Breakage of an intramedullary rod after bone union in congenital pseudoarthrosis of the tibia: a report of two cases.

We report 2 patients with congenital pseudoarthrosis of the tibia who underwent intramedullary Rush rod transfixation through the ankle joint following refracture and nonunion of vascularised fibular grafting 6 and 8 months earlier. After 9 and 5 years, both Rush rods were broken at the level of the ankle joints, while the reconstructed area was solidly united. The growth of the distal tibia increased the distance of the tips of the broken rod and hence the ankle joint motion. The broken tips may damage the articular cartilage and result in valgus deformity of the ankle and limb length discrep

Saving the Limb-Tissue Collectors, Growth Factors, and Antibiotics in a War Injury.

ABSTRACT: As a life-saving procedure, limb amputation is frequently indicated in severe war injuries, especially in suboptimal medical supply. We report of a 56-year-old Ukrainian male who was wounded on the battlefield by an air attack during Russian early invasion in Zaporizhzhia. The blast injury caused a critical soft-tissue defect of the lower leg combined with comminuted fractures of the tibia and fibula. Although indicated, the patient refused the amputation of the lower leg. Initially closed reduction and stabilization by an external fixator, local debridement as well as soft-tissue co

Rickets in the New Age of Food Allergies: A Case Report and Comprehensive Review.

INTRODUCTION: Rickets is a disorder of defective bone mineralization resulting in skeletal deformities, growth retardation, and increased risk of fractures. Nutritional rickets, once a historical disease, has reemerged in select pediatric populations despite modern food fortification efforts. Many of the most common food allergens - dairy, eggs, and fish - are primary dietary sources of vitamin D, which presents a unique risk factor for development of rickets. Children with multiple food allergies are at a heightened risk, particularly if their diets are not adequately supplemented. CASE REPO

Novel Elastic Syndesmosis Hook Plate Fixation Versus Routine Screw Fixation for Syndesmosis Injury.

Unstable external-rotation type ankle fractures with concomitant syndesmosis injury commonly occur. Syndesmosis screw fixation has long been regarded as a reference standard treatment for syndesmosis injury. However, its complications and biomechanical disadvantages have become controversial; thus, we designed a novel elastic syndesmosis hook plate (ESHP) that combines the features of both rigidity and flexibility. The purpose of the present study was to introduce this new method and compare its clinical outcomes with those of routine screw fixation. We randomized 25 patients to the screw fixa

Hybrid III Lower Leg Injury Assessment Reference Curves Under Axial Impacts Using Matched-Pair Tests.

The objective of the present study was to derive injury probability curves applicable to the Hybrid III dummy (also termed the Anthropomorphic Test Device, ATD) lower leg under axial impacts for military applications. A matched-pair approach was used. Axial impacts were delivered to below knee foot-ankle complex preparations of the lower leg of the ATD using pendulum and custom vertical accelerator devices. Military boot was used in some tests. Post mortem human surrogate (PMHS) preparations were used as matched-pair tests for injury outcomes. The alignment was such that the foot-ankle complex

Fluoroscopy-guided reduction and fibular nail fixation to manage unstable ankle fractures in patients with diabetes: a retrospective cohort study.

AIMS: Patients with diabetes are at increased risk of wound complications after open reduction and internal fixation of unstable ankle fractures. A fibular nail avoids large surgical incisions and allows anatomical reduction of the mortise. PATIENTS AND METHODS: We retrospectively reviewed the results of fluoroscopy-guided reduction and percutaneous fibular nail fixation for unstable Weber type B or C fractures in 24 adult patients with type 1 or type 2 diabetes. The re-operation rate for wound dehiscence or other indications such as amputation, mortality and functional outcomes was determine

Partial failure of a muscle perforator free flap salvaged using its source vessel as a "flow-through" pedicle to a second perforator free flap.

Partial free flap failure can sometimes be more exasperating than total failure as the reason may be unknown, the best potential recipient site has already been violated, and the risk of undertaking a mandatory second free flap perhaps too extenuating. Muscle perforator flaps, often for inexplicable reasons, are also susceptible to partial flap necrosis. However, the large source vessel to their requisite perforator(s) can serve as a convenient second recipient site for attachment as a "flow-through" to another perforator free flap to salvage the overall reconstruction. In a metachronous fashi

Do superoxide radicals in blood indicate anastomotic patency after microvascular tissue reperfusion?

Superoxide radicals were measured in the blood of six patients who underwent vascular reconstruction after ischemic injury in an attempt to predict prognosis following surgery. Three free-tissue transfers (two free latissimus dorsi flaps, one free vascularized fibular osteocutaneous graft) were performed on patients with skin or bone defects associated with open tibial fractures. Vascular reconstructions were performed on two patients with popliteal vascular injuries, in one case with an open femoral fracture and in another with an open knee dislocation. A third vascular reconstruction was per

Maxillofacial ballistic injuries: A 13-year single-institution experience.

INTRODUCTION: Maxillofacial ballistic and pyrotechnic injuries represent complex reconstructive challenges due to severe bone and soft tissue loss, infection risk, and high demands for both function and aesthetics. Recent trends favour early, definitive reconstruction with vascularized tissue, but optimal timing and outcomes remain controversial. MATERIALS AND METHODS: Patients with ballistic or pyrotechnic facial injuries treated between 2010 and 2023 were retrospectively analysed. Data included mechanism of injury, anatomical and soft tissue regions involved, reconstructive approaches, comp

[Secondary orthopaedic complications after childhood tumors of the musculoskeletal system].

BACKGROUND: Multidisciplinary care, modern care management, and medical progress have brought significant gains in modern survival rates for children and adolescents with tumors of the musculoskeletal system. OBSERVATIONS: The surgical approach must rest on the consideration of the long-term orthopedic sequelae likely to be caused by the elected treatment (limb amputation versus limb conservation - reconstruction choices), as well as by adjuvant therapies, such as chemotherapy or radiotherapy. Complications due to allograft reconstructions (infections, fractures, pseudoarthritis) occur within

Arthroscopic Evaluation of Syndesmotic Instability in a Cadaveric Model.

BACKGROUND: Ankle fractures are among the most common lower extremity injuries. Proper care requires evaluation for syndesmotic ligament disruption. Ankle arthroscopy has been proposed as an intraoperative tool that can evaluate stability. Our focus was to evaluate the amount of displacement produced in the coronal, sagittal, and transverse planes visualized through ankle arthroscopy in a cadaveric model. METHODS: Seven below-knee specimens were mounted in a traction tower. Four groups were evaluated: no ligamentous disruption; anterior inferior tibiofibular ligament and interosseous ligament

How Do Gunshot and Explosive Injuries to the Lower Extremities Differ in Severity and Treatment? A Comparative Study From the Israel-Gaza Conflict.

BACKGROUND: Lower extremity injuries are common in conflict-related trauma, with gunshot wounds (GSWs) causing localized damage and explosive trauma leading to extensive tissue injuries. Existing research lacks direct comparisons of injury severity and treatment outcomes between GSWs and explosive trauma in modern conflicts. This study clarifies these differences to improve triage strategies, surgical planning, and rehabilitation protocols. QUESTIONS/PURPOSES: (1) How did GSWs and explosive trauma differ in terms of injury severity, including the proportion of patients in each group who exper

Reconstruction of a Complex Foot Injury With Free Remodeled Fibular Osteocutaneous Flap: A Case Report and Literature Review.

Management of complex foot injuries, which involve open fractures and severe trauma to soft tissues, represent a challenge to orthopedic clinicians. In the present case report, we treated a complex foot injury with a remodeled fibular osteocutaneous free flap to reconstruct the anterior and lateral areas of the foot. The flap survived completely. At the 9-month follow-up examination, bony union of the graft bone was identified by radiographic examination. The reconstructed foot could bear body weight, and the patient could maintain a bipedal gait without discomfort. The remodeled fibular osteo

Critical Care Transport: Blunt Polytrauma in Pregnancy.

AUDIENCE: This simulation is designed for critical care transport nurses and attending physicians. It can also be adapted for critical care transport paramedics and respiratory therapists as well as emergency medicine nurses, residents, and attending physicians. INTRODUCTION: Emergency and trauma surgery practitioners routinely perform primary and secondary surveys as a systematic approach to trauma care. While this approach has broad applications, clinicians must also be versed in the nuances of caring for special populations in trauma. One such example is the obstetric patient. The incidenc

Cannulated Intramedullary Screw Fixation of Distal Fibular Fractures.

Background: Open reduction and internal fixation (ORIF) methods, primarily plates and screws, remain the standard of treatment for distal fibular fractures. This study evaluates the use of a cannulated intramedullary screw as a minimally invasive treatment method for distal fibular fractures, which has not been reported in the current literature. Methods: This retrospective study included 45 patients with distal fibular fractures treated with cannulated intramedullary screw fixation. All patients included in the cohort had a soft-tissue condition and/or comorbidity. The mean age was 54 years.
