Bedside tracking of functional autonomic age in preterm infants.

BACKGROUND: Preterm birth predisposes infants to adverse outcomes that, without early intervention, impacts their long-term health. To assist bedside monitoring, we developed a tool to track the autonomic maturation of the preterm by assessing heart rate variability (HRV) changes during intensive care. METHODS: Electrocardiogram (ECG) recordings were longitudinally recorded in 67 infants (26-38 weeks postmenstrual age (PMA)). Supervised machine learning was used to generate a functional autonomic age (FAA), by combining 50 computed HRV features from successive 5-minute ECG epochs (median of 2

Machine Learning Prediction Models for Mortality in Intensive Care Unit Patients with Lactic Acidosis.

BACKGROUND: Lactic acidosis is the most common cause of anion gap metabolic acidosis in the intensive care unit (ICU), associated with poor outcomes including mortality. We sought to compare machine learning (ML) approaches versus logistic regression analysis for prediction of mortality in lactic acidosis patients admitted to the ICU. METHODS: We used the Medical Information Mart for Intensive Care (MIMIC-III) database to identify ICU adult patients with lactic acidosis (serum lactate ≥4 mmol/L). The outcome of interest was hospital mortality. We developed prediction models using four ML appr

Mental health risk stratification using heart rate variability and prefrontal electroencephalography: eXtreme gradient boosting-based classifier and latent class analysis-based subtyping.

Heart rate variability (HRV) and electroencephalography (EEG) are reflective of cardiac autonomic modulation/emotional regulation and cortical brain activity/psychopathology, respectively. This study aimed to determine the key HRV/prefrontal EEG classifiers of healthy controls (HC) versus patients with psychiatric disorders (PT) and to identify the subtypes of these features. A simultaneous 5-min-long acquisition of earlobe photoplethysmography and resting-state prefrontal EEG signals was performed in 147 HC and 123 PT [major depressive disorder (MDD; N = 107) and panic disorder (N = 16)]. Supply-demand coordination of soil conservation services and its driving mechanism in grassland ecosystems of northern China.

Soil erosion poses a persistent threat to grassland ecosystems and sustainable land management; previous large-scale assessments focused on soil conservation supply but ignored its alignment with long-term spatially heterogeneous erosion pressure. This disconnect limits the ability to diagnose where and why soil conservation services fail to meet ecological needs. In this study, a 24-year long-term dataset that integrated multi-source remote sensing, land-use data, and erosion modeling was leveraged to quantify the soil conservation supply and erosion pressure and analyze their coordination ac

A serum proteomics-based predictive model for steroid-resistant immune checkpoint inhibitor associated myocarditis.

BACKGROUND: Immune checkpoint inhibitor-associated myocarditis (ICIAM) is one of the immune-related adverse events (irAEs) with low incidence but extremely high mortality. High-dose glucocorticoid pulse therapy remains the recommended initial treatment for patients with ICIAM. However, an inadequate response to corticosteroid therapy is observed in some patients, who are subsequently classified as having steroid-resistant immune checkpoint inhibitor-associated myocarditis (srICIAM). Early combination of second-line immunosuppression may represent an effective therapeutic strategy. However, Bio

Vocal Fundamental Frequency and Generalized Anxiety Disorder: Insights from a Large Multi-Country Cohort in Africa.

OBJECTIVE: Voice-based biomarkers have been proposed as objective tools for anxiety assessment, yet existing research relies on small, demographically homogeneous samples and shows inconsistent associations between fundamental frequency (F0) and anxiety. Using a large, diverse, multi-country African cohort, we evaluated whether F0 measures extracted from uncontrolled audio recordings are associated with generalized anxiety beyond demographic and self-reported health factors. METHODS: Speech recordings and survey data were collected from 1788 adults in Botswana, Ghana, Nigeria, and Tanzania us

Developing and validating an electronic health record-embedded AI model for managing multimorbid hospitalisation risk in patients with chronic RESpiratory disease (AiRES): a study protocol.

BACKGROUND: Chronic respiratory diseases (CRDs), such as asthma and chronic obstructive pulmonary disease (COPD), are heterogeneous conditions with a high multimorbidity burden. However, existing risk assessment instruments prioritise physiological measures while overlooking systemic comorbidities. We aim to develop and validate an electronic health record (EHR)-embedded artificial intelligence (AI) model-AiRES (AI in patients with RESpiratory disease)-to predict the 30-day, 90-day and 180-day risks of all-cause and index-disease hospitalisations. This model represents a first step towards a c

Machine learning-based prediction of Familial Hemiplegic Migraine risk from genetic variants.

BACKGROUND: Familial Hemiplegic migraine (FHM) is a rare migraine subtype characterised by transient unilateral motor weakness. Although familial forms are associated with variants in CACNA1A, ATP1A2, and SCN1A genes, many cases remain genetically unexplained, suggesting contributions from additional rare variations including single-nucleotide variants (SNVs) and copy-number variants (CNVs). METHODS: Whole-exome sequencing (WES) data from 182 FHM cases and 1035 controls were analysed. Rare SNVs (allele frequency AF <0.01) were prioritised using pathogenicity annotation and ACMG criteria, whil

Comparison of Interpretable Machine Learning Models Using Systemic Inflammation Index to Predict Preterm Birth in Gestational Diabetes Mellitus.

BACKGROUND: Gestational diabetes mellitus (GDM) elevates preterm birth risk, highlighting the need for improved prediction methods to enhance outcomes. Current models show limited accuracy by ignoring some inflammatory biomarkers (eg, PLR, LMR, SII). Machine learning (ML) can better analyze complex patterns but remains underused for GDM preterm birth prediction. OBJECTIVE: This study develops an interpretable ML model combining systemic inflammatory indices and traditional clinical markers to predict preterm birth in GDM. Enabling early risk stratification at diagnosis, it facilitates timely

Predictive modelling of fine-mode combustion aerosol events near Indonesian coal-fired power plants using AE-filtered Himawari-8 satellite data.

This study develops a predictive framework to identify fine-mode combustion aerosol events around coal-fired power plants (CFPPs) in Java, Indonesia, using Himawari-8 satellite data filtered using the Ångström Exponent (AE). Although an AE > 1.2 indicates fine-mode particles characteristic of combustion processes (including biomass combustion, traffic, and industrial emissions), the persistent nature of CFPP operations allows for the characterization of aerosol signals in their vicinity. Analysis of 41,119 daily observations (2015-2025) across 13 major CFPPs reveals that 8.52% of days meet the

Use of Artificial Intelligence to Predict Outcomes in Critically Ill Patients Receiving Extracorporeal Membrane Oxygenation: A Systematic Review.

Extracorporeal membrane oxygenation (ECMO) is widely used in patients with severe cardiac or respiratory failure. ECMO is resource and cost intensive and carries the potential for significant complications; therefore, identifying patients who would derive the most benefit and personalizing its application are important. A number of scoring systems have previously been developed to predict the outcomes of patients receiving ECMO. Machine learning (ML)-based models are highly effective at incorporating the large amount of complex data and large number of variables that may be associated with out
