12/17 patients had disease activity score of more than five. The median duration of steroid treatment was 12 months (range 0-96 months) and the median cummulative dose of steroid was 7.75 gms (range 0-22.1 gms). Pulmonary TB (miliary-5, nonmiliary infiltrates-7 and pleural effusion-2) was the commonest type and there was an average diagnosti

Empyema and the urban black.

Fifty consecutive Black patients with thoracic empyema were studied retrospectively. The causes of the empyema were penetrating thoracic trauma (27 cases), lung abscess (10), post-aspiration pulmonary disease (4), and pulmonary and pleural tuberculosis (4); a miscellaneous group of 5 patients had empyema complicating an infected pulmonary cyst, a hepatic amoebic abscess, necrotic pulmonary lymphoma, thoracic actinomycosis and blunt thoracic trauma. Fourteen patients had bronchopleural fistulas. Patients were treated by closed-tube thoracostomy drainage alone (6 cases), rib resection and draina

Tuberculosis in children with sickle cell anaemia: a retrospective study in French tertiary care centres.

UNLABELLED: Tuberculosis (TB) and sickle cell anaemia (SCA) may affect the same population of patients, particularly in Africa but also in high-TB incidence areas in developed countries. However, few data are available from children with SCA who develop TB. The aim of this study was to describe the clinical features and outcome of TB diagnosed in children with SCA. We conducted a retrospective, descriptive study in three referral centre of Sickle Cell Disease in Paris, France. We included 11 patients with SCA who develop TB. The median age at TB diagnosis was 11 years [7.5-14.5]. Two patients

The Effect of Empiric Initiation of Treatment on Outcomes Among Patients Presenting With Extrapulmonary Mycobacterium Tuberculosis Infection in a Community Hospital Setting.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) accounts for ~20% of Mycobacterium tuberculosis infections in Pakistan. Diagnostic delays are common and may worsen outcomes. We evaluated whether empiric treatment initiation improves outcomes among patients with EPTB at a large public hospital in Islamabad, Pakistan. METHODS: Retrospective cohort of consecutive EPTB cases at Pakistan Institute of Medical Sciences (January-June 2016). Patients were categorized as clinically diagnosed (symptoms/radiology) or laboratory-supported (histology, microbiology, or biochemical tests). Standard Directly O

Point-of-Care Ultrasound for Extrapulmonary Tuberculosis in India: A Prospective Cohort Study in HIV-Positive and HIV-Negative Presumptive Tuberculosis Patients.

Diagnosing extrapulmonary tuberculosis (EPTB) is challenging. Point-of-care ultrasound (POCUS) for human immunodeficiency virus (HIV)-associated EPTB is applied in sub-Saharan Africa. This study aimed at evaluating the applicability of POCUS for diagnosing EPTB in HIV-positive and HIV-negative presumptive tuberculosis (TB) patients in India, a country of moderate relative TB and HIV burden. Presumptive TB patients at Kasturba Hospital, Manipal, India, prospectively underwent POCUS evaluating for pericardial, pleural and ascitic effusion, abdominal lymphadenopathy, and hepatic and splenic micro

A radiologic study of tuberculosis of the abdomen (gastrointestinal tract).

The roentgenologic features of 160 cases of tuberculosis of the abdomen are reviewed. The 3 standard examinations of the gastrointestinal tract (plain film abdominal roentgenography, barium meal and follow-through studies, and barium enema examinations) are required to provide useful pointers towards the diagnosis. In plain film roentgenography of the abdomen, the triad of ascites, absence of gas shadows in the right iliac fossa, and segmental dilatation of the terminal ileum demonstrates features which are very suspicious of intestinal tuberculosis. In the barium meal studies showing disorder

Comparison of chest computed tomography features between pulmonary tuberculosis patients with culture-positive and culture-negative sputum for non-mycobacteria: A retrospective observational study.

Although complication with non-mycobacterial pneumonia among patients with pulmonary tuberculosis (TB) may lead to poor prognosis, discrimination between TB complicated with and without non-mycobacterial pneumonia using radiological imaging has not been fully elucidated. We aimed to clarify the differences in chest computed tomography (CT) features between pulmonary TB patients with culture-positive and culture-negative sputum for non-mycobacteria.We retrospectively included consecutive patients admitted to our hospital from January 2013 to December 2015 for bacteriologically-confirmed pulmona

Tuberculosis and risk of cancer: a Danish nationwide cohort study.

OBJECTIVES: To investigate the short- and long-term risk of cancer in patients with active tuberculosis (TB). DESIGN: Using Danish nationwide databases, we quantified cancer risk in TB patients during 1978-2011 compared with the general population. RESULTS: We observed 1747 cancers in 15 024 TB patients (median follow-up 8.5 years), reflecting a standardised incidence ratio (SIR) of 1.52 (95%CI 1.45-1.59). All-time SIR for extra-pulmonary cancer was 1.29 (95%CI 1.22-1.36) and for lung cancer it was 3.40 (95%CI 3.09-3.74). Absolute cancer risk 3 months after TB was 1.83% (SIR 11.09, 95%CI 9.8

Clinical diagnostic utility of adenosine deaminase, interferon-γ, interferon-γ-induced protein of 10 kDa, and dipeptidyl peptidase 4 levels in tuberculous pleural effusions.

OBJECTIVE: Current tools for the diagnosis of tuberculous pleural effusions are suboptimal. The study was undertaken to evaluate the accuracy of pleural fluid adenosine deaminase (ADA), interferon (IFN)-γ, interferon-γ-induced protein of 10 kDa (IP-10), and dipeptidyl peptidase (DPP) 4 levels in differentiating tuberculous pleural effusion (TPE) and non-TPE. METHODS: A total of 122 samples of pleural effusion were studied. Pleural fluid ADA activity was measured with the colorimetric method, and IP-10, IFN-γ, and DPP4 levels were measured with enzyme-linked immunosorbent assay. RESULTS: ADA

A Diagnostic Puzzle: Unveiling Tuberculosis Peritonitis in an Immunocompromised Patient.

Tuberculosis (TB) remains a leading cause of infectious morbidity and mortality worldwide. Extrapulmonary TB, particularly abdominal TB, is a diagnostic challenge due to its nonspecific presentation and often low sensitivity for conventional tests. Immunocompromised patients, such as those with human immunodeficiency virus (HIV), are at increased risk and may lack classic systemic symptoms, preventing timely diagnosis. A 61-year-old HIV-positive female on antiretroviral therapy and with an undetectable viral load presented with progressive abdominal distension, postprandial fullness, and alter

PD-1 modulating Mycobacterium tuberculosis-specific polarized effector memory T cells response in tuberculosis pleurisy.

Host-pathogen interactions in tuberculosis (TB) should be studied at the disease sites because Mycobacterium tuberculosis (M.tb) is predominantly contained in local tissue lesions. T-cell immune responses are required to mount anti-mycobacterial immunity. However, T-cell immune responses modulated by programmed cell death protein 1 (PD-1) during tuberculosis pleurisy (TBP) remains poorly understood. We selected the pleural fluid mononuclear cells (PFMCs) from TBP and PBMCs from healthy donors (HD), and characterized PD-1-expresing T-cell phenotypes and functions. Here, we found that the PFMCs

Clinical features of tuberculosis associated with HIV infection in Taiwan.

To understand the clinical characteristics and outcome of tuberculosis (TB) in patients with acquired immunodeficiency syndrome (AIDS) in Taiwan, we reviewed the medical records of 118 adult AIDS patients who were hospitalized at National Taiwan University Hospital between January 1988 and September 1995. Among them, 29 (24.6%) had TB. The mean age of the AIDS patients with TB was 37 years (range, 25-66 yr). Most patients were in the advanced stages of AIDS when human immunodeficiency virus (HIV) infection and/or TB were first diagnosed. The mean CD4+ lymphocyte count was 0.037 x 10(9)/L (rang

Serological response of tuberculosis patients to antigen 60 of BCG.

Two ELISA tests (IgG and IgM) for the serodiagnosis of tuberculosis, both based on antigen 60 (A60) of M. bovis BCG, were applied to 1,644 controls and patients to analyse the immune response in different forms of this infectious disease. Out of 200 healthy individuals, 148 being tuberculin--positive BCG-vaccinated adults, only 10 contacts--nurses of the pneumology department and laboratory technicians of the mycobacterial laboratory--were found positive for anti-A60 IgG. One quarter of hospitalized patients affected by non-tuberculous pneumopathies (194 in total) were found weakly positive fo
