AV node dysfunction in the mitral valve prolapse syndrome.

The mitral valve prolapse syndrome is associated with a variety of atrial and ventricular arrhythmias. A portion of these patients manifest bradyarrhythmias, which in turn, may be the cause of lightheadedness and syncope. Thie study details the clinical and electrophysiologic characteristics of seven patients with symptomatic mitral valve prolapse and AV node dysfunction. The electrophysiology study demonstrated either a prolonged AH interval or abnormal response to atrial pacing in six of seven patients. A significant proportion of these patients had abnormalities of sinus node function and d

Anatomic substrate of impaired antegrade conduction over an accessory atrioventricular pathway in the Wolff-Parkinson-White syndrome.

We present a patient with Wolff-Parkinson-White syndrome who died of a myocardial infarction 19 months after clinical electrophysiologic studies. These studies suggested the presence of a left atrioventricular accessory pathway that sustained conduction well in the retrograde direction but only intermittently in the antegrade direction. Postmortem examination of the heart revealed three accessory atrioventricular pathways in proximity to each other in the posterolateral atrioventricular region. One pathway showed complete fibrosis and two showed patchy fibrosis. The fibrosis suggests an anatom

The R-on-T phenomenon during supraventricular tachycardia in childhood.

The electrocardiographic R-on-T phenomenon was observed in the absence of Wolff-Parkinson-White syndrome during recurrent episodes of supraventricular tachycardia (SVT) with rates between 240 to 275 per minute in a 5 year old boy. The rapid ventricular response during SVT corroborates the recent finding of a short refractory period of the A-V node in children. The demonstration of the R-on-T phenomenon in this child may be important in attempting to understand the pathogenesis of sudden death in children.

[Localization of the conduction defect in complete A-V block by means of His bundle electrograms (author's transl)].

The localization of the conduction defect in so-called complete A-V block can be established by electrocardiography only indirectly and unreliably. In principle the region of the atrio-ventricular junction, i.e. the A-V node and the bundle of His, may be involved just as well as the two branches. Analysis of the configuration of the QRS complex in the electrocardiogram is unreliable, particularly in cases with bundle branch block, because any pattern can arise from the appropriate site of a tertiary focus of stimulation in one of the two ventricles. Recording His bundle potentials (His bundle

Nkx2-5 suppresses the proliferation of atrial myocytes and conduction system.

RATIONALE: Tight control of cardiomyocyte proliferation is essential for the formation of four-chambered heart. Although human mutation of NKX2-5 is linked to septal defects and atrioventricular conduction abnormalities, early lethality and hemodynamic alteration in the mutant models have caused controversy as to whether Nkx2-5 regulates cardiomyocyte proliferation. OBJECTIVE: In this study, we circumvented these limitations by atrial-restricted deletion of Nkx2-5. METHOD AND RESULTS: Atrial-specific Nkx2-5 mutants died shortly after birth with hyperplastic working myocytes and conduction sy

[Radiofrequency fulguration for supraventricular tachycardia in pediatrics].

Supraventricular tachycardias (SVT) are the most frequent cause of tachycardia in children. Its pharmacological treatment has adverse effects, is not curative, and is not always effective. During the last few years radiofrequency ablation (RF-A) has changed the treatment. The purpose of this study is to evaluate our experience in RF-A in children with SVT. Between 1990 and 1995, 92 patients (1 month to 17 years old) underwent electrophysiological study after the diagnosis of SVT. RF-A was attempted in 55 patients with accessory pathways (AP), slow-pathway of the atrioventricular node, or ectop…

Mustard's operation modified to avoid dysrhythmias and pulmonary and systemic venous obstruction.

Between, May of 1973 and May of 1978, 130 patients underwent correction of complete transposition of the great arteries by a modification of … d. Our operative and autopsy findings point to the importance of avoiding the area of the sinus and atrioventricular nodes, since by doing so, we have reduced postoperative dysrhythmias. Operative technique, in particular, wide excision of the septum secundum and baffle design as suggested by Brom, have reduced the incidence of superior and inferior caval obstruction a …

A concealed left-sided accessory pathway with nodoventricular conduction features: a case report.

BACKGROUND: Nodoventricular accessory pathways are rare connections between the atrioventricular (AV) node and ventricular myocardium, typically exhibiting decremental conduction and often mimicking AV nodal re-entrant tachycardia. Their diagnosis remains challenging, particularly in the absence of manifest pre-excitation. CASE SUMMARY: A 30-year-old male with recurrent paroxysmal supraventricular tachycardia (SVT) underwent an electrophysiological study. Electrocardiogram showed no pre-excitation. Pacing manoeuvres revealed concentric, decremental ventriculoatrial conduction, with no evidenc

Pacemaker-mediated reentrant arrhythmia facilitated by an atrial-tracking leadless pacemaker.

INTRODUCTION: Pacemaker-mediated tachycardia is a known arrhythmia in patients with dual chamber pacemakers and defibrillators and intact ventriculoatrial (VA) conduction. We report a case of pacemaker-mediated reentrant arrhythmia (PMRA) in a patient with an atrioventricular (AV) synchronous leadless pacemaker. METHODS AND RESULTS: A 91-year-old female presented with 2:1 AV conduction and received an AV synchronous leadless pacemaker. She had atrial mechanical sense-ventricular paced beats between heart rates of 80-100 bpm more than 80% of the time. She was found to have a new cardiomyopathy

Atrial tachycardia originating from a right atrial free wall diverticulum: case report.

BACKGROUND: Atrial tachycardia (AT) is an arrhythmic disorder originating from the atrium, independent of the atrioventricular node, and includes various types based on different mechanisms such as abnormal automaticity, triggered activity, and re-entry. These mechanisms are often related to specific anatomical structures. Focal AT, though relatively rare, typically arises from well-known locations in the left and right atria, such as the pulmonary veins, mitral valve annulus, crista terminalis, and coronary sinus ostium. CASE SUMMARY: We report a rare case of AT originating from a diverticul…

Hospitalization for permanent pacemaker implantation in the context of isolated sinus node dysfunction is associated with increased mortality compared with an outpatient strategy.

BACKGROUND: Permanent pacemaker (PPM) implantation is a well-established treatment for symptomatic sinus node dysfunction (SND). The optimal timing of this intervention is unclear, with atrioventricular blocks often prioritized in resource stressed waiting lists due to mortality concerns. METHODS: Mortality data was compared between patients receiving elective outpatient (OP) PPM implantation, and those presenting to hospital for urgent inpatient (IP) management for symptomatic SND. Survival analysis was conducted using Kaplan-Meier plots and compared using the log-rank test. Univariable and

Predictors of permanent pacemaker implantation after sinus conversion of cavotricuspid isthmus-dependent atrial flutter.

It is unclear which factors are associated with progressive sinus node dysfunction after cavotricuspid isthmus (CTI)-dependent atrial flutter (AFL) ablation. We sought to evaluate the incidence and predictors for permanent pacemaker (PPM) implantation after CTI-dependent AFL ablation. Between January 2011 and June 2021, 353 patients underwent CTI-dependent AFL ablation were studied. During a median follow-up of 31.6 months, 30 patients (8.5%) received PPM implantation, 24 for sick sinus syndrome and 6 for atrioventricular block. In multivariable model, prior atrial fibrillation (AF) (HR 3.570;

Lacosamide-induced sinus node dysfunction followed by severe agranulocytosis.

BACKGROUND: Lacosamide (LCM) is the antiepileptic drug approved by the U.S. Food and Drug Administration in 2008 that facilitates slow activation of the voltage-gated sodium channels. Neutropenia and cardiac events including sinus node dysfunction (SND) and atrioventricular block have been previously reported as adverse effects of LCM. To date, there have been no reports of severe agranulocytosis resulting in death associated with LCM. Additionally, there have been no reports of concomitant SND and agranulocytosis after LCM administration. Herein we report the first case of LCM-induced severe

Radiofrequency ablation of anteroseptal accessory pathway--a challenge to the electrophysiologist.

INTRODUCTION: Anteroseptal accessory pathways (APs) are located in the apex of the triangle of Koch's connecting the atrial and ventricular septum in the region of the His bundle. Ablation of anteroseptal pathway locations remains a challenge to the electrophysiologist due to a very high risk of transiet or permanent atrioventricular (AV) block. CASE REPORT: A male, 18-year-old, patient was hospitalized due to radiofrequency (RF) ablation of APs. He was an active football player with frequent palpitations during efforts accompanied by dyspnea and lightheadedness, but without syncope. Electroc

Current status of the surgical treatment of atrial fibrillation.

Atrial fibrillation (AF) affects several million patients worldwide and is associated with a number of heart conditions, particularly coronary artery disease, rheumatic heart disease, hypertension, and congestive heart failure. The treatment of AF and its complications is quite costly. Atrial fibrillation usually results from multiple macro-re-entrant circuits in the left atrium. Very frequently, particularly in association with mitral valve disease, these circuits arise from the area of the junction of the pulmonary venous endothelium and the left atrial endocardium. Pharmacological therapy i
