[Epidural empyema and orbital phlegmon. Computerized tomographic diagnosis of rare complications of sinusitis].

Among 4019 computed tomograms of the head performed in 2 years we found 2 epidural empyemas combined with an ipsilateral orbital phlegmon and one periorbital abscess with an incipient orbital phlegmon. In all 3 patients they turned out to be intracranial or orbital complications of an adjacent paranasal sinusitis. The CT diagnosis was a guideline for further therapy and was verified by immediate surgery. The reliable diagnosis of orbital and intracranial space-occupying lesions makes CT the radiological method of choice in case of suspicion of a complicated sinusitis.

Etiological diagnosis in sinusitis: ultrasonography as clinical complement.

Accumulation of purulent sinus secretion reflects an inflammation caused by pyogenic bacteria, whereas the accumulation of nonpurulent effusion may have origins other than a bacterial infection. Ultrasonography differentiates the disordered sinus from the healthy one, but not purulent from nonpurulent secretion. In 90 patients, supposed to suffer from sinusitis, a correct diagnosis, sinus empyema vs. not sinus empyema, was established in a majority of cases by means of clinical evaluation alone. By introducing ultrasonography as a complement to the clinical evaluation, the diagnostic reliabili

Retro-orbital hematoma and proptosis associated with chronic sinus disease.

Sudden unilateral proptosis occurred in a patient with a history of chronic sinusitis. The radiologic description of chronic sinusitis associated with the computerized tomography scan picture of a retro-orbital mass justified surgical exploration and evacuation of a hematoma. The proximity of the orbital cavity and the sinuses, plus their common venous drainage system, can account for this disease.

3D Imaging of the Comparative Anatomical Structure of the Ossa Cranii in Some Ruminants.

Three-dimensional comparative descriptions of the ossa cranii across representatives of the Camelidae, Giraffidae, and Bovidae remain limited despite their relevance to comparative veterinary anatomy and clinical practice. Given the distinct structural variations and functional adaptations of these families, a descriptive comparison of ossa cranii morphology is needed to advance anatomical knowledge and establish species-specific reference data. This study compared the principal ossa cranii of camel, giraffe, cattle, sheep, and goat to identify conserved anatomical features and species-specifi

Management of multiple spontaneous nasal meningoencephaloceles.

OBJECTIVES/HYPOTHESIS: Multiple spontaneous nasal meningoencephaloceles in the same patient are rare lesions. Although many skull base defects occur after prior trauma or surgery, otolaryngologists must be aware of the potential for spontaneous encephaloceles. We present our experience with this unusual condition and discuss its pathophysiology and unique management issues. STUDY DESIGN: Retrospective. METHODS: Review of medical records, radiographic images, and cerebrospinal fluid pressures. RESULTS: We identified 5 patients with multiple, simultaneous, spontaneous encephaloceles: 4 patien

Right orbital edema masquerading a hematologic malignancy.

INTRODUCTION: Multiple myeloma is caused by abnormal proliferation of plasma cells that affects more commonly African Americans. It classically presents with hypercalcemia, renal failure, anemia, and lytic bone lesions. The aim of this article is to present an unusual case of a 63-year-old African-American female with multiple myeloma who presented with worsening right-sided eye swelling for the past 3 weeks and to briefly review ophthalmologic manifestations of multiple myeloma. CASE DESCRIPTION: Our patient's presentation was associated with a throbbing frontal headache, nasal congestion, m

Effects of a dexamethasone-releasing stent on osteoneogenesis in a rabbit model.

BACKGROUND: This study is an evaluation of wound healing in an animal model for surgery of frontal sinusitis and treatment effect of topically released dexamethasone using a drug-releasing stent with special emphasis of osteoneogenesis. METHODS: A prospective, controlled, randomized, double-blinded animal study was performed. Nineteen New Zealand white rabbits were subjected to surgery via an external approach, a 4-mm circular wound was created on the medial side of the maxillary sinus and the underlying bone was denuded of periosteum. The wound was covered in a randomized fashion with either…

Treatment of chronic frontal sinus disease with the galeal-frontalis flap: a long-term follow-up.

BACKGROUND: Management of benign chronic frontal sinus disease is difficult. Patients are frequently seen by multiple specialties for medical treatment and endonasal procedures before they seek or require definitive treatment with frontal sinus obliteration. The progression of the disease may lead to serious or life-threatening conditions such as local bone destruction, periorbital abscess, osteomyelitis, meningitis, cranial epidural abscess, or septicemia. This study presents the use of the galeal-frontalis myofascial flap as part of the treatment of this disease. METHODS: Thirty-one patient

Frontal sinus disease. III. Experimental and clinical factors in failure of the frontal osteoplastic operation.

The surgical approach to frontal sinus disease has been subject to much variation. Experimental evidence for new treatment modalities is quite limited. Frontal osteoplasty, while probably the best procedure to date, has up to a 25 percent failure rate. Possible complications include recurrent disease, incomplete bony obliteration (Macbeth technique), infection of the adipose implant, frontal bossing or depression, and laceration of the dura. Four experimental groups were designed using the canine frontal sinus model. Results indicated that stripping the mucosa in a normal sinus with intact per

Submental intubation as an alternative to tracheostomy in complex panfacial fracture: a case report.

Submental intubation, first described in 1986, is an effective alternative to tracheostomy in patients with complex facial fractures in whom nasotracheal intubation is contraindicated and orotracheal intubation would interfere with the surgical field. We report the case of a 38-year-old man with a history of morbid obesity who suffered a panfacial fracture with involvement of the frontal sinuses and intracranial communication after falling from a second floor. Nasotracheal intubation was contraindicated, and orotracheal intubation would have interfered with the planned intermaxillary fixation.

Hypoosmolar Hyponatremia as Presenting Symptom of a Rare Case of Olfactory Neuroblastoma.

Hyponatremia is a common electrolyte disturbance. The syndrome of inappropriate antidiuresis (SIAD), characterized by nonosmotic release of arginine vasopressin, is a typical cause of hyponatremia. Ectopic secretion of arginine vasopressin by a tumor is often associated with small cell lung cancer but can also be seen in other neoplasms. We present a rare case of a previously healthy 34-year-old woman, evaluated for refractory hypoosmolar hyponatremia consistent with SIAD. After excluding other causes of SIAD, imaging revealed contrast-enhancing mucosal thickening in the left maxillary sinus w

Nasolacrimal Duct Obstruction Secondary to Eosinophilic Chronic Rhinosinusitis: A Case Report.

A 47-year-old Japanese woman presented with a 1-year history of right-sided epiphora. On initial consultation, the patient had a high right tear meniscus height. CT images revealed bilateral soft tissue opacification in the nasal cavity and maxillary, frontal, and ethmoid sinuses. The lesion in the right nasal cavity and maxillary sinus involved the right lacrimal sac and nasolacrimal duct. Blood test results showed elevated eosinophil count. Endoscopic sinus surgery and excisional biopsy of the nasolacrimal duct were performed. Histopathological examinations of the excised right nasolacrimal

Maxillary sinus mucoceles and other side effects of external-beam radiation in the pediatric patient: A cautionary tale.

A sinonasal mucocele can develop after the obstruction of a sinus ostium, which can occur secondary to trauma, infection, a neoplasm, or an iatrogenic cause. These mucoceles typically arise in the frontal and ethmoid sinuses. Sinonasal mucoceles are epithelium-lined cysts that can gradually expand to fill a paranasal sinus. They can present with ophthalmologic, rhinologic, and neurologic symptoms. There are reports in the literature of sinonasal mucocele development after radiation for nasopharyngeal carcinoma. However, these cases are rare; when they do occur, they tend to arise in the spheno

Endoscopic treatment of benign tumors of the nose and paranasal sinuses: a report of 33 cases.

BACKGROUND: The endoscopic approach can be used successfully for the treatment of benign tumors such as fibroosseous and vascular lesions, pleomorphic adenoma, glioma, meningioma, and schwannoma. METHODS: Thirty-three patients diagnosed with benign tumors of the nasal cavity and paranasal sinuses and treated using an endoscopic approach were reviewed retrospectively. The endoscopic approach was simple in 28 cases and associated with an external approach in 5 cases (because of an intracranial extension of the tumor in four patients and its location at the level of the anterior wall of the fron

Malignant tumours of the nasal cavity and the paranasal sinuses in a Melanesian population.

Malignant tumours of the nasal cavity and paranasal sinuses are usually associated with poor prognosis. From 1986 to 1995, 50 such tumours were managed in Papua New Guinea. Twenty-nine of these arose in the maxillary sinus, 15 in the nasal cavity, four in the ethmoid sinus and two in the frontal sinus. Males were more frequently affected than females. No specific carcinogenic factor was apparent in these patients who were uniformly distributed all over the country. Advanced local disease with multiple symptoms and signs was common during presentation. Systemic and neck node metastases were inf

Further clinical delineation and increased morbidity in males with osteopathia striata with cranial sclerosis: an X-linked disorder?

Osteopathia striata with cranial sclerosis (OS-CS) is a bone dysplasia characterized by hypertelorism, macrocephaly, frontal bossing, broad nasal bridge, palate anomalies, hearing deficits, and mental retardation. The radiographic findings include cranial sclerosis, linear striations in the long bones and iliac wings, small poorly aerated sinuses, scoliosis, and increased bone density. The sensory deficits are disabling, but the condition generally is not life threatening. We describe 4 brothers with the characteristics of OS-CS, 3 of whom have died from more serious complications of the disor
